    In September 2015, AEI opened up it's sec-

    ond day habilitation program Mansfield

    Connections.

    An interdisciplinary team including a Regis-

    tered Nurse, Behavioral Clinician, Speech

    Therapist, and Developmental Specialist

    designs individual-

    ized programs

    addressing the

    skills, knowledge,

    and abilities one

    needs to live as

    independently as

    possible.

    Our goal at Mans-

    field Connections is for individuals to learn

    various life-enhancing skills, participate in

    stimulating activities, achieve greater self-

    esteem, and achieve a strong committed

    presence through an active schedule of

    volunteerism.

    Referrals to the Day Habilitation Program

    are typically made through the Department

    of Developmental Services (DDS) or the

    Massachusetts Rehabilitation Commission

    (MRC).

    Mansfield Connections Day Programs Provide:

    Hours of Operation

    Monday to Friday

    8:30am—2:30pm

    Registered Nursing

    Administration of medications and treatments prescribed by a physician

    Education in hygiene and health concerns

    Coordination of each member’s day habilitation service plan with other health care profession-als

    Coordinated implementation of physician’s or-ders with the member, family, and program staff

    Exercise Groups

    Aerobics

    Bowling

    Dance Class

    Fitness Equipment

    Walking Club
