Deadline to register for the hotel is February 12. See page 6 for more details!

Please be sure to fill out and return the registration form to Paul McCall (information on sheet) EVEN IF YOU’RE ONLY ATTENDING FOR YOUR PRESENTATION. This way we can sign you in the day of, and you will have a name badge. The registration form has been emailed to all speakers and is available at NJMCA.org.

PROJECTION COMMITTEE REQUESTS:

The projection committee asks that you have your power point presentation to them by the evening before your presentation time slot, at the very latest. If it is too large to email, please email Eric at [email protected] for an alternative way to upload your presentation. If you need to make any changes to your presentation, you must see Eric before the start of the session that your presentation is in. Please let me know ASAP if you plan on having video with sound in your presentation.

POSTERS:

The poster session is lining up to be another success. If you are submitting a poster, please be advised that you should provide poster handouts for anyone who might request one. You should also be present at your poster at the designated poster session time to answer questions/ discuss your poster. We hope to see you there with your poster in hand. We’ll accept posters up until the morning of the poster session.

SPECIAL EVENT INFO:

As always, the NJMCA holds a Silent Auction (benefits the NJMCA scholarships) and Photo Salon during the banquet. If you are interested in submitting donations for the silent auction, or photos for the photo salon contest: please bring your item, and/or your photos on thumb drive and submit to the registration desk no later than Thursday 3/3/16 at 12pm.

BELOW are the instructions for paying with a P.O. for the 2016 Annual Meeting. Also to reserve a room, whether paying by P.O. or not, they must give our group code to receive the low room rate. The code is AMOSQ16. Note: the cutoff date to reserve rooms in our block for that rate is February 12th.

Guests to make reservations directly with the hotel Reservation Department by calling 1.800.777.8477 Select Option 3 to be connected with a Reservation Agent. Guest to identify themselves as being with the NJ Mosquito Control Association or group code: AMOSQ16 to receive the special group rate of $66.00 over the dates of Feb 29-March 4, 2016

Inform the agent that you will be paying by purchase order or voucher. You will need a credit card to make the reservation and receive a confirmation number but the card will not be charged.

Please be sure to request from the agent that a written confirmation be emailed to your attention at the time of making the reservation. You will need this as back up for payment for your PO or voucher. This will be your only means of proof of your reservations since the Sales Department, Front Office nor Accounting has the ability to generate invoices.

Guest reservations paid by Purchase Order or Voucher are exempt from the 14% tax but still need to pay the $2.00 Tourism Promotion, $3.00 Occupancy and $5.00 Resort Fees.

Total: $66.00 + $10.00 = $76.00 inclusive per room per night.

The PO or Voucher will be checked for accuracy, signed and mailed back with the Golden Nugget Federal Tax ID and W-9 Form to the originator of the document. It would be helpful to have a contact name, phone number and email address for the originator of the PO/Voucher so that we can give personalized attention during the process.

Payment checks must be received by the Golden Nugget Sales Office no later than Monday, February 22, 2016 for processing. Once we receive the payment check then the appropriate funds will be applied to each reservation that the PO covers.

Cancellation of individual reservations not received by Golden Nugget at least 48 hours in advance of your group’s arrival date will be charged one night’s room revenue. This charge will be applied to the individual’s credit card received at the time of making the reservation
