Provider Reviews/Revises MLT
The provider reviews and edits MLT.
Provider Saves Revised Medication List
The finalized med list updates the Patient Summary (PSS).MLT MUST be completed and saved to Pt. Summary for reconciled meds to be viewable in Discharge Medication Reconciliation (DMR) during Discharge Process.

Effective July 15th
Providers begin using MLT to document home medication list.
Search for MLT in HEOSelect from Actions Menu
Ways to Access the Med List Tool
Review/ Edit medication list Click “Send to Pt. Summary” when
finished• Select No Current Medications if the patient does
not have any home medications. This button will only be available when the scratch pad has no meds listed or all meds have been deleted.
• Select Unable to Obtain and denote the reason that obtaining a medication history is not possible.
Note Status & Date/time, Click “Edit” (must click edit to open MLT, even if list is correct,
in order to save list to patient summary)
STATUS:Patient Summary Medication List as of date/time- MLT has not been initiated this admission/ encounter. In Progress – MLT has been opened but not submittedObtained by – a nurse has submitted the MLT for provider reviewFinalized by – a provider has reviewed and completed the MLTUnable to Obtain/Reason – an unsuccessful attempt was made to collect the medication list

Left column pulls previous VUMC medication list from Patient
Summary (PSS)– such as from clinic visit, last hospitalization.
NON EDITABLE
Right column is an EDITABLE version of the VUMC List.EDIT list to reflect medications patient is currently taking at home. If nurse has collected medication
history , the edits are displayed. Provider reviews, edits prn and selects “Send to Pt
Summary”
The Updated Med List Displays:
• Med List Tool with finalized status• Medication section of H&P ( if MLT updated
from within H&P) • Pt Summary• All Docs as a PDF
