Kaiser Foundation Health Plan. Dr. Southam is a seniorexecutive at Kaiser Foundation Health Plan, and a trendsetterin providing cost effective high quality care. His presentation isentitled “How Kaiser Permanente Implements Accountable CareUnder Health Care Reform”. Kaiser is unique in being both aplan and provider, but much can be learned about how they areoperating during this rollout of healthcare reform.

The Health Information Technology for Economic and ClinicalHealth Act (known by its clever acronym,“HITECH”) wasintroduced as part of the American Recovery and ReinvestmentAct (ARRA) of 1999. The legislation was created to both stimulatethe adoption of electronic health records in the United Statesand to require “new and improved”privacy safeguards forprotected health information.

In addition to the goal of stimulating the American health carecommunity’s transition from paper medical records to electronicmedical records, the HITECH Act incorporated several amend-ments to the Health Insurance Portability and AccountabilityAct (HIPAA), the be-all-end-all source of regulations regardingthe protection of confidential health information. Due to ananticipated universal increase in the transmission of electronicprotected health information (ePHI), the HITECH Act soughtto expand the scope of privacy and security protections availableunder HIPAA, specifically as it relates to ePHI. Some of the

notable elements of the HITECH Act that all covered entitiesmust be aware of are (a) breach notification requirements,(b) privacy and security obligations for business associates ofcovered entities, including civil and criminal penalties for non-compliance therewith, (c) penalties – both civil and criminal –associated with non-compliance, and (d) establishment of greaterenforcement mechanisms, such as mandatory periodic auditsby the Department of Health and Human Services (HHS).

While the HIPAA regulations may have once been viewed assparingly enforced, times have certainly changed. In November2011, as required by the HITECH Act, the Office of Civil Rights(OCR) commenced in-depth audits of 150 covered entities toreview compliance with the privacy and security provisions ofHIPAA. If your organization was not among the lucky 150organizations selected as part of the OCR audit roll-out, it is notyet time to breathe a sigh of relief. More audits are on the way,and every covered entity needs to make sure that its complianceplan and associated privacy and security policies are adequate,complete, and, most importantly, being complied with by everysector of the organization.

Under the audit program, health care providers will be requiredto provide documentation within ten days after receiving notice.In addition, key personnel must be prepared to answer inter-view questions during the“on-site”portion of the investigation.Once the audit report is prepared, the provider will have onlyten days to provide comments before the report becomes final.Although this is a trial program, the program is likely to beexpanded in 2013. With careful advance preparation, you willbe ready when the auditors come calling.

The spring educational outreach sessions for Orange Countyand the Inland Empire were recently held on April 20 and 27.Curtis Degenfelder and Kim Harrison provided presentationsthat included updates on the legislative changes and bestpractices for designing FQHC collaborations. Kim also provideda presentation on how to develop strategies to address theIRS-required Community Health Needs Assessment for501(c)(3) providers. The Educational Outreach Committeewould like to extend a special“Thank you”to Tim De Cou forproviding a valuable roundtable discussion session on ACOsat the Orange County session.

Several upcoming opportunities for “Lunch and Learn”webinarsessions are scheduled for the coming months. On May 17, a“Lunch and Learn”session will be provided on the topic of“Medicare Compliance Challenges in the Age of HealthcareAccountability”from John Zelem of Executive Health Resources.John will be discussing the latest challenges that providers facefrom Medicare’s healthcare audit accountability. This sessionwill include a review of the Medicare regulatory groups andtheir roles, as well as explain how to identify an organization’sgreatest threats and opportunities. FQHC and CommunityHealth Needs AssessmentPresented at SpringOutreach SessionsJonathan Perry _________________________

The recent HFMA Southern CaliforniaEducational Program held in downtown LosAnge les was chock fu l l o f l ea rn ingopportunities, including three sessions fromthe Government Programs committee. Themorning presentation was given by Dale Bakerof Baker Healthcare Consulting. Dale providedan update on the Medicare IPPS Wage Indexincluding the current changes and somepossible future changes. MedPAC has proposedthat the future wage index be based on BLS(Bureau of Labor Statistics) data, which, Dalepointed out, has many problems. This proposalis opposed by the American Hos-pital Association, the Federationof American Hospitals and bymost State and Regional hospitalassociations. Dale has been travel-ing around the country proactivelysuggesting a solution and hopingthe hospital industry will supporthim. His proposal is outlined in hisslide deck which is available onthe HFMA SoCal website.

The second session, immediatelyfollowing lunch, was a MACupdate. Marty Lothes, Director orProvider Audit for J1, returned for another session since he was inundated with questions inNovember regarding the new guidelines for producing cross-over bad debt logs. Another hot topic was the recently publishedSSI ratios and how the MAC will implement them. Unfortunately,instructions had not been issued to the MAC at the time ofthis update, but Marty anticipates they will be available soon.He thinks CMS will most likely instruct the MACs to settle oneyear of cost reports at a time, quarterly, over a one-year period.For the four years of SSI data that were issued, there areapproximately 827 cost reports impacted. Marty also respondedto member submitted questions. The questions and answerscan be found at the end of the presentation which is availableon the HFMA SoCal website.

The last session of the daywas titled“Medicare Cross-over Bad Debt – Change inSOC Methodology, plusUpdated SSI Ratio.”KevinPascoe, of Gong NashedPascoe, Inc. and Scott Ujita,of Toyon Associates, Inc., whohave many years of dealingwith crossover bad debtsbetween them, providedsome tips on how to dealwith the new requirement ofdetermining actual Medi-CalShare of Cost, (“SOC”) for

the crossover bad debt logs. This new requirement was setforth in a letter to providers dated January 27, 2012.The letterstated that the 2% SOC estimate that has been used inCalifornia for over a decade is no longer acceptable andgoing forward, providers must determine the actual SOC.This new requirement applies to all unaudited logs so youmay be asked to update a log already submitted if it hasn’tbeen reviewed. For future logs and audits, the speakerssuggest printing and keeping the POS information for allMedi-Cal patients. Also, be sure to keep notes in the patient file that state whether or not there is a SOC so that someonereviewing the files years after the fact doesn’t have to guess.

On Thursday, April 26th, HFMA membersand guests stepped aboard the historicQueen Mary for the Southern CaliforniaChapter Annual Awards Dinner and BoardInstallation. As we boarded this legendaryocean liner, now retired in Long Beach,guests gathered in a beautiful ballroomwhere you could feel the history dating back to the 1930s.

This was a special event not only to honor the many volunteers of HFMA, but also a fun evening reception to reconnect withfriends and colleagues, and the opportunity to meet new ones, while listening to thecool sounds of Matthew Sikora and theHarvestmen band.

ideas and helped me understand some of the basics that I wasmissing.The mission of HFMA, to provide unity, leadership and,especially, education will be served in great fashion this year. Iwould love to see all of you at the HFMA planning meeting onJune 1st so you can get some of the enthusiasm to volunteer.

Kiet Lam – Voting Board MemberAs a new board member, and having never previously attendedLTC, I was a bit nervous, not knowing what to expect. Uponarriving at the beautiful Harbor Beach Marriott in Fort Lauderdale,I was taken aback by the sheer number of HFMA attendees.The lobby and hotel restaurant were buzzing with energy andchatter; everyone laughing and connecting. It was the perfectsetting for what turned out to be an unforgettable conference.What I valued the most from this experience was the opportunityto meet and share ideas with so many motivated and talentedHFMA leaders from across the country. I learned a number ofcreative ideas to enhance educational programming and I lookforward to incorporating them in the upcoming year. I also trulyenjoyed the opportunity to team build with my fellow SouthernCalifornia chapter directors.Thank you to the Executive Board,and especially Scott Ujita, for giving me this wonderful opportunity!

Kristina Cabral- Membership Membership is a vital role and the foundation of our organization.As the incoming Membership Chair, I came to LTC with an openmind and an eagerness to learn. I have been involved withHFMA for a few years, but thought I would kick things off byattending the HFMA 101 course for new leaders that broughtthings back to basics. Each day there were fantastic breakoutsessions which covered membership recruitment throughmembership retention.The most valuable ideas I will take awaywill be from my fellow peers across the country. With knowledgeof our membership resources and new concepts, the MembershipCommittee will strive for continued growth for our Chapter.

The Leadership Training Conference (LTC) was held in beautifulFort Lauderdale.This year, the chapter leaders selected tenmembers to represent the Southern California Chapter; they are,left to right above, Kiet Lam,Victoria Morgan, Jack Gilbertson,Sam King, Tim Banta, Kathy Hammack, Scott Ujita, KristinaCabral, David Blunt and Jim Moynihan. Below they share someof their experiences from this year’s LTC.

Scott Ujita – PresidentI enjoyed LTC because it helped me to plan the upcoming yearas President of our chapter.The Region 11 planning time isalways an important time to get together and learn the latestchanges within the HFMA organization.The President’s break-out sessions taught me many skills that will be needed for theupcoming year.The National Chair’s theme,“Leadership Matters”,will help me focus this year on transitioning our members intofuture leadership positions within our chapter.

Kathy Hammack – Vice PresidentThe LTC Program Planning track was filled with suggestionsand tools for planning and executing meaningful educationalevents. Representatives from HFMA National shared successstories both from National and other chapters. We had theopportunity to sit in groups with HFMA program chairs fromother states such as Michigan, New York, Ohio, North Carolinaand Maryland. Each group was able to share what has workedin their chapters and the problems encountered along the way. We brought back several ideas that we hope to includenext year to bring more value to our program attendees and to our membership.

2012-2013Southern California Corporate SponsorshipProgramThe Corporate sponsorship program is now inits third and most successful year. We currentlyhave fifteen corporate sponsors at a variety ofsponsorship levels. It is my belief that corporatesponsors truly value the unique position ofbeing a corporate sponsor. As a corporatesponsor they are recognized at each of our threechapter education sessions. That recognitiontakes three forms: 1)All employees of the spon-soring firm wear name tags that identify themas a sponsor, 2) The names of all corporatesponsors are posted prevalently in theregistration lobby, 3)All sponsors are featuredin a power point presentation that highlightsthem and upcoming chapter events.

In addition to the increased visibility at chapterevents, Corporate Sponsors have links to theircorporate website posted on the SoCal chapterwebsite. Our chapter has over one thousandmembers who routinely visit the web-site, inaddition to members of other HFMA chapterswho also visit our website. CORPORATE SPONSOR INFORMATION SHEET

START DATE This Corporate Sponsorship Program begins on June 1, 2012, and runs through May 31, 2013.

DETAILS OF THE PROGRAM Enrollment period will be throughout the chapter year. An email announcement will be sent to all chapter members and vendors listed in the current membership directory. Selected vendors who have expressed an interest in sponsoring past HFMA events will also be contacted.

PAYMENTS Payments are due with the application/agreement and can be submitted at any time during the chapter year. Quoted rates assume a full year’ssponsorship at the various levels. Payments received between June and August will pay 100% of stated donation, Payments received between September and November will pay 75% of stated donation, and those paying between December and February will pay 50% of the stated donation.

A confirmation letter will be mailed after the potential sponsor commits to the agreement. The sponsor will be sent a “Thank You” note once the payment is received. The website will be updated to reflect sponsorship agreement within two weeks of receiving payment.
