About This Book let
Getting the health care you need can be hard. Especially if language makes it difficult to find what you are looking for.
In this booklet, you will find information about useful services in the Côte-des-Neiges, Notre-Dame-de-Grâce and LaSalle areas and how to get them.
In partnership with ACDPN and the Côt e-des-Neiges Black Com m unit y Associat ion , we have created this tool to make health care easier to use.

This project finds active senior volunteers to offer
support to socially isolated English-speaking seniors
in the Black community. They will connect seniors
with programs and services that respond to their

Do you wish to make changes because of diabetes, hypertension, excessive weight or other health conditions?
CLSC Côte-des-Neiges offers educational programs. The programs are intended to help people understand and manage their disease.
A nutritionist is also available for people who would like to adjust their diet to suit their health problems.
Also, you can visit your local CLSC for consultation with a dietitian.

Seniors Program
The Seniors Program offers many social activities such
as workshops, games, picnics, music, arts and crafts,
and lunches.
Fit ness Class
Designed to build muscle mass, strength and flexibility, improve heart and lung functions, burn calories, etc.

They are there to listen and to provide information, support and referrals.
