INTRODUCTION

The Following pages list all active status domestic (United States) Fraternal, HMO, HMDI, Life, LHSO, Property, Title, and Combined groups of companies with their NAIC company and group codes, status, Federal Employer Identification Numbers where available, addresses and phone numbers. Alien insurers are listed with their alien identification numbers and countries. These listings have been divided into the following sections:

Fraternal Alpha, Health Maintenance Organizations (HMO) Alpha, Hospital, Medical and

Dental, Service or Indemnity (HMDI) Corporations Alpha, Life Alpha, Limited Health Services Organization (LHSO) Alpha, Property Alpha, Title Alpha

These sections list all companies and their corresponding company codes in alpha order. They also include domiciliary state, group code, Federal Employer Identification Numbers (FEINs), active company status, addresses, and telephone numbers. These sections will assist you when you know the company name and need to know the corresponding company code. Combined Property Alpha This section lists all combined property groups of companies in alpha order. It also includes group codes and active company status. This section will assist you when you know the combined name and need to know the corresponding company code. Numerical Listing This section lists Fraternal, HMO, HMDI, Life, LHSO, Property, Title, and Combined Property entities in numerical order by company code. This section will assist you when you know the company code and need to know the name of the corresponding entity. Group Alpha This section lists active status groups in alpha order. This section will assist you when you know the name of a group and need to know the corresponding group code. Group Listing Numeric This section lists all members of active status group in numeric order by group code. This section provides access to group information when you know only the group code. Alien Alpha This section lists all active status alien insurance companies and their corresponding identification numbers in alpha order. It also includes the country where the company is domiciled. This section will assist you when you know the company name and need to know the corresponding identification number.

Alien Numeric This section lists all active status alien insurance companies by their corresponding identification numbers. It also includes the country of domicile. This section will assist you when you know the identification number or the country of domicile and need to know the corresponding company name. Pools/Associations Alpha This section lists all active status pools and associations that have been assigned an identification number by the NAIC. This number should be used in place of the Federal Employer Identification Number (FEIN). The listing is sorted alphabetically. If you do not find the reinsurer you are seeking, please contact the NAIC Reinsurance Department at 816-842-3600. Pools/Associations Numeric This section lists all pools and associations that have been assigned an identification number by the NAIC. This number should be used in place of the Federal Employer Identification Number (FEIN). The listing is sorted numerically. If you do not find the reinsurer you are seeking, please contact the NAIC Reinsurance Department at 816-842-3600.
