None need apply who does not possess qualifications &  practica l experienc e in mines as prescribed under Regulat ion 15, 16 (amended on 5 th November, 2001) & 17 of the Metalliferous Mines Regulations, 1961 read with relevant bye-laws and who does not hold valid First Aid, Mate’s and Gas-testing certificate unless exempted otherwise. The validity of First Aid certificate should extend to a date, which is not earlier than the date of the examination. No provisional certificate shall be accepted . Mate Certificate with relevant experience as  prescribed.  Note: Candidates are requir ed to mention the scheme and name and number of paper(s) he wishes to appear in. Application forms should specifically be obtained from the office of THE DIRECTOR OF MINES SAFETY (EXAM) & SECRETARY, BOARD OF MINING EXAMINATIONS, DHANBAD by sending a self-addressed envelope (size 10 cm x 22 cm) or can be downloaded from the Website www.dgms.gov.in. Every application form should be accompanied by: (i) The prescribed form duly filled in with recent attested passport size photographs affixed at the space provided therein. (ii) DULY ATTESTED PHOTOCOPIES OF ALL CERTIFICATES SHALL BE SUBMITTED. HOWEVER, MEDICAL, CHARACTER AND PRACTICAL EXPERIENCE, WHICH SHALL BE SUBMITTED IN ORIGINAL ONLY. Other original Certificates/documents shall be required only at the time of ORAL EXAMINATIONS. (iii) Two self-addressed stamped (Rs.25/- each) envelopes of size 22 cm x 10 cm enclosing four additional  passport size attested photographs. THE AP PLICATIONS WILL BE RECEIVED BY REGISTERED POST ONLY AND NOT BY HAND/ PERSONAL DELIVERY. Applications, which are not IN PRESCRIBED FORM, not properly filled in, and are not accompanied by the required documents, shall stand rejected. All old and new candidates must submit all required documents along with the application to avoid rejection. NO DOCUMENT SHALL BE ACCEPTED LATER.
