DrugTesting - Must be paid before class begins, out of pocket. Urine drug screens are conducted afterclasses begin. Should the urine drug test be positive, the student may be removed from the program.

Readmission will be handled on a case-by-case basis.

Additional Requirements -Before acceptance into theVocational Nursing program, applicants willberequired to complete a tuberculosis screening test. Students are also required to providedocumentation of having medical insurance throughout the program.

6, Texas Board of Nursing Background Check -In orderto be considered as a q ualified applicant for a

nursing program, theTexas Board of Nursing requiresallprospective studentsto befingerprinted

and undergoa criminal background check (CBC), Fingerprints are analyzed by the Texas Department

of Public Safety (DPS) and the Federal Bureau of Investigation (FBI) in accordance with BON

standardsforcriminal history,includingfeloniesandmisdemeanors,ResultsoftheCBCmustbeavailable to the nursing program before an applicant can be accepted.

All applicants mustcomplete aTexas Board of Nursing Background Checklnformation Form and

submit it to the Health Sciences Division office. The applicant's information will be forwarded to the

(BON). Uponreceivingverificationthattheapplicanthasbeenaddedtothefingerprintlist,instructions will be e-mailed forthe procedure for obtaining fingerprints.Applicants must carefully

follow the instructions and cannot initiate the process on their own.Applicants are responsible for all

associatedfeespertainingtotheCBCprocess,Currentlythecostis$39,75. PleaseNotethattheBackground check processfakes at ieast 3O daysto complete,Afterthe results of the CBC are received and reviewed bythe BON, the applicant is notified of the

findings. If the applicant has a clearbackground check, he/she will receive by maila "Blue Card"

whichstatestheBONhasreceivedthebackgroundcheckandtheapplicant'sfileisclear, Acopy

of the blue card (both sides) must be included in the applicant's application packet. Prospective

applicants with a positive CBC will receive an "Outcome Letter" by mail. One type of outcome letter

comes from the BON Department of Operations. This letter states that there was criminal activity

notedonthebackgroundcheck,buttheBONhasclearedtheindividualandtheyare notrequiredtodoanythingfurther,Thisletterissentwhenthereareminoroffensesthatare reviewedand

clearedbyBONofficestaff, Acopyofthislettermustbeincludedintheapplicant's applicationpacket.Theapplicantshouldkeepthisletterinasafeplacebecauseacopymustbe attachedtothe application for licensure at the end of the program.

Another type of "Outcome Letter" is one that directs the applicant to pay a fee and submit a Petitionfor DeclaratoryOrder. The vocationalnursing program willhelp the applicant understand whatthisprocessentails.ThislettercomesfromtheEnforcementOfficeattheBON. ThedeclaratoryorderprocesscontakefromsixmonthstoayearbeforeadecisionismadebytheBON. lfthedeclaratoryorder is granted, the applicant will receive an "Outcome Letter". A copy of the letter should be

included in the applicant's application packet. lf the declaratory order is denied by the BON, theapplicant cannot be admitted into the vocational nursing program.

7, lmmunizations-Allapplicantsmustshowdocumentation(shotrecordsortiters)thatthefollowingimmunizations have been completed:

I Mumps-Measles-Rubella (MMR, 2 doses or titer)

' Hepatitis B (3 doses or titer)0 Varicella (Chickenpox) (2 doses or titer)

' Tetanus Diphtheria Pertussis (Tdap) (one dose within the past 10 years). Students who are 22 years old or younger will be required to have a Bacterial Meningitis

vaccine as part of the college/state requirements. This dose must have been taken within thepast 5 years. Exemptions can be filed with Enrollment Services forthe Bacterial Meningitisvaccine.

8, lnternational Students-Applicants who did not graduate from high school in the United States

9. Cardiopulmonary Resuscitation (CPR)Certification -Applicants must be certified in American Hea rtAssociation Health Care ProviderorBasic LifeSupportcardiopulmonary resuscitation (CPR). The CPR

certification mustbecurrentthrough the applicant's anticipated graduation date. Online courses are

not accepted. CPR courses are offered once a month by the Midland College Health Sciences

Continuing Education (HSCE). ln ordertoenroll forthe class, contact Kimberly Daw in the HSCE

office located attheAdrrancedTechnologyCenter,3200WestCuthbert. Thecostof the course is $59.00.
