Social Determinants of Health (SDOH)
Tackling SDOH in Health Centers
• Addressing social determinants through providing
enabling services
• Enabling services are defined as:
“Non-clinical services that aim to increase access to
healthcare and improve health outcomes”
• Examples: health education, interpretation, and case
management
Work Force
• Workforce:
– UDS collects data on 7 categories of enabling
services personnel
– 89% of FTEs come from 4 categories:
• case managers
• eligibility assistance workers
• outreach workers
• patient/community education specialists
PRAPARE
• The Protocol for Responding to and Assessing
Patients’ Assets, Risks, and Experiences
(PRAPARE)
• National effort to help health centers and other
providers collect and apply the data they need to
better understand their patients’ social
determinants of health
Future of the O&E, Assister Workforce
• Enrollment assisters play
an important, growing role
at health centers
• Assisters address many
social determinants of
health
• Investing in the
cross-training of
CACs/Navigators
provides multiple upsides
for health centers and
patients:
• CHWs
• Patient Navigators
• VITA Volunteers
