The NPSF Lucian Leape Institute takes its name from the internationally recognized leader of the patient safety movement, Lucian Leape. Dr. Leape is a health policy analyst whose groundbreaking research on patient safety and the quality of care began with the publication of his seminal article "Error in Medicine" in the Journal of the American Medical Association in 1994. He was also a member of the Institute of Medicine's Quality of Care in America Committee, which issued To Err Is Human: Building a Safer Health System in 1999 and Crossing the Quality Chasm in 2001.
Dr. Leape is one of the founders of the National Patient Safety Foundation, the Massachusetts Coalition for the Prevention of Medical Error, and the Harvard Kennedy School Executive Session on Medical Error.

Dr. Cipriano is the 35th president of the American Nurses Association (ANA), the nation’s largest nurses organization rep-resenting the interests of the nation's 3.4 million registered nurses. A distinguished nursing leader, she has held executive positions in health care systems, academia, and national professional organizations. In 2015, she was named one of the “Top 100 People in Healthcare” and one of the “Top 25 Women in Healthcare,” by Modern Healthcare magazine.

The National Patient Safety Foundation‘s Lucian Leape Institute
The National Patient Safety Foundation created the Lucian Leape Institute in 2007 to provide vision and a strategic roadmap for the field of patient safety. Named for internationally known patient safety leader Dr. Lucian Leape, who served as the Institute’s initial chairman and continues to serve as a member, the Institute is charged with identifying new approaches to improving safety in health care at the systems level.
In their initial work, the Institute members outlined five concepts fundamental to making sustainable improvements to the safety of our health system: Transparency, Care Integration, Patient/Consumer Engagement, Restoration of Joy and Meaning in Work, and Medical Education Reform.
Subsequently, the Institute members began their work exploring the concepts in depth and reporting on their findings and recommendations. The series of completed reports has revealed how much the issues overlap and intersect. It is difficult to imagine robust patient and family engagement without greater transparency, for example. Likewise, greater patient and family engagement is essential if we are to work together to improve care integration.
What has become particularly clear is the fact that strong leadership and a culture of safety are essential for lasting improvement in patient safety. But chang-ing culture takes time, and not all leaders know where or how to begin. The move from fee-for-service to value-based payment models is forcing CEOs, and board members, to become much more engaged in quality and safety issues than they have been tradi-tionally. Leaders, directors, and others who hold the power to improve patient and workforce safety need to ask the right questions and demand results.
It is important to note that we have made progress in patient safety. Sys-tem flaws are now much more widely recognized as causes of medical error than they were 20 years ago. Research about human factors, adverse events, and sources of errors has prolifer-ated, with a number of journals now focused specifically on quality and safety. Indeed, the field is now recog-nized as a unique medical discipline, with nearly 1,200 people worldwide holding professional certification in patient safety.
But changing the status quo requires more than research, science, and the development of protocols. It requires—from providers, industry, and all those determined to ensure the delivery of safe care—leadership, commitment, vision, and the will to make the right choices.
Recommendations from LLI command attention and action. Engaging at both the influencer and grassroots levels, this work, as intended, has resulted not only in awareness of critical issues and needed corrective measures, but implementation efforts as well.
We are encouraged by the progress that has been achieved and the groundswell of attention generated by the Institute's activities. It is a time of great change in health care, and we look forward to the Institute’s continued impact, and your support for this essential work, in the years ahead.
