2. Any cancellation or change of appointment must be made through KPJ Wellness & Lifestyle at 06-767 8272. This must be done minimum seven (7) days prior to the appointment date.

3. An exception to the appointment cancellation shall be made for ladies who are menstruating on the day of the Health Screening appointment. For this, please contact KPJ Wellness & Lifestyle Customer Relations Executive.

4. Failure to turn up for a confirmed appointment would result in your entitlement for the year being forfeited.

5. This Confirmation Slip is valid for the purpose of Health Screening only at the KPJ Hospital Wellness Centre and does not include any diagnostic or treatment at the KPJ Group of Hospitals.

6. For details on the Health Screening preparation, please refer to KPJ Wellness & Lifestyle Programme Rules & Regulations.

8.30am (your punctuality is appreciated). At the Wellness Centre, you will be attended to on

a first-come-first-served basis. Kindly inform staff at the Wellness Centre during

registration if you are pregnant, suspected to be pregnant, feeling discomfort or if you have recently undergone any surgeries.

You are required to fast from food and beverages (except plain water) for at least 8 hours before the Health Screening appointment

Avoid bringing expensive valuables during the Health Screening.

Exercise shoes and attire are required if you are scheduled to undergo the Stress ECG (Treadmill) Test, which is subject to the doctors’ recommendation upon certifying you as fit. For further clarification, contact KPJ Wellness & Lifestyle Programme Customer Relations Executive.
