1. DESCRIPTION OF THE PROJECT

1.1) Project description; The project is consists of the demolition of a recently acquired building in Poulton followed by a new build Lidl Store. Demolition works will include:

• Site set-up of compound, hoarding & signage erection; • Demolition of existing building and associated services (Mechanical Garage and 2x Existing NHS buildings); • Crushing of materials on site and form stock pile • Removal of redundant materials to site landfill

External New Build works include but are not limited to construction of the building extension and all associated works and temporary works:

• Ground works • Block/brick work • Underground drainage including attenuation tank • Drainage connections • Install cold water supplies • Floor slab and construction of a new load bay • Internal / external walls • Roofing, including rainwater goods • Alucobond cladding • Window and door systems • Hard & soft landscaping and car park works • Signage etc.

Internal New Build works include [but are not limited to] the fit-out of the store and all associated works:

• M&E - electrical, HVAC etc. • Flooring • Retail fixtures and fittings • Decoration

Contact: Mr Paul Jones (Project Manager) Mob: 07425 623407 1.3) Extent and location of existing records and plans; Drawings provided for the purpose of this new build consisted of: 8594-P01G Proposed Site Plan 8594-P03C Proposed Elevations Poulton Existing Red Line with Notes 1.4) Restrictions that may affect the works Construction work operations will be carried out within the limits of the site restrictions regarding noise, working hours and environmental constraints are as specified within the construction requirements of the contract. Access / Egress A detailed Traffic Management Safety Plan will be developed prior to arrival on site and issued with the CPH&SP. Access to the site is off Garstang Road East. Rear access to the site is Via Furness Road. Site is bordered by Domestic and commercial properties and pedestrian walkways. Access to commercial properties and pedestrian walkways will be maintained at all times during the works. A separate access gate must be provided for pedestrians entering the site to reduce the risk of collision with site traffic. Designated pedestrian access routes around the site will also be required and must form part of the Principal Contractors Traffic Management Plan. Segregated walkways for pedestrians will be considered on site providing safe access / egress routes from site welfare areas to place of work as far as is reasonably practicable. A separate access gate with a digital keypad will be provided for pedestrians entering the site to reduce the risk of collision with site traffic. Designated pedestrian access routes around the site will also be required and must form part of the Principal Contractors Traffic Management Plan. TC will be allocated a suitable compound area on the site for storage of materials etc. this area must be cordoned off and at a minimum this fencing must have adequate signage and be locked to prevent access to member of the public. TC will be responsible for the erection of the required safety signage – adequate to warn others of the rules and dangers presented by the proposed works. Note: Any work that takes place outside of the site boundary must be fully protected at all times, each individual contractor will be responsible for ensuring the protection is in place before work commences and in maintained for the duration of the works,

Security Arrangements and Perimeter Fencing Works area will be secured by means of total perimeter hoarding. Gate and warning signs will be erected the designated access point. All persons entering site during hours of business at the designated access point will be required to report to the site office. Unauthorised access will be prevented as far as reasonably practicable. Plant will be immobilised by means of removing ignition keys and locking of cabs. The Poulton site should be considered high risk and CCTV will be installed. All materials, tools, plant and fuel should be safely stored away at the end of the day.

All plant should be fully inspected each morning for damage. Existing Services TC referred to service drawings prepared by the design team. In the absence of service drawings the contractor will be required to conduct an underground survey (LUGS) and a standard survey of the buildings to gain the necessary information to ensure safe systems of work can be executed. TC will also not commence works until it receives a letter from LIDL/Power contractor confirming power to site has been isolated. It will also verify this by requesting its own Electrical contractor check the cables. Deliveries and traffic Due to a large school (Hodgson Academy) been located at the end of Moorland rd, it is recommended that any deliverers avoid school start/end times to avoid issues with pedestrians (school children) and congestion. It is also recommended that the site traffic plan and rules are forward to any delivery companies once they have been established. A record of these emails will be kept in the site safety file. Working Hours There are restrictions on working hours due to the close proximity of residential housing. The hours of permitted working on site are; 07:00 to 19:00 Mon to Fri 07:00 to 13:00 Sat No working on Sundays Please also refer to any planning conditions that may be stated within the planning consent from the local council. Asbestos Garage and NHS building An asbestos survey has been carried out on the 14

th

July 2015 by Trident Surveying Limited and it has been confirmed by the survey that the site is contaminated with;

• Asbestos insulating board forming the ceiling and associated debris. • Asbestos cement sheet debris and also forming the border along the driveway.

TC construction has engaged a fully licenced contractor (Malrod) to remove these materials. All RAMS, insurances, Licences, etc will be requested and verified by TC and reviewed by Sandia before

the contractor comes to site. Before any demolition works begin, Malrod will confirm in writing that the areas are clear of asbestos and provide us with relevant clean air certificates at each phase passes. They will also secure any areas while works process, communicating on a daily basis with the site manager providing information on the progress of works and any concerns they may have. If it is a requirement for the site manager to enter any of the restricted areas, they will need to have the suitable PPE and inform Malrod that they are doing so. Asbestos contractor is to be on site for 4/5 weeks. Stage 1 - garage Contractor is to start works in the garage area of the site. They are to seal of area and remove all asbestos from area. Once complete they are to confirm with site manager and provide clean air certificate. Once done, TC operatives are to enter building and carry out soft strip using scaffold were necessary. Once completed, plant will move in to pull down walls and roof. All arising’s transferred using skidsteer loader to relevant waste skip for recycling. Stage 2 – NHS building In week 2, the asbestos constructor is to enter NHS building to start works. They will work from the Garston Road end of the building. The building is to be sealed and only authorized personal may enter. This building is to have a phased approach to the asbestos removal (Asbestos contractor is to provide a detailed phased plan before work start). As each phase of the building is completed, the contractor is to seal of the area and present the site manager with a clean air certificate. TC operatives will then move into this area to carry out soft strip. This will continue until the building is completely free from asbestos and materials. TC operatives will remove all external windows and doors and sort into skips for recycling. This also creates a thru-flow of fresh air for operatives working inside. All areas to be dampened down to avoid dust/asbestos fibres. All operatives on site will be given a tool box talk on the risks of asbestos. Trees There is a tree protection order on the site. 1nr tree Immediately adjacent to the car park entrance. This will require a protective fence placed around it and should not be damaged during works. See site setup plan for location. Oil tank in basement area There is an oil tank located in the basement area of the NHS building. TC will be employing the services of a specialist contractor to empty and make safe the oil tank and provide in writing that this has taken place before demolition. All RAMS from contractor will be checked and forwarded on to Sandia for approval before works start.

600mm water main There is a 600mm water distribution main running through site. Area highlighted below. Movement of vehicles and plant with a total weight exceeding 6 tonnes across the unprotected Pipeline is forbidden. The repetitive movement of vehicles or plant of any weight over the unprotected Pipeline in the same position is forbidden. Where temporary or permanent access is required, the Promoter must consult with united utilities prior to gaining access.

A temporary crossing over the Pipeline will need to be constructed for the duration of the works with the use of goal posts. See file for further information 13552_201 Drainage Layout T5

2. MANAGEMENT OF THE WORK

2.1) Management Structure and responsibilities; The Management structure for this contract is listed below :-

Responsibilities for key staff directly in relation to this project are noted below:-

In addition to this all TC personnel on this project have specific health and safety responsibilities the full list of these are available in the TC Ltd Management Manual this can be provided upon request to the Systems Manager. 2.2) General Statement of Principles and Objectives for the Project; It is the policy of TC Ltd to carry out this project within the agreed timescales to the design requirements and specification agreed. The Company will give the highest priority to the Safety, Health and Welfare of staff, operatives and others who may be affected by the Company’s undertaking within the project. The Company shall comply with current and applicable Safety legislation, and the requirements contained within the Safety Management Systems proposed for the Contract. The aim of the Company is to avoid accidents, injuries, ill health, damage to property, and damage to the environment.

Towards the achievement of our aim, our main goals will be to ensure that:

• Safety duties are clearly defined and accepted. • Hazards are identified, risks assessed, eliminated where practicable, or reduced to an

acceptable level. • Safe Systems of Work are implemented. • Safe and Healthy working conditions are maintained. • Safety is promoted across the project. • The Construction Phase Plan for the contract is reviewed and updated. • Accident Statistics are maintained and corrective / preventive action taken where

required. • The same level of attention is given to near misses as is given to accidents causing injury. • Management and employees look after themselves and others who may be affected by

their acts or omissions at work. • Management and employees co-operate with each other to achieve the Company’s

Health, Safety, and Welfare objectives. • Management and employees are competent to carry out their duties safely. • Adequate environmental monitoring is in place.

TC are dedicated to an accident free site, and the same dedication is required from all personnel working on this Contract. It is the responsibility of all site staff to monitor and report on health and safety issues on a daily basis as part of their normal working procedure. 2.3) Arrangement for monitoring Safety & Health Performance: Audits of the project’s safety management system compliance are conducted by the Systems Manager (Jerome Hamill). In-house safety inspections are carried out on an ad hoc basis the Site Manager and Subcontractor are given no notice as to where or when the inspections will occur. The minimum frequency of audits will be at least 1 per month. All inspection and audit reports are in duplicate. A copy is retained on site with the Site Manager to ensure corrective action is taken whilst a second copy is return to the head office which is issued to the Directors and Project Manager for review and discussion with the Site Manager. External Audits from Construction Employers Federation are conducted annually to ensure our Management System remains accredited to the Safe T Cert standard, it may be possible that this project will be subject to external audit. If, during an inspection/audit any issues highlighted will have a Non-conformance report completed and remedial action programmed for the earliest available date. The NCR Form ensures that the issue is brought to the attention of the Directors, Project Manager & Site Manager who can authorise the agreed action and complete out the non-conformance (note a visual inspection of the corrective action is required). 2.4) Arrangement for communication between parties associated with the works The successful implementation of this Plan depends upon there being adequate co -ordination, co-operation and liaison between the various parties. Good co-operation and co-ordination of work between all of the parties involved in this project is essential if risks are to be identified early and properly controlled. TC will take the lead and actively encourage co-operation and co-ordination

between interested parties, and the local community at an early stage. Good, timely communication is essential for the co-operation and co-ordination of activities. Information about risks and precautions must be shared when it is needed to plan and manage work. The Client (or his representative) shall be invited to review the implementation of the Safety parts of the Plan as an agenda item at their progress meeting which will be held weekly, or twice weekly depending on client’s requirements. The PD shall be invited to agree a programme of formal meetings to review on-going and/or outstanding design matters as they relate to health and safety, as well as the provision of information relevant to the Safety Files. There will be several contractors working on site simultaneously and TC Construction will ensure that all works are carried out in a co-ordinated manner at all times to ensure the safety and health of site staff at all times. Until the site is fully operational, meetings will be held at the following intervals: Weekly Safety Review Meeting with TC Construction Site Management and the foreman from each subcontracting company on site to be held at a time to be decided. These will be chaired by Mr Gerard Tinnelly or his chosen deputy. Weekly meeting with client, PD, CDM Advisors and designers at a time to be decided. At present, there is no additional need for any other meetings but on-going liaison with the surrounding neighbours will be paramount and the company will ensure that the project integrates itself into the local community. 2.5) Consultation with the workforce The following mechanisms will be established for effective ongoing consultation and communication with all operatives:-

• Site induction training where all staff and operatives will be informed of their on-site roles. • Open door policy exists for all operatives and staff employed on site where the individual

can approach the supervisory/management staff on matters relating to health and safety. • Toolbox Talks given by the site supervisory staff to contractors / sub-contractors, by way of

safety bulletins and accident alerts. • Meetings held with the site management team and employees representatives. • Sub-contractor review meetings.

Any person wishing to discuss health and safety issues may at any time approach any member of the project management/ safety management team. Alternatively, representations can be made through the elected Safety Representative or at project meetings. Further, where a Safety Representative is appointed the safety representative is encouraged to represent the workforce and communicate workforce issues with the project team. Scheduled Site Safety meetings will be held when necessary to update or reinforce safety matter.

2.6) The exchange of design information The procedure for the approval of designs is as follows: During the detailed Design and Construction Phase, TC Construction as the PC and their designers shall carry out their duties including ensuring that all designers (including designers working for sub-contractors of TC Construction) fulfil their duties with respect to taking risk into account, providing information and co-operating with other designers, etc. Santia Consulting acting as the CDM Advisors will also be informed of any design changes that may effect the risks. TC Construction and their designers shall liaise with the Project Manager so that appropriate notice is given for them to attend design review meetings, etc, In the event of unforeseen circumstances that may require a design input, TC Construction shall inform the Project Manager, the Designers so that associated risks may be assessed and appropriate avoidance/control measures determined. This procedure applies to all design issues on site and extends to the design inputs of specialist subcontractors on site. Information arising from this continuing liaison shall be entered as appropriate in the Construction Phase Plan as part of its ongoing review and development by TC Construction. 2.7) Handling design changes during the project The PD will be responsible for the co-ordination of design information between relevant parties. Where design work, including temporary works design, is undertaken during the construction phase, the PC will liaise with the PD to ensure that both are satisfied that design risk assessments are being undertaken and, where necessary, information is being passed from designers to the construction team by drawing notes, etc.

Where TC Construction is undertaking the design work, the PC will liaise with the responsible person to ensure that both are satisfied that design risk assessments are being undertaken and, where necessary, information is being passed from designers to the construction team by drawing notes, etc. Strict records of change will be maintained. Santia Consulting acting as the CDM Advisors will also be involved in handling design changes during the project. Santia will be available to provide guidance in respect to H&S issues that may arises in design changes. Where design becomes an issue of concern it will be placed on the relevant project meeting agenda and discussed with all parties concerned. 2.8) The selection and control of contractors The appointment of contractors will be on their ability to safely meet the requirements of the contract, giving due consideration to health, safety, quality, environmental performance, programme, and economics. All contractors will be asked to complete TC 024 SHEQ Questionnaire this will require written evidence of the following:-

• Health, Safety and Welfare Arrangements • Insurance Arrangements • Safety Performance Review • Sample Risk Assessments and Method Statements • Evidence of Competency / Training • Arrangement for Plant and Equipment • Quality Assurance • Environmental Arrangements

Only contractors who can demonstrate their competence will be permitted to work on this contract. The Site Manager will review all of the contractors’ site specific risk assessments and method statements to ensure that all information is relevant to the work undertaken and also implemented on site. During the review any comments / advice given will be communicated via TC 009 Form (Risk Assessment Review Form). It is the responsibility of Site Manager to ensure that suitable and sufficient site specific risk assessments are compiled, communicated to all those affected and enforced. All contractors who intend allocating work to other subcontractors will be required to notify TC before commencing work. The competence of sub-contractors will also be assessed by TC. 2.8) Arrangements for securing co-operation between contractors Good co-operation and co-ordination of work between all of the parties involved in this project is essential if risks are to be identified early on and properly controlled. TC will take the lead and actively encourage co-operation and co-ordination between all parties from an early stage. Good, timely communication is essential for the co-operation and co-ordination of activities, information about risks and precautions must be shared when it is needed to plan and manage the

work. TC will provide all project specific information to all contractors in a timely manner to ensure work can progress safely. 2.9) Site Security TC will take all reasonable steps to prevent access by unauthorised persons to their site. Only people who are explicitly authorised, individually or collectively, by TC will be allowed access. Hoarding will be installed to prevent unauthorised entry around the site boundary. Authorised personnel will have site rules explained to them and undertake any necessary site induction, and should comply with site rules and co-operate with TC. Some visitors may need to be supervised or accompanied whilst on site or visiting specific areas. All work areas will be suitably signed at all access and egress points warning persons of construction activity, such as ‘construction site – keep out’ and ‘visitors/deliveries to report to site office’. HSE Guidance Note HSG 151 ‘Protecting the public – your next move’ will be followed where appropriate. The site management team will be vigilant to children and unauthorised people/vehicles coming onto site. All site entrances will be supervised during working hours. Entry to the site will be controlled by means of signing in / out system to inducted personnel and visitors. Every effort will be made to reduce the impact of the development on its neighbours and the passing public. 2.10) Site Induction Site induction, training and information are vital to securing health and safety on site. TC Ltd will ensure so far as is reasonably practicable, that every visitor and worker has:

• A relevant site safety induction; and • Any further information and training needed for the work they are undertaking i.e.

Site specific inductions will cover:

• Senior management commitment to health and safety; • The outline of the project; • The individual’s immediate line manager and any other key personnel; • Any site-specific health and safety risks, for example in relation to access, transport, site

contamination, hazardous substances and manual handling; • Control measures on the site, including:

any site rules, any permit to work systems, hearing protection zones, arrangements for personal protective equipment, arrangements for

housekeeping and materials storage, facilities available, including welfare facilities. Emergency procedures

• Arrangement for first aid;

• Arrangement for reporting of accidents and other incidents; • Details of any planned training, such as toolbox talks; • Arrangements for consulting and involving workers in safety and health • Information about the individual’s responsibilities for safety and health • That they have been made aware local environmental issues;

Been made aware if issues with asbestos on site. 2.11) Onsite training TC Ltd will continuously review the training needs of all its employees and will proceed to organise training where deemed necessary. Training is reviewed on a regular basis by the Systems Manager and anything identified on routine audits is also implemented as far as reasonably practicable. Individual contractors are responsible for the training of their own employees however TC Ltd will request evidence of training from the contractor before commencing on site to ensure competence of site operatives. The site management team will make suitable arrangements for dealing with site sewage. This will be achieved by means of connecting to the sewer network at the first available opportunity. Within the site compound TC will provide, clean drinking water, warm water for washing, clean well-lit toilets, hand dryers, towels, soaps and Sun Protection barrier creams. A daily cleaned canteen with a fridge, microwave, kettle and refreshments will be available for all operatives. A drying room will be provided to ensure operatives have an area to keep clothes warm and dry. A first aid facility and first aider will always be available on site. The site plan displayed on site will indicate where the first aid boxes are kept. TC Construction Ltd. will supply all the equipment and facilities required for enabling First Aid to be rendered to injured or ill employees. First Aid kits are available in the site office and are under the control of supervisors and trained First Aiders. Employees must seek First Aid treatment for any injury; no matter how slight, they receive at work. 2.13) The reporting and investigation of accidents and incidents including near misses Responsibility for the initial recording and reporting of all injuries, dangerous occurrences, property damage and all related incidents lies with all employees that first note such incidents. All employee injury, damage, dangerous occurrences and `near miss’ incidents are reported by means of the Company’s Internal Accident Report form and forwarded to the Systems Manager. Such reports help the Company prioritise areas for incident investigation, prompt remedial actions if necessary and help monitor compliance with Company procedures. In the event of a fatal or major injury, or dangerous occurrence as defined in the Reporting of

Injuries, Diseases and Dangerous Occurrences Regulations (or equivalent Regulations), the Site Manager must immediately telephone:

• The Emergency Services • The Systems Manager • The Director(s) • Where required the accidents/incidents will be reported via online to RIDDOR

All incidents will be investigated by the Systems Manger to help prevent re-occurrence. Santia and Lidl will be notified immediately should there be any RIDDOR reportable accidents, accidents, near misses, incidents on site as soon as is reasonably practicable after the event. They should also be informed of any HSE visits to site. 2.14) The production and approval of risk assessments and written systems if work Method statements and risk assessments will be drawn up to allow for the initial phase of the works to commence. Hazards/risks identified in the Preliminary Safety & Health Plan and will be considered in their entirety and appropriate control measures put in place. TC Construction site management team or the sub-contractor will produce and approve method statements and risk assessments for all elements of work associated with the project which will be recorded on a register. All RAMS are to be reviewed. Failure to get approval will result in the delayed start of the proposed subcontractor starting on site. The site management team, at the weekly review meetings, will identify the health and safety hazards that are anticipated for each phase of the works programme, and agree safe methods of work to eliminate or control the risks to those persons carrying out the work itself and all other persons who may be affected. Method statements and risk assessments will then be prepared, reviewed, communicated, and issued in good time prior to commencement of work. The site management team will then ensure that all control measures referred to are implemented. The Company recognises that elements of the works may change during the course of the project. Steps will be taken prior to any modification of the agreed works to revise risk assessments and methods statements to ensure all safety implications have been addressed and preferred work methods documented. This in turn will be communicated to the workforce. Subcontractor Method Statements and Risk Assessments identified in the weekly safety review meetings will be drawn up by the relevant subcontractor in good time and issued to TC Construction’s for review. This will have to be signed off as approved/rejected by both parties and details recorded on the method statement and risk assessment register. All other method statements and risk assessments for TC Construction works must be drawn up by the TC Construction Site Manager. All method statements and risk assessments must be signed by the relevant parties/operatives before any of the work in question is to be carried out. Failure to follow this instruction will result in disciplinary action taken against the person who did not follow this requirement.

2.15) Site Rules (including drugs and alcohol policy): The site will display a set of site rules in the form of a poster, the rules are detailed in the list below:-

• All personnel must undergo site induction • All personnel must take a current Safe Pass/ CSR / CSCS card (etc.) with them to induction • All personnel must wear appropriate PPE at all times • Report all accidents and near miss events to TC Construction • Do not interfere with or misuse fixtures, fittings or equipment provided in the interests of

health and safety • Smoking shall only be allowed in designated areas • All visitors must report to TC Construction site office prior to entering site • Lorries are not to be reversed in construction areas without an authorised banksman • Mobile phones are prohibited when operating/driving plant • Safety signs and notices must be followed • Transistor radios or personal stereos/walkmans must not be used • You must understand and follow your method statement • All companies must have a nominated safety person in charge of their operations at all times • Follow the site fire procedures • The consumption of alcohol and/or drugs is prohibited • No person to operate mechanical plant or equipment unless they have been trained and

have been certificates as competent • Any mechanical plant or equipment found to be defective is not to be used • Ladders must be secured at all times during use • Trestles (modern types with guard rails and toe boards) can be used up to a height of 1.2m,

mobile towers or scaffolding with guard rails and toe boards to be used above 1.2m. • Food to be consumed in canteen. Respect canteen and toilet facilities • Only qualified electrician to make alterations to temporary electrical supply. • Only 110V power tools are permitted on site • No fighting, horseplay or practical jokes allowed on site • The wearing of football tops, or display of photographs or other material which could be

perceived to be of a sectarian nature is strictly prohibited. • A speed of 5mph shall be adhered to • All emergency routes/exits are to be kept clear at all times

The Site Management Team, if necessary, will draw up further site specific rules, at the beginning of the contract to ensure the safe completion of the project i.e. Display of offensive material prohibited. 2.16) Fire and Emergency Procedures Clear emergency procedures will be developed and documented by TC Construction which is applicable to this contract, as well as taking account of any existing site procedures, including any emergency procedures specified by the Client. Site specific risk assessments will be produced for each situation encountered and this will determine the exact level of emergency procedures or arrangements required. For the duration of the works, all operatives/persons associated with the works will be informed of an emergency situation either by manually operated alarm bells or shouting, depending on the size

of the site at the time. A map will be displayed showing emergency exit points, muster points, fire extinguishers and first aid box along with an emergency contact notice. Firefighting equipment, CO2 and H2O extinguishers, will be located at strategic areas on-site. At the outset, these will be located in the compound and security areas along with a fire bell at each point. The site will operate as a ‘No Smoking’ site to co-operate with the Smoking laws established in 2007. As the building progresses a CO2 and a H2O extinguisher, a fire bell, a floor drawing with clearly defined escape routes and the procedures to be adopted in the event of an emergency will be located on all stair landings / escape routes on each floor. Relevant signage will be fitted within the corridors pointing the way to the fire escapes. Persons observed tampering with the fire extinguishers or removing them from their set location will be instantly dismissed from site. Fire marshals will be appointed and in the event of an emergency will notify the emergency services. These persons will ensure that ‘hot works’ are done under permit only, that regular checks are carried out on fire fighting equipment / alarms and that escape routes are kept free of obstruction. They will liaise with site security, monitor and check procedures for calling the fire brigade or other emergency services carry out duties required for safe evacuation and ensure that all site operatives and visitors report to the muster point. Hot Work Permits will be issued by the Site Foremen prior to any hot works commencing on-site. Within the Hot Work Permits the relevant controls measures shall be stated and delivered to those carrying out the hot works to ensure they fully understand the risks associated with the activity. Access to the site will be kept free of obstruction at all times in case of an emergency. All site personnel will be informed of emergency procedures during induction. Evacuation Procedures On leaving the building all doors and windows should be closed if safe to do so. Personnel should move to the assembly point area by the nearest safe route of exit. The fire marshal will take a head count, reporting to the most senior member of staff present. Following the head count the fire marshal will advise emergency services of the following:

• missing personnel and their location; • emergency vehicle access to the scene of fire; • location of the main electrical isolation switch • location of any gas cylinders used or stored within the site; • No persons are to leave the assembly points until instructed to do so by the fire marshal.

3. ARRANGEMENTS FOR CONTROLLING SIGNIFICANT SITE RISKS

The following is a non-exhaustive list of particular risks to the safety and health of persons as set out in Schedule 1 of (Construction) Regulations, together with the opinion of the design team and Project Supervisor for Design Process on what elements, if any, of the works may fall within each particular risk category. It should be noted that many of the risks on the project may arise out of working methods which are at the discretion of the Contractor and as such cannot be determined by the Project |Supervisor for Design Process / Design Team. 3.1) Burial under earth fall, for example deep foundations, deep cuttings, works in excavation which are in or adjacent to unstable ground, hazardous underground services or where this is a risk of undermining existing foundations. Where excavation works are being carried out, these will only operate under a Live Permit to Work, Site specific RAMS and will be checked on a daily basis. An underground survey will be carried out; a banksman will be used when excavations being. Emergency procedures will be available and presented to operative prior to works starting. Excavations will be clearly marked and secured when works within the area have stopped. 3.2) Engulfment in swampland, for example: unstable ground. Work adjacent to lakes and ponds or reclaimed ground. Ground stability will be tested prior to works commencing. There is a small river located close to site which will be cordoned off and adequate signage displayed. 3.3) Falling from height for example: where the risk associated with working at a height is likely to be aggravated by the presence of another significant hazard then a particular risk may be present. TC acknowledges The Work at Height Regulations 2005. Our contractors will prepare a written method statement with hazard and risk assessment in advance of works. The safe system of work employed must take into account the hierarchy of controls. Scaffolding: Any scaffolding erected onsite will be done so by a competent scaffolding erector. Weekly checks will be carried out to ensure its integrity. Any scaffolding that does not meet acceptable standards will be cordoned off and disused until site management has given permission for its use. 3.4) Work on or adjacent to fragile roofing materials: Fragile signs must be posted and a drawing made available highlighting the fragile areas. Access to these spaced must be risk assessed, main contractor to control access with the aid of a permit to Work. Safe system of work required before work commences. 3.5) Operations where the nature of the work or other restrictions makes it impracticable to provide standard scaffolding or other suitable and sufficient means of support.

Should non-standard scaffolding be required a temporary works design certificate must be completed and forwarded to the design team for approval. TC must liaise with ASM in advance of any specialist scaffolding being erected. 3.6) Works, which put persons at task from chemical or biological substances constituting a particular danger to the safety of such persons or involving a legal requirement for health. Not applicable.

3.7) Where certain materials or substance may give rise to particular risks based on their toxicological or physio-chemical properties, for example: the presence of explosive atmospheres (including potential for dust explosions) Not applicable. 3.8) Work in confined spaces (i.e. areas where there is a possibility of a deficiency of oxygen or presence of dangerous gases or vapours) Not for seen. 3.9) Any other work activities involving the use of any chemical or biological agents where risk assessment in accordance with REACH Regulations (2006) reveal a risk to safety or health. TC is aware there is an oil tank located in the basement area of the NHS building. This provides a significant hazard’s from;

• Burns • Asphyxiation • Fire • Trip, falls • Explosion

TC will insure adequate control measures and emergency procedures are in place TC will be employing the services of a specialist contractor to empty and make safe the oil tank and provide in writing that this has taken place before removal of tank. Contractor site specific RAMS will be checked and forwarded on to Sandia for approval before works start. 3.10) Work with ionising radiation requiring the designation of controlled or supervised areas as defined in \article 20 of \directive 80/836/Euratom: Not applicable. 3.11) Work near high voltage power lines & sub-stations TC is aware of the likelihood of overhead lines, in particular at site entrances. TC will carry out an extensive survey of the area prior to commencing work on site to identify the extent of cables. Tying into existing main and sub-boards. Contractors to work to LOTO procedures and to liaise with shop manager. There are no overhead cables present within the site, but there is a sub-station adjacent to the building on Clark street. TC will ensure safety measures are put in place prior to works starting to avoid any damage to the sub-station or exposing employees to any hazards associated with this. As the works progress towards the farness rd end of the building, a scaffolded crash deck will be put in place to protect the sub-station from any possible damage. Operatives will be informed of the substation and the dangers associated with it. 3.12) Work exposing persons at work to the risk of drowning Not applicable.

3.13) Existing underground services TC to consult with the design team – all excavation work must be risk assessed. Persons trained in “Location of Underground Services” will be required – NRSWA Cable Avoidance Training or Similar. Before starting work in any area the Site Manager shall in consultation with the relevant specialist establish the location of all existing services (including foul and surface water drainage) that may be disturbed by the work. An illustration will be draw up to show isolation points for operatives and will be shown to them at induction. Below are Identified locations of services from drawings provided.

Telecoms

Water services There is a 600mm water distribution main running through the centre of the site. Movement of vehicles and plant with a total weight exceeding 6 tonnes across the unprotected Pipeline is forbidden. The repetitive movement of vehicles or plant of any weight over the unprotected Pipeline in the same position is forbidden. Where temporary or permanent access is required, the Promoter must consult with UU prior to gaining access

Tanks which have contained hazardous chemicals require special precautions during the cleaning/removal process. Exposure to hazardous chemicals can arise from skin contact, inhalation or accidental ingestion. Hazardous chemicals often encountered during tank cleaning include; Lead anti-knock compounds, Hydrogen Sulphide, Polycyclic Aromatic Hydrocarbons (PAH), and Benzene. Only trained authorized operatives will carry out any works on this tank once it has been emptied and made safety by a specialist contractor. Suitable PPE will be provided 3.19) Other risks not identified in the particular risks Demolition Prior to any demolition works beginning TC will develop a site specific Method Statement on how they plan on carrying out the works without causing any undue hazards and risk to others. Demolition works will be thoroughly planned and executed with details and arrangements conveyed to all prior to commencing works. Only trained, experienced and competent staff is to be employed following site training and toolbox talks on the works and working techniques employed. The aimed method of demolition is as follows, but this may vary depending on works;

• Set up site welfare facilities, parking area, storage compound etc. • Locate, mark up and isolate utility services • Erect/repair secure timber hoarding around entire site boundary, set up vehicular and

pedestrian access gates. • Commence stripping of inside of building (ceilings, walls etc.). All arising’s placed in

dedicated skips. • Remove all external walling, windows etc ,taking rubble to crusher for recycling, glass etc to

dedicated skip. • Commence removing roof sheeting and insulation as per Method Statement below.

• Following sheet removal, commence dismantling of steel structure – this will be ongoing after sheet removal as per MS.

• Load all sheeting/steel onto waiting trailers and remove from site. • Complete roof sheeting and steel structure dismantling and removal from site. • Break up existing floor slab and process through crusher. Stockpile crushed material on site

for re-use. • Take delivery of re-bar/mesh etc. ordered for new build and place in storage area. • Formwork contractor will begin to fabricate steel mesh cages and store in dedicated area

until required for new build. Asbestos A demolition survey will be completed prior to demolition works commencing. If any materials containing asbestos are found whilst works are on-going, works will cease immediately, Lidl & Santia will be informed. Works will not continue until it is safe to do so. A Geo-Environmental Investigation had taken place on 14th July 2015 by Trident Surveying Limited. Asbestos was found, so a contractor has been engaged to remove this safely. Weather Contractors are to cease work in high winds or severe weather conditions. Contractors are to ensure that all materials brought out of the building are safely stored and prevented from taking flight. Manual Effort This project will require a significant amount of manual effort. The chosen contractor is to investigate known devices capable of delivering and when require, removing materials to / from site. Impalement Prevention Consider impalement hazards when pre-task planning. When possible, plan the project so that elevated work will be conducted prior to creating impalement hazards (e.g. vertical projections from lower levels). Space limited and high activity tasks are also best conducted prior to creating impalement hazards. Remove or eliminate (e.g. cut off) impalement hazards. When it cannot be done, guard the hazard. Horizontal and vertical projection can often be protected with approved re-bar caps. Store thin diameter piping and other impalement hazards so that the ends do not project into walkways and work areas. Flag the ends of thin diameter piping and other impalement hazards when moving it within the works area. Erection Of Hoarding The erection of hoarding will be carried planed and risk assessed. Those involved in erecting the hoarding will have went through site induction, job specific Risk Assessment and Method Statement. Risk Assessment and Method Statement will include the require to ensure the below points are not obstructed during and after the erection of hoarding: • Existing services. • Warning signs. • Security lighting. • Existing fire exits. TC will be generally using Foundation block / kentledge foundation hoarding methods. Resistance to overturning may be provided by either a foundation block or kentledge, Where this

type of foundation is used, the stability in both directions should be carefully considered. The kentledge may be precast concrete, a purpose made block, or a number of scaffold tubes, or a container acting as ballast and filled with sand or a liquid. Ballast in the form of liquids should be securely contained to prevent inadvertent or malicious removal. Whenever using kentledge, see Figure 1, the exact value of overturning resistance is known, and a lower factor of safety can be utilised.

The minimum quality of timber used in hoardings should be grade C16 (BS EN 338). Where subject to moisture and possibilities of decay, all hoarding timber should be treated with a wood preservative, preferably supplied pre-treated with a pressure applied wood preservative.. Working @ Height Keeping in line with the Working at Height Regulations 2005, a risk assessment for all works conducted at height will be completed and arrangements put in place for: • Eliminating or minimising risks from working at height. • Safe systems of work for organising and performing works at height. • Safe systems for selecting suitable work equipment to perform works at height. • Safe systems for protecting people from the consequences of works at height. Any contractors planning on carrying out activities at height will have to produce a detailed Risk Assessment and Safe System of Work Suitable and sufficient, safe working platforms must be established for all working at height and safe means of access must be provided. Edge protection must be provided to all areas where there is a risk of persons or materials falling. Particular Considerations: • Works of cladding • Electrical/mechanical works • Works of decoration • Roof works • Ceiling works Working at height activities will be assessed throughout the works. The site manager will inspect all working at height activities on a daily basis. Tower Scaffolding Tower scaffolds are used on site that they are all covered by a permit to work, issued and controlled

by the site manager Injury from Falling Materials Throughout the demolition works all staff on site will have undergone a site induction, went through the relevant Risk Assessments and Safe Systems of works. Pedestrians will not be permitted to access areas where plant & machinery are in operation. Uncontrolled Collapse A detail method statement and relevant risk assessments will be issued to all those connected to the works in question outlining how works are to progress. However to control the risk to operatives, the manager will do a role call each morning and at lunch to see whom is on site and working in the affected areas. He will also check areas before demolition starts and inform ops of the areas that are to be demolished each day. We will be making use of no go areas and highlight them on the site plan. Suitable tool box talks will be given to keep operatives informed of the dangers. Risks from connected Services All services will be disconnected prior to any demolition works commencing. This is been organized by Lidl. No works will start until Lidl has forwarded the relevant letters of confirmation.

All utility services are to be isolated and terminated to site demise prior to commencing works with written confirmation obtained

from construction manager. Stripping and Dismantling Detailed Safe Systems of Work & site specific Risk Assessments will be developed and issued to operatives prior to demolition works commencing onsite. Lifting Operations TC Ltd will be the ‘’Appointed Person’’ to co-ordinate the lifting activity on site. The responsibility of the Appointed Person will include:-

• Assessment of the proposed lift(s) to provide for planning, selection of equipment, instruction and supervision to enable the work to be carried out safely.

• Drawing up and implementing a suitable lift plan for all lifting operations. • Implement a safe system of work • Control / Responsibility for all lifting operations.

All slingers and signallers involved in lifting operations must hold a training qualification and be considered competent by the Appointed Person for the task.

Lifting operations will not be undertaken in high wind conditions. Noise and Vibration TC Ltd will aim to adopt best practice in accordance with BS4142 and BS 5228 pt1 to minimise and noise/vibration disturbance, so far as is reasonably practicable. Dust Various tasks onsite will create dust which could potentially cause ill health to those exposed. To reduce this risk and the amount of dust, TC will dampen down these areas or use other methods other if available (vacuum extraction). Where and when this cant be done, TC will promote dust awareness on site and provide RPE FFP3 masks. TC Construction is in the process of training various employees on the ‘Train the Trainer – Face Fit Test’. This will allow for in-house Face Fit Testing to be carried out ensuring all those required to be face fitted will have gone through the relevant testing. To avoid COSHH All hazardous material will be subject to a COSHH Assessment and accompanied by the MSDS. A copy of this will be attached to RAMS and all operatives advised of the requirements prior to commencing work. Permit to work Permits to work will be utilised throughout the project for various tasks. The Site Foreman will issue a permit to work when required. Copies will be on display will the permit is opened and stored within the Health and Safety folder when the task is completed. Traffic, Including Deliveries The site boundary will secured with hoarding to prevent unauthorised access. The site team will have cognisance of the Health and Safety Guidance Note HSG144 ‘’Safe Use of Vehicles on Construction Sites’’. Operatives will be informed at site induction and daily task safety talks of the importance of providing protection to the public, and others not employed by the company. Parking for contractors will be with the site demise. Deliveries will be organised by the Site Foreman, he will ensure deliveries are only accepted on site during times that will cause less obstruction. It is also recommended that the site traffic plan and rules are forward to any delivery companies once they have been established. A record of these emails will be kept in the site safety file. Traffic in the proximity of the store is busy and typical of the location with pedestrians and vehicles passing the store and through road and pedestrian pavements. A traffic plan will be provided before

commencing work detailing movement of vehicles around the site. Mechanical Plant & Equipment The Company will ensure that all plant delivered to site is in good working order, in a safe condition, and that in relation to lifting appliances and tackle, has been thoroughly examined within the defined statutory period and has a current test certificate. Daily inspections will be recorded on a pro-forma record sheet and weekly inspections will be recorded in the PUWER and LOLER register for machines and lifting tackle. All excavators that lift structural sections and other items will be deemed to be lifting appliances for the purposes of statutory inspections. The site management team will be responsible for ensuring that when not in use each item of plant and equipment is suitably immobilised and secured so as to prevent unauthorised use or interference. All plant operatives must be competent, trained and be able to provide relevant qualifications for the item of plant they are operating. With regards to foreign plant operatives who do not have a U.K. recognised training certificate or skills card; TC Construction will contact CITB to arrange a competency assessment for that driver. This will also be applicable for operatives who are required to operate a long reach excavator for which they cannot prove competency i.e. by providing the number of hours previously spent driving a long reach excavator. All dumpers, excavators and cranes will have either a CCTV or convex mirror fitted for all round vision. All excavators, used as lifting equipment, should be fitted with “Check Valves” and equipped with fire extinguishers and spill kits. Quick hitch pins and retaining clips will be used at all times. Telehandlers will have all protective screens intact prior to being used; this is due to the hydraulic boom being positioned in close proximity to the operator’s cab. If the screen is missing and the operator has any body parts outside of the cab then the boom could potentially dismember limbs. All lifting equipment used in lifting operations will be accompanied by appropriate “Test” certificates and undergo “Thorough Examinations” every 12 months. Lifting accessories such as chains/slings must be “Thoroughly Examined” every 6 months. Each item of lifting equipment put in place for the duration of the works must be appropriately tested and thoroughly examined prior to use. The records for all subcontractors’ plant on site will be handed in at the weekly safety review meeting. These will be filed and available for inspection. All records for plant owned or hired by TC Construction will be held on site.

Estimated Plant/Vehicle requirements In order to limit disturbance to traffic and residents, TC will limit the number of vehicles during the construction process as much as possible. It has been estimated that the below plant will be required; • 8t track digger x 2 • 22t Volvo duck 360 x 1 • 10t Crane x 1 • Boom Genie lift • JCB Teleporter

Waste Waste on site will be controlled via TC Environmental policy which will be implemented throughout the site: • All deliveries will be made via the main access route and deliveries will go to the off-loading area, any materials will be stored in the material storage areas. • Good visibility and a banksman will be provided for offloading of deliveries. • All storage of materials will be contained within the site boundaries. • Waste will be managed in line with the Site waste management Resurfacing works Risk Assessments and Safety Method Statements will be developed and implemented to ensure this activity is carried out in a safe manner. Documentation from the relevant contractors will be presented to the site foreman and a copy held on site. Temporary Site Electrics All portable electrical appliances used on site will be powered with a 110v supply and will be inspected/tested in accordance with the requirements of the Electricity at Work Regulations. Temporary supplies will be installed and tested by a competent electrical contractor to the recognised standard. Site huts will be electrically inspected and tested approximately every twelve months. Electricity Supply A generator or temporary mains supply will be available on site and transformers will be installed to provide a 110 volt supply for portable electrical appliances, generally on each floor. These transformers will be inspected by a competent person before first use and subsequently on a weekly basis to ensure electrical safety, the results of those inspections being recorded in the site office. Welfare facilities and offices will be provided with a safe 240V electricity supply, protected by RCD devices. Electrical Appliances Only 110-volt electric or battery powered tools will be used on site. All component parts will be colour coded yellow for easy identification. Special allowances may be made for the use of higher voltage appliances, but ‘Residual Current Devices’ (RCD’s) or other precautions must be in place before approval can be granted for their use. Portable electrical appliances will be inspected regularly; PAT (Portable Appliance Tested) every three months and certified that they comply with industry standards. Extension leads must be placed where they cannot be damaged or cause a tripping hazard.

Damaged or defective equipment will be replaced or taken out of service until repairs have been carried out. Environmental & Welfare Good housekeeping will be enforced throughout the site to ensure any loose materials etc. do not cause an obstruction during strong winds. Relevant PPE will be provided to operatives, which will include PPE to deal with adverse weather. Washing facilities, dry rooms, canteens etc. will be provided by TC Lighting around the site will be erected. Erection will be completed after risk assessment and method statements have been developed. Inspections on lights will be carried out weekly to ensure there integrity. Roadside Elevations Any demolition required at roadside will be carried out with due care and attention to ensure no concerns are raised throughout. Prior to any demolition works beginning Section 80 will be submitted to local authority with section 81 requirements adhered with, with regard to any demolition activities. Road Side Enclosure There is a roadside enclosure to one elevation of the site which must be considered when working in close proximity to during any works, and consideration given to protecting from damage or disruption of use. Continued Operation of Neighbouring Businesses Neighbouring businesses consist of an I love crumpet dinner of Clark street, various warehouses, various businesses on Furness drive and an FX Leisure on Aldone road. There are also domestic premises located of Garston Road. It will be ensured that demolition activities undertaken on and off site (such as high noise activities, deliveries, road cleaning etc.) have minimal effect on the above neighbouring homes and business operations alike. TC Construction to liaise with each of the named neighbouring residents and businesses prior to commencement to create a communication channel and assure them that demolition & construction activities will not affect their ongoing business operations. Continued liaison throughout the project is also encouraged. Protection of the Public Ensuring the public are safe from the construction activities carried out by TC is a main priority. All works that may have a likely effect on the public will be assessed for associated risks and where applicable a Safe System of Work developed. All sub-contractors must ensure their works will have an effect on the health and safety of the public and will be made aware of these dangers at site induction.

Maintaining Escape Routes The escape routes will be kept clear at all times throughout the project. Daily checks on these routes will conducted to ensure compliance. Contact with Vermin Locations of bait boxes will be established prior to works. Operatives will be made aware of the dangers associated when Vermin. All operatives will be provided with appropriate PPE and will have the use of good welfare facilities ensuring they can keep up a high standard of hygiene. Manual Handling Manual Handling Risk Assessments will be issued to all the operatives’ onsite. Manual Handling training will also be provided. Lifting aids will be provided and will be used when possible. No operative will be required to lift more than he/she is personnel fit for. Good manual handling techniques will be explained and induction, during training and information on the same displayed on the canteen information board. 4. SAFETY FILE

The Health and Safety File for this contract will be prepared, reviewed, amended and added to by TC and the PD, as the project progresses. PROJECT REVIEW

The Safety and Health Plan will be reviewed throughout the duration of the project by the management team. These reviewed shall be carried out at 1 month intervals. The review will consider:

• Risk Assessments;

• The relevance and appropriateness of standards and procedures set out in the plan and how they may be improved;

• An assessment of the degree of compliance with standards;

• Incident and Ill health data;

• Feedback following audits.
