Paola’s Sample LetterPlease consider this letter my application for maternity and parental leave. I would like to begin this leave on Monday, March 3rd to Friday, February 28th. The leave would be for a full 52 weeks.

In the event that the baby arrives before the leave commences, I would like to reserve the right to start my leave early. The length of time off would not vary.

Attached is a doctor’s note verifying my pregnancy and expected due date.
