Changing Lip Shrouds on Loader

A contractor employee was in the process of replacing the lip shrouds on loader #40. He was using a 1” impact to take off the bolts that hold the lip shrouds in place. He was using a hammer to hold the head of the bolts in place, when it slipped out of his hand causing the palm of his left hand to come into contact with the head of the bolt. The head of the bolt was worn to a razor sharp edge and was spinning at a high rate of speed, causing a severe injury to the palm of his hand.

A procedure was developed and reviewed with all personnel involved in this process.Procedure:

If the lip shrouds are to be changed in the shop, the loader bucket is to be supported on the required stands. The loader is to be locked out and wheel chocks are to be in place before any work commences. The new adapter is to be placed on a floor jack and positioned under the head of the bolts that hold the lip shroud on the bucket. The jack is to be raised in order to keep the appropriate pressure on the bolts while the nuts are removed using an air impact. Once all the nuts are removed then the lip shrouds can be lowered to the ground and replaced with a new one.

Note: No workers are to have their hands in the vicinity of the head of the bolts while the impact is being used.

• If the lip shrouds are to be changed in the field , the bucket shall be supported using proper cribbing. The loader must be locked out and wheel chocks put into place before any work commences. Once this is done the head of the bolts shall be welded , in order to keep them in place. Only then can the nuts and the lip shrouds be removed.

– Note: No workers are to have their hands in the vicinity of the head of the bolts while the impact is being used .
