Welcome to beautiful, sunny San Diego, known as “America’s finest city.” It’s situated at the southern
tip of SoCal—the local name for Southern California. With 263 days of sun, the weather is perfect
almost all the time. Be prepared for the UV rays with hats, sunglasses and sunscreen!
It is a city of microclimates, so plan ahead if you wander far from downtown. Despite all that sun, the
coastal daytime temperature is mild, typically in the mid-70s, but it cools off into the 60s at night—so
don’t forget a jacket. We hope you get to catch one of our spectacular sunsets or even a rare “green
flash.” The green flash only occurs under the right conditions as the sun disappears over the horizon.
It is a green spot that is visible at the solar apex, but only for a second or two. For the nerds among
us, the flash is caused when the atmosphere refracts the sunlight into different colors. Although blue
light is the most refracted, it is scattered out of our line of sight and what is left is the famous green
flash.

PRESIDENTIAL ADDRESSASTRO President Brian D. Kavanagh on “Effort’s Bounty and Other Imaginings of Past and Future”I n yesterday’s Presidential Address, AS-TRO President Brian D. Kavanagh, MD, MPH, FASTRO, covered a lot of ground. There was poetry, humor—even an ostrich cameo. We sat down with him to discuss some of the key points of his speech.
ASTRO Daily News: Dr. Kavanagh, in your address, you used one patient’s story as a framework to begin and end your pre-sentation. Why did you pick her story? Brian Kavanagh: I suspect that all of us can remember a handful of patients who left an indelible, lasting impression on us for one reason or another. Dee-Anna was one of those patients for me. I was at an early point in my career, so I was probably more impressionable than I might be now. Also, she was about my age, or just a little bit older, and so there is the element of wondering about how easily by chance our roles could have been reversed. I am sure that I would not have processed the prospect of a terminal illness with half the grace and good hu-mor that she showed throughout her entire journey. So she really taught me a lot in that regard.
You said at the end of the talk that you were not completely sure why she gave you her poems or what she was hoping for if you ever read any of them to an audience. Could you expand on that?Although I do think I came to know her as well or better than any of my patients, there was still a big distance between the worlds we each lived in. I also know that patients can some-
times project an image when they go to the clinic to meet with their doctors that is quite different from how they are at home. I actually learned that lesson well when I had a family member from out of town stay with me during his course of radiation treatments. I saw him at home every day, and he was really hav-ing quite a rough time of it. But he didn’t want to disappoint his doctors or the rest of the team at the hospital, and so he would drag himself off the couch, put on a nice shirt and a smile on his face, and do everything he could to convince his caregivers that he was doing just great, when I knew otherwise.
The Latest in Clinical Trials OutcomesBy Kerri Fitzgerald
D uring the Clinical Trials Session on Sunday at ASTRO’s 59th Annual Meeting in San Diego, researchers shared the latest outcomes in eight clini-cal trials in the field of radiation oncology.The double-blind, multicenter, place-bo-controlled, phase III PACIFIC study showed that median progression-free survival (PFS; co-primary endpoint) was significantly longer with durvalumab compared with placebo in patients with locally advanced, unresectable non-small cell lung cancer. “We still want to see the overall survival (OS) data, but a takeaway is that future studies should consider bringing the PD-L1 in sooner to the treatment, either in the concurrent or perhaps neoadjuvant and adjuvant set-ting,” said Marka Crittenden, MD, PhD, who commented on the trial’s findings.A phase II, five-arm study looked at different treatment orders (immunother-apy first vs. radiation first or both started together), dose levels and location of ra-diotherapy (RT; lung vs. liver) and found that combinations of ipilimumab and ste-reotactic ablative radiation therapy have acceptable toxicity profiles for patients with advanced malignancies. “The lung lesions did better,” said Dr. Crittenden, noting that at 12 months, PFS and OS were better for the sequential lung group than for the sequential liver group. A prospective, multicenter, phase II trial evaluated late gastrointestinal and genitourinary adverse events (AEs) after salvage low dose rate prostate brachyther-apy and found that 12 patients (14%) experienced late grade 3 GI/GU AEs, the vast majority of which were urinary; there were no treatment-related grade 4 or 5 AEs. Higher dose predicted both occur-rence of late AE and time to occurrence. No pretreatment variable predicted late AEs, including prior external beam radio-therapy dose and elapsed interval.A prospective study found that mid-treatment circulating tumor DNA (ctDNA) levels during chemotherapy/RT predict treatment outcomes in patients with localized lung cancer. The re-searchers used CAPP-Seq (CAncer Per-sonalized Profiling by deep Sequencing) and found that patients with detectable

The ASTRO Annual Meetings are each like a separate
chapter in our field’s ongoing narrative. Some storylines
advance, some familiar faces reappear—but a few plot
twists also spring up, and some new characters emerge. All of
us together are writing the story, and on one level I view my
role in the process as this year’s editor-in-chief.
Last year, David C. Beyer, MD, FASTRO, called attention to
our place in the evolving landscape of health policy across the
entire house of medicine, a theme he was especially well-qual-
ified to craft given his deep fund of knowledge and expertise
in this area. For 2018 (spoiler alert!), I anticipate that a physi-
cian-scientist extraordinaire like Paul Harari, MD, FASTRO, will
capitalize on his bench-to-bedside brilliance to showcase cut-
ting-edge innovations that have the potential to transform our
methods of cancer treatment. Thus, the question for a dilettant-
ish, jack-of-all-trades, master-of-none charlatan like me was this:
What is the place in between those two places? What should we
talk about right now? Where in the storyline are we?
The advantage to being anxious, insecure and prone to
bouts of existential terror is that I think I know anomie when I
see it. My growing sense was that, with so much potential for
upheaval in the structure of medical care delivery and the rela-
tionships among stakeholders (patients, providers and payers),
it would be timely if we re-examined some core principles and
refreshed our outlook on how we do things, what we like about
our specialty and what we should think about in the future.
My hope is that you leave this Annual Meeting with these
goals accomplished. To help you fulfill this, I’ve created “crib
notes” of a sort to guide you through my chapter in our special-
ty’s tome. With that in mind, here are the top five things not to
miss at this year’s Annual Meeting, in no particular order.
1. Presidential Symposium.
This event will capture the essence of the meeting’s overall
theme, “The Healing Art and Science of Radiation Oncology.” We
will try to generate excitement for the creative sparks that are
moving the field toward new paradigms in translational research
and clinical practice in the first session, “The Scientific State
of the Art.” Next, in “The Art of Quality,” we start a conversa-
tion about how we should identify hallmarks of quality care in
radiation oncology and track them—a topic that is
crucially important and sounds simple but is much
trickier to wrap our heads around than it seems.
Finally, we arrive at “The Quality of Mercy,” where
we will look into the soul of our field. We’ll also
have a little fun along the way with cameo appear-
ances from some famous radiation oncologists of
artistic renown. Read more about the Presidential

2. ASTRO Connect booths.

The International Journal of Radiation Oncology•Biology•Physics, known as the Red Journal, offers authoritative articles linking new research and technologies to clinical applications. Original contributions by leading scientists and researchers include but are not limited to experimental studies of combined modality treatment, tumor sensitization and normal tissue protection, molecular radiation biology, particle irradiation, brachytherapy, treatment planning, tumor biology and clinical investigations of cancer treatment that include radiation therapy. Practical Radiation Oncology (PRO)
Practical Radiation Oncology (PRO) is a bi-monthly journal with the mission to improve the quality of radiation oncology practice. This journal is a natural complement to ASTRO’s clinical research journal, the International Journal of Radiation Oncology • Biology • Physics, and aims to be what MEDLINE defines as a clinical or practice journal, a journal that documents the state of current practice, providing background for those in training or the continuing education of practitioners. PRO is now accepted in MEDLINE.
Advances in Radiation Oncology
Advances in Radiation Oncology is a wide-ranging open access journal publishing both full-length articles and brief reports. As the sister publication to the International Journal of Radiation Oncology∙Biology∙Physics and Practical Radiation Oncology, Advances aims to complement the research in those two journals. Advances is a gold open access journal, so authors, readers and patients have free access to all articles. All work published by this journal is peer reviewed by the editorial board and expert reviewers in radiation oncology.
