The Nurse Unit Manager and all of the staff welcome you to our friendly facility, where the consumers are the focus of everything we do.

Location We are located in the center of Swan Hill close to the shops, Logan Lodge is attached to hospital which provides 24 hour nursing care staff and access to on call Doctors.

Care Staff The care staff are comprised of Registered Nurses and Enrolled Nurses, most of whom have many years of experience in delivering high quality aged care.

Swan Hill District Health Aged Care We operate under the umbrella of Swan Hill District Health, an arrangement which brings with it a wealth of support services.

Fully Accredited Logan Lodge is fully accredited by the Aged Care Quality and Safety Commission and is committed to the implementation of the Aged Care Quality Standards and the Charter of Aged Care Rights.

Accomodation Options There are 45 single bedrooms in total at Logan Lodge each with their own ensuite providing privacy and dignity.

Aged Care Types Logan Lodge is able to provide both Residential Respite Care and Permanent Residential Care, provided the prospective client has ACAS approval for the type of care they need to access.

Green Thumbs There are extensive garden beds in the court yards throughout the facility building and we are constantly tinkering with them. Consumers are encouraged to get involved in helping our gardens to evolve continuously. There is a strawberry patch and help with maintaining this is always welcomed.

Dedicated visiting Geriatrician Swan Hill District Health Aged Care now has a dedicated Consultant Geriatrician, based in Swan Hill. The Geriatrician provides specialist knowledge above and beyond the services provided by General Practitioners. Medical care of aged people can be quite complex, given that they often have complex medical histories. Having a dedicated Geriatrician available to refer consumers to is a very valuable resource which adds another layer to the quality of care for consumers at Logan Lodge Lodge.

Pre-admission Where ever possible, if time permits, we like to spend time with you and your loved ones before admission, so that we can get to know you and get an idea of your likely care needs. It also provides an opportunity for you and your significant others to ask questions regarding living in an aged care facility, such as Logan Lodge. We can also have a discussion regarding Advanced Care Planning.

Video Conferencing Facilities We are able to connect with distant resources, such as medical specialists, via teleconferencing facilities.

Leisure and Lifestyle Program Our Leisure and Lifestyle Coordinator runs a program with group activities and activities tailored to individual consumers, based on assessments of interests and likes and dislikes of the consumers. Movie Buffs We have a large screen projector in the main lounge room for that “at the movies” feeling. We have a DVD player and an extensive library of DVD titles to choose from. There are also large screen LED TV’s in some of the smaller lounges around the facility so that consumers can watch programs of choice.

Out and about There are also regular bus outings for consumers who like to get out into the community, into the outdoors and who love to get into the countryside.

Library Service

The Swan Hill Rural City Library bus visits Logan Lodge on alternate weeks to provide books, magazines, cassettes and videos, along with larger print books and talking books. A small permanent collection of library books is also available in the small lounges.

Bowling Green Logan Lodge has a small bowling green in the central courtyard with a large umbrella to provide shade on warmer days. Consumers are encouraged join in and play bowls or to watch others enjoying a game.

Meals Meals are cooked in our kitchen by our chefs and served from the servery in Logan Lodge. We aim to provide a well-balanced diet which consumers enjoy and which meets the Dietary Guidelines for Older Australians. The menus are approved by our dieticians. The menus are discussed with consumers. A daily menu is displayed on the noticeboard in the dining area. Meals are supplemented with morning tea, afternoon tea and supper. Consumers are assisted to make menu choices if required.

Special Diets

Logan Lodge caters to all special dietary requirements such as diabetic diet, gluten free, low sodium, etc. and texture modified meals and drinks for consumers with swallowing difficulties. Consumer Preferences We also make it our business to find out individual consumers likes and dislikes, if they choose to share this information with us, so that we can cater to the individual preferences of consumers. The consumers are always able to change their choices and preferences at any time.

Care recipients generally require two assessments to access aged care in services that receive financial assistance via government subsidies: - An Aged Care Assessment Team assessment to determine the needs of the care recipient. - Assets and Income test to determine the level of fees payable.

Consumers can now apply for an ACAT assessment directly via My Aged Care website www.myagedcare.gov.au or by calling the My Aged Care hotline: 1800 200 422 Combined Assets and Income Test (Means) Assessment Used to determine the daily rate of aged care fees payable by a care recipient. It is also used to calculate the amount of government assistance the provider may receive on the care recipient’s behalf.

Costs of Care Consumers pay fees to help with the cost of their care, if they can afford it. An assessment by the Commonwealth Department of Human Services (DHS) determines the cost of residential aged care. The fees vary according to a consumer’s income and assets, level of care, date of entry and whether or not the consumer is a pensioner. DHS review these rates regularly. Costs include: 1. Basic Daily Fee 2. Means Tested Care Fee (if applicable) 3. Accommodation Payments (if applicable)

1. Basic Daily Fee* The Basic Daily Fee is paid by all residents. This fee pays for living expenses such as meals, cleaning & laundry.

The standard consumer contribution is 85% of the single person rate of the basic age pension. It is indexed by the Commonwealth government each March and September with increases to the age pension.

2. Means Tested Care Fee The Means tested care fee is paid by some consumers. The care services you receive are calculated and based on an income and asses assessment.

This is determined on individual circumstances. An annual cap and a lifetime cap applies to this fee.

3. Accommodation Payments Depending on your income and assets you may be asked to pay an accommodation payment. This pays for your accommodation in the home & based on the result of the Centrelink assessment lodged. Accommodation payments are payable from the day of admission. An accommodation payment is payable where a consumer’s means-tested amount is greater than zero. Consumers have the option of paying the accommodation payment as:

1. A lump sum – ‘fully refundable’ referred to as a Refundable Accommodation Deposit (RAD)

2. Rental-type payments referred to as a Daily Accommodation Payment (DAP) 3. A combination of RAD and DAP 4. A combination RAD & DAP with the DAP being deducted from the RAD paid
