2. POSSIBLE CONSIDERATIONS

(Tick if relevant and address each point in ‘Step-by-Step’ section below)

Weight not verified   Lifting of chemicals   Conflicting tasks in area

High centre of gravity   Load on pallet requires

securing  Dynamic factors involved

Stability of load   Seafastening removed   Hazards to personnel in the area

Awkward size/shape/sharp edges   Restricted headroom or

confined work area  Communication requirements

No dedicated lifting points on the load   No lifting point directly above

the load  Adequate lighting and visibility

No certified suspension points for the lifting equipment

  Appropriate and correctly installed

Is the use of tag lines required? Then consider personnel positioning

3. ROUTE TO BE TRAVELLED AND LAYDOWN AREA

(If you can’t answer either, address in the ‘Step-by-Step’ section) Yes NA

1) Are the route and laydown area clear of obstructions?    2) Is the laydown/landing area adequate in terms of size and load-bearing ability?    3) Is suitable packing available for protection of the load, lifting equipment, slings etc?    4) Have barriers been positioned to prevent access by unauthorised personnel?    5) Have you confirmed that the laydown area is within the operating limits/radius of the equipment?    6) Have environmental conditions been considered with regards to the safety of the lifting operation?

7) Will the Lifting Equipment Operator be able to see the Banksman throughout the operation, or has another suitable, risk-assessed means of communication been checked and made available (eg radios)?    8) Are there hazardous process plant or materials in the area?    4. STEP-BY-STEP DETAILS OF THE LIFTING OPERATIONDescription Responsible Person LO REF

1. Prepare a lifting plan to include all the key element and approved by the supervisor and PTW approved by OIM

SUPERVISOR

2. Prepare a Job Hazards and Risk assessment for the taskSUPERVISOR

3. Conduct the ToolBox Talk (TBT for all the crew involved). Crane Operator must attend the toolbox meeting

SUPERVISOR/FOREMAN/ TEAMLEADER/ SLINGER

4. Pre use check for the Crane assembly, parts by the crane operator CRANE OPERATOR

5. Perform operational check for the Crane by the Crane Operator (boom angle, length, radius, load chart capacity, limit switch etc,) CRANE OPERATOR

6. Conduct pre use check for 2 pieces of 3 tons wire rope slingsSLINGER

A5 38 A8 3.1.4

7. Examine and determine the weight of the load (1500 kgs)SLINGER

A5 43A8 3.1.4

8. Attached the wire rope sling to the load (pipe) by using double choker hitch at lifting point at approximately 25% of total length from both end

SLINGER /BANKSMAN

A5 43, 44 A8 3.1.5

9. The chocker hitch shall be secured to avoid damage to the wire rope by using a bow shackles

SLINGER

A5 42 A8 3.1.5

10. The load shall be attached with 2 taglines placing either direct to the pipe at both end or at the chocker hitch point by using bowline or clove hitch knots

SLINGER

A5 42 A8 3.1.5

11. Banksman to communicate to Crane Operator informing that the load is ready to be lifted

BANKSMAN

A5 39 A8 2.2.4

12. The Crane Operator shall swing to the direction of the load following the direction of the banksman

CRANE OPERATOR

13. Banksman to communicate by using hand signal to indicate which type of block to be used (Used whipline or auxiliary block)

BANKSMAN

A5 39 A8 2.2.4

14. Banksman to communicate with the Slingers that they are ready for the lift and ensure everyone is at the correct standing position.

BANKSMAN

A5 39 A8 2.2.4

15. Banksman to direct the crane operator to lower down the block by using hand signal (used hoist/block down hand signal)

BANKSMAN

A8 2.2.4

16. Once the block is down at approximately chest level, the banksman shall instruct the Slinger to attach the wire rope slings to the crane hook

SLINGER /BANKSMANA5 42 A8 2.2.4

A8 3.1.6

17. The banksman then will direct the crane to hoist up slowly until the sling is tension. He then must confirm that the sling angle and centre of gravity (CoG)

BANKSMAN

A5 39 A8 2.2.4

18. Once the CoG is confirmed, banksman to instruct the Crane Operator to hoist up the load at the same time 2 Slingers to control the load using the taglines

SLINGER /BANKSMAN

A5 43 , 39A8 2.2.4

19. Banksman to direct the crane by hoist up signal until the load is 4 metres high. He then use ‘boom down and hoist up signal to swing until the load is above point ‘B’

BANKSMAN A5 39 A8 2.2.4
