1-2 days after evaluation, the proponents shall return THIS FORM to the evaluators with the (a) evaluated manuscript with correction and (b) newest version of the manuscript, to further check if the proponents have actually edited their papers.

Failure to submit this form with the revisions will result to a failing grade on manuscript/research paper.

Third and Fourth column (Date rechecked and Signature) shall be filled AFTER revisions were made and there were no further revisions needed. This will mean that the paper is ready for final defense.

In case that the paper still has corrections on the second round of evaluation, another evaluation form shall be used until the paper was finalized.
