• Copy of passport in COLOUR• Signed, notarized letter of permission to

travel (date in February 11-13th)• Alcohol Form• Medication (copy of rx)• Signed permission forms

Parental Authorization FormYour tour requires that minors under the age of 18 present a Parental Authorization Form at the airport, indicating the traveller has permission to travel with the group. A parent or guardian must complete the form and have it notarized no earlier than 30 days prior to departure. This form can also be downloaded

by logging in to your personalized website at eftours.ca/login and clicking on the ‘Forms and Resources’ tab.

Health Insurance & Info• It is imperative that everyone has health

insurance and coverage for while they are in Europe. This form should already be submitted.

• The deadline for our travellers to purchase EF's recommended All-Inclusive Travel Protection Plan is 12/23/2013

• Roommate information and Special Meals/Allergies to have been submitted as well

• Always travel in groups and watch out for each other
