Telehealth delivery could be as simple as two healthcare professionals discussing a case over the phone, or as sophisticated as using videoconferencing between providers at facilities in two different countries. In any form, telehealth helps bridge distances and extend medical practitioners when resources are limited. With this type of technology, a physician or nurse is able to access a wide variety of data on a large number of patients from one central location, creating a more efficient healthcare delivery model. In home care, telehealth can provide a sensible and effective means of caring for a large number of patients with a variety of health conditions. Because home-based telehealth directly involves patients in their own care, telehealth can also be an excellent way to educate patients and families on managing their chronic illnesses, enabling them to avoid relapses and re-hospitalizations. Northeast HomeCare, Beverly and Addison Gilbert hospitals’ Medicare-certified home health agency and a member of the Northeast Senior Health continuum of services, utilizes telehealth in the care of patients with congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD) and other chronic diseases. Telehealth is the delivery of health-related services and information via telecommunications technologies. Telehealth delivery could be as simple as two healthcare professionals discussing a case over the phone, or as sophisticated as using videoconferencing between providers at facilities in two different countries.

Following the model of the highly successful, 20-year-old Spectrum at Beverly Program, Spectrum at North Andover will offer a range of services to residents of Andover, Boxford, Georgetown, Groveland, Haverhill, Middleton, North Andover, North Reading, Topsfield and other surrounding communities. Not only will the program provide a valuable community-based resource to enable clients to remain in their own homes for as long as possible, but it will also offer peace of mind, respite and practical support to family caregivers.

Spectrum at North Andover’s many features include:

1:4 staff-to-client ratio, with a highly trained multidisciplinary team of caregivers On-site nursing, medical monitoring and medication administration Clinical social work supportTherapeutic, failure-free activities and outings Assistance with activities of daily living Nutritious and enjoyable meals and snacksExpansive and secure indoor and outdoor spaces, tailored to the specific needs of people with cognitive disorders Free caregiver support and education

To finance the renovation, furnishing and other expenses of Spectrum at North Andover, Northeast Senior Health, in partnership with the Northeast Health Foundation, has raised nearly $120,000 in the private fundraising campaign, including several large gifts from board members and individuals, as well as grants from foundations, bringing us to one-third of our overall $319,000 goal.

in the set-up. Disease-specific symptom questions and patient education are also displayed on the screen. Once the data is sent over the patient’s phone line to a secure website, the Northeast HomeCare nurse evaluates the patient’s readings to determine whether some

sort of intervention is needed. Perhaps the patient’s weight has increased by a few pounds. This could be the first sign of an acute problem for patients with CHF. Sometimes a phone call is all that is needed, but at times, a same-day home visit by the nurse is in order. The nurse also can communicate any concerns directly to the patient’s primary care physician (PCP), who may make adjustments to medications or other changes to the plan of care. All of these interventions are designed to detect and address the problem early so that the condition can be stabilized at home, preventing the need for hospitalization.

Home-based telehealth empowers patients to care for their own chronic diseases and engage more efficiently with their healthcare providers. Patients on the Northeast HomeCare Telehealth Program learn about the inherent relationships between the food they eat and their weight. They learn the importance of following their therapeutic diets. Patients monitoring their blood pressure learn how important it is to take their medications as prescribed. Additionally, oxygen saturation can be monitored while a patient is being weaned off home oxygen. Trends can be printed out as an additional tool to help teach patients about their condition.

Telehealth does not replace nurses by any means. Instead, it enhances what nurses can do, and helps nurses, physicians, patients and families increase communication and collaboration in order to more effectively manage chronic diseases at home.

The 2011 North Shore Arthritis Walk will take place on Sunday, May 22, 2011 at Lynch Park in Beverly. More than 600 participants from nearly 20 North Shore communities will form teams and walk together to fight arthritis by raising funds to help find a cure.
