• 63 yo male admitted with cerebellar stroke, who it was later discovered actually had a new diagnosis of HIV and had PML.
• 30 yo male admitted with HUS. • A case of neuro Behçet’s disease • Mesenteric ischemia secondary to massive
mesenteric vein, portal vein, and splenic vein clot.
VA UPDATES
• VA Version 5.0 • Goals:
• Improve patient safety during the afternoon bolus of patients by minimizing holdovers for the OBMT resident
• Improve patient safety from a cross-cover standpoint • Improve educational value of night shift for float interns • Increase attending contact for float interns

• VA Version 5.0 • Implemented March 1
• OBMT resident now arrives at 3PM • If on-call team not capped, OBMT resident works to facilitate their
departure by supervising procedures, etc. • If on-call team has capped, OBMT resident starts admitting
according to the night algorithm • OBMT resident cross-covers on MICU patients and float intern
cross-covers on floor patients (signout still at 6PM) • Float intern now staffs up to 2 admissions with the OBMT resident
and up to 2 with the hospitalist

• 74 y/o M with RA admitted for acute hepatitis (AST 2357 and ALT 3321) 2/2 either methotrexate or rituximab
• 65 y/o M with ACE inhibitor-induced angioedema
• 66 y/o M with recurrent pneumococcal pneumonias with probably common variable immunodeficiency

• Dan finished first round of attending without his senior resident quitting • (though it was close a couple of times)
• New Phones in the MICU working well- they should for 1000$ each. • Residents who have used them, send me your feedback
• Coming soon- IV metoprolol, diltiazem and adenosine on the floor! • Many an ICU transfer will be prevented

• 46 year old female with severe pulmonary HTN and RHF secondary to chronic hypoventilation secondary to Duchenne Muscular Dystrophy • X- linked recessive, she is a know carrier and has
enough penetrance to manifest the disease • 66 year old female with hx of RA and ESLD 2/2
NASH who had UGIB that precipitated myxedema coma- TSH >100- no prior dx of hypothyroidism
• Tuberculous pericardial effusion resulting in tamponade

• 28 y/o male presenting with neutropenia and septic shock found to have hemophagocytic lymphohistiocytosis
• 30 y/o female admitted with bilateral idiopathic PE’s, developed hemopericardium + tamponade requiring pericardiocentesis
• Precursor T-ALL in patient with AIDS • New diagnosis of mycosis fungoides

• Average daily medicine admits: 4.1
• Average overnight admissions: 9.8
• Numbers slightly down from last month (4.3 and 10.2)
