    The following information is provided as an overview of the new Control of Hazardous Energy - Lockout/TagoutStandard for Transmission and Distribution.

    The presentation provides key elements of the Control of Hazardous Energy - Lockout/Tagout Standard

    Lockout/Tagout Standard Purpose

    The Control of Hazardous Energy -Lockout/Tagout standard is designed to:

    Protect employees from serious injuries that couldresult from result from the unexpected release of electrical energy while working on isolated andgrounded electrical circuits/equipment and equipmentover 750 volts; and

    Provide employees with an understanding of the formalprocedure required to isolate a circuit from all electricalsources and ensure that the circuit/equipment willremain isolated by locking out and/or tagging sourcesthat can energize the circuit/equipment.

    Regional Responsibilities

    Using the standard, each AES Operation must:

    Develop site specific procedures that, at a minimum,comply with this safety standard and all applicable

    regional regulations regarding the Control of HazardousEnergy Lockout/Tagout ;

    Develop and provide training on the procedures to allaffected employees; and

    Monitor compliance with the procedures through on-going supervision and periodic formal auditing.

    Applications

    This Control of Hazardous Energy -Lockout/Tagout Standard applies to:

    AES employees that work on electrically isolatedtransmission and distribution circuits and equipment. It covers the process involved in ensuring that a circuitis properly isolated, all isolating devices are locked

    and/or tagged, an authorized person signs-off that theisolation is complete and protective grounds areinstalled.

    Affected Employee An employee who works on or near anelectrical circuit that has been trained to understand thehazards involved when working on isolated and groundedcircuits or equipment.

    Authorized Employee An employee who is designated asbeing trained and able to establish an isolation guaranteeon a circuit or equipment using a lockout/ tagoutprocedure.

    Controlling Authority the custodian, system operator,dispatch or organization that has operating control of aspecific transmission system, distribution system or pieceof equipment.

    Definitions of Key Terms

    Holder of the Isolation Guarantee is an authorizedemployee to whom an Isolation Guarantee is issued andresponsible to ensure that the proper switching, lockingand tagging has been carried out before installing

    protective grounds.Isolating Device (electrical) is a device that provides a

    visible open point between the isolated circuit and theenergized circuit.

    Energized Connected to an energy source or containingresidual or stored energy.

    Definitions of Key Terms

    Isolation Guarantee is a formal document (master tag)signed by an authorized employee that a given circuit or equipment is isolated and that the correct isolating devicesare visibly open, locked and/or tagged and that protectivegrounds are applied.(The term isolation guarantee is a more accurate thanterms such as clearance, work permit and mark -upused in various locations.)

    Lockout/Tagout (LOTO) The placement of a lockingdevice along with a Do Not Operate tag (card) on anenergy-isolating device or the placement of a Do NotOperate tag where the device is not lockable.

    Definitions of Key Terms

    Switching Order - A form used to record the sequence of operation of isolating devices, for the isolation and re-energization of a circuit or equipment.

    Tags Plus System - A process that renders an isolation pointinoperable for the protection of the worker. Examples arethe removal of a isolating device component such as afuse holder or riser (lead, jumper or loop).

    Work Authority AES employees or contractor responsiblefor the work and qualified to recognize the need for anisolation guarantee when required to carry out work safely

    Written Program Elements

    The Control of Hazardous Energy Lockout/Tagout Program must have the followingprogram elements:

    The written program must identify the person who isultimately responsible for the implementation,interpretation and maintenance of the T&D ElectricalLockout/Tagout program.

    The program must identify who or which organization is

    the controlling authority, e.g. dispatch, operators,workers, for transmission lines, distribution lines andsubstations.

    The program must have the following forms anddesignations:

    An Isolation Guarantee Application form, designed toprovide the controlling authority the information neededto plan and approve the isolation of a circuit or equipment.

    A Switching Order form that identifies the purpose of theswitching, devices to be operated, the operation (open,

    close, check open), tags or locks placed, the personoperating the isolating device, time of the operation, thepreparer of the switching order and the checker of theswitching order.

    The program must have the following forms anddesignations: contd

    Isolation Guarantee form that is signed by an authorizedemployee when the Isolation guarantee is in effect. (TheIsolation Guarantee is often incorporated into the sameform as the Switching Order or the Do Not Operate tagfor when less than three operations are involved in theisolation.)

    Standard tags (cards) and locks capable of withstandingthe environment and inadvertent or accidental removal.

    The written program must outline the scope, purpose,responsibility, authorization, rules, and techniques to beapplied for the control of hazardous energy and themeasures to enforce compliance with the program,including:

    A specific statement of the intended use of the program

    Identify the controlling authority for each electricalsystem; and

    Application for an Isolation Guarantee - TheLockout/Tagout Program must have a process where thework authority makes an application to the controllingauthority for the isolation of a circuit or equipment. The

    application must meet the following requirements:The application for an Isolation Guarantee for scheduledwork on a circuit or equipment is documented, in advance,so that the controlling authority has the necessary time to

    determine the impact on the system;

    Application for an Isolation Guarantee contd

    The application for an Isolation Guarantee for unscheduled work can be verbal. If the work authority isalso the controlling authority for the circuit or equipment,no application for an Isolation Guarantee is required. If averbal application is used (radio etc.) the message mustbe repeated back for confirmation; and

    The application for an Isolation Guarantee must clearlyidentify the circuit and or equipment to be isolated usingthe nomenclature of the isolation points.

    Preparation of a Switching Order - The Lockout/TagoutProgram must require the use of a Switching Order (Order to Operate or Field Tag) form to identify and document allisolation devices, in the proper sequence, to achieve the

    Isolation Guarantee. When developing switching ordersthe Lockout/Tagout Program must ensure: An operating diagram is consulted to accuratelydetermine the isolation.

    The switching order is signed by an authorizedemployee.

    Preparation of a Switching Order contd When more than three operations are required to achieve theIsolation Guarantee, a second authorized employee mustverify and sign the switching order;

    The switching order identifies what tags and/or locks areplaced on isolation devices; and After each operation, the switching order is initialled by anauthorized employee as being completed.

    Switching/tagging/locking The Lockout/Tagout Programmust require that:When switching, the worker conducting the switchingshould have a copy of the switching order, or when

    instructions from the controlling authority are verbal (radioetc.) the agent must repeat the instructions back to thecontrolling authority for confirmation;Locks and/or tags must be placed on the switchgear

    providing the isolation. If the switchgear cannot be locked,then, if practicable, it should be disabled by removing anelement such as a fuse holder or riser (lead, jumper or loop). Motorized disconnects switch must be disabled;

    Switching/tagging/locking contd The isolating device(s) must be checked to verify itsposition; andIsolating devices used as part of an Isolation Guarantee

    must provide a visible open point, to ensure all deviceshave operated correctly and have not remained closeddue to mechanical failure. If devices such as oil circuitreclosers or sectionalizers are used and the contacts arenot visible, the device feed leads must be removed toensure there is no electricity flow through the isolatingdevice.

    Activation of the Isolation Guarantee - TheLockout/Tagout Procedure must require that:

    The controlling authority signs a formal document (often asection of the Switching Order form or Do Not Operatetag) certifying that the circuit is isolated from all sourcesand will remain isolated until the Isolation Guarantee isformally surrendered by the work authority;The controlling authority notifies an authorized employee

    who has been designated as the holder of the IsolationGuarantee that the Isolation Guarantee is in effect;

    Activation of the Isolation Guarantee - contd The controlling authority will instruct the work authority toinstall protective grounds;Upon completion of the installation of grounds the

    controlling authority shall record the number and locationof the grounds on the operational maps, SCADA systemsor switching orders; andThe authorized employee designated as the holder of theIsolation Guarantee conducts a job briefing, showing theisolation points and discussing the requirement for testingfor electrical potential and grounding with all affectedemployees.

    Releasing Stored Energy - The Lockout/Tagout Proceduremust require:

    That all stored energy is relieved or rendered safe through

    the installation of protective grounds;b. That a potential check is carried out to ensure theabsence of a normal line voltage, before protectivegrounds are installed;

    Releasing Stored Energy contd

    Adherence to the principle If an isolation of a circuit isrequired in order to carry out work safely, then protectivegrounds must be installed. Isolation of a circuit tofacilitate switching is exempt from this principle.

    The specification as to whether the protective grounds arecontrolled by the controlling authority or whether the work

    authority controls the placing of protective line grounds.

    Equipment Testing During Open Work Permit

    The Lockout/Tagout procedure must require that if testinginvolves energizing a circuit with one of the guaranteedlocked/tagged devices, then the isolation guarantee mustbe surrendered. If the testing involves other electricalsources such as a hipot (high voltage potential device,e.g. used to test underground cables, insulators), anapproved work procedure must be followed to ensure

    everyone remains clear of the circuit or equipment duringthe test.

    Surrendering or Releasing the Isolation Guarantee The Lockout/Tagout procedure must require that theholder of the isolation guarantee notifies and confirms tothe controlling authority the following:

    All affected employees are clear of the circuit or equipment;

    All protective grounds are removed; andThe holder is surrendering the isolation guarantee.

    Energizing the Circuit or Equipment - The Lockout/TagoutProgram must require:

    The controlling authority directs the removal of locksand/or tags and switching required to energize the circuitor equipment;

    A Switching Order form be used to identify the switchingdevices and the operating sequence to energize the circuitwhen more that three operations are required; and

    Energizing the Circuit or Equipment contd That in exceptional circumstances, when a circuit needsto be energized and the holder of the Isolation Guaranteeis not available to surrender the isolation guarantee. An

    approved procedure must be followed. The proceduremust define:The approval process needed to proceed withenergizing the circuit.How to ensure that all affected employees are clear and protective grounds are removed; andHow to ensure that the affected employees arenotified of the surrender of the isolation guarantee.
