Registration i nfo「mation

The Extended Day Program is a community service avaiIable to alI K-5th grade students at C「ookshank

Elementary School as we= as St. 」ohn’s county schooI Board empIoyees. The registration fee and first

Week’s p∂yment is due at the time of registration. Registration fees are NON-R詐UNDABしE even if you

decide not to enro町our chiId/c刷dren after payment is received` You may, however, enrolI or

Withdraw from the program at any time during the schooI year without paying the fee a second time.

The Extended Day Program is tot訓y seIf supportive. New enro=ments and reでnrO=ments are required

to pay in advance for services rendered accordingly to the FIorida Constitution (A巾cle VIrsection lO〉.

Payment is requested by check, CaSh. or money o「der payable to Crookshank Elementary

School.Payments for the Extended Day Program MUST be paid in advance on a F「iday for the folIowing

Week’s care to remain in compliance with State and District policies and procedures. Payments that are

received on a Monday after 3 pm. w紺be considered late and a $10.Oo !ate payment fee w紺be appIied

to γOur aCCOunt. Any fam時two days behind in payment w紺not be a=owed to attend until payment is

made in ful=f a student is withdrawn from the p「ogI’am With a past due payment notice, that student

W紺not be al!owed to reでnter the program until all past due payments are made in full. Any family

Discipline Policy

We occasiQnally have students who make errors in judgement during Extended Day" We feel strongly

that when we have support from the parents at home, We Can be more e什ective in dea臨g w軸

discip冊e problems. PIease 「ead over the Program Safety GuideIines with your c刷d. After an informal

Waming, yOu Wili be n〔癌fied in writing of any behavior problems with your c胴d. After the third wri龍en

notice, yOur Child w紺be suspended from the program for l daY. After five wr請en notices the c輔d w紺

be suspended for 3 days. 1fγOur ChiId receives a sixth notice at any point during the year, he or she w紺

be asked not to return to the Extended Day Program. The Extended day Program reserves the right to

SuSPend or expeI a child if at any point in time he/§he threatens the safety of himseluherself or another

C輔d.

Program Safety Guide臨es

We want your child to have fun during Extended Day, however we are conce「ned for their safety. We

ask no more from the students during Extended Day than du「ing the reguIar school day. The rules are

exactIy the same. Your c剛d’s teacher as we= as our group ieaders w紺be working doseIy with your ch潤

from the very first day of school to teach them the proper use of equipment and proper behavior inside

and on the playground. Any c嗣d endangering themselves or others, misusing the pIayground

equjpment or not following the safety rules w紺Be unabIe to continue use of the playg「ound fac賄ies.

*Ch胸ren should a!ways be ∂ble to see thei「 group leader.

*Proper use of equipment is to be displayed at a冊ime§.

*AIways use inside voiees when in the building. You cah be as Ioud as yo両ke outside.

The Extended Day Prog「am is offered each day schoo=s in session f「om the time students are released

聞ti1 6:00pm. Throughout the afternoon, Ch潤ren w紺have the opportunfty to participate in any games,

SPOrtS, and activities. The「e w紺be a 45 minute time sIot reserved da時for homework and reading

adivities. if you do not want your ch潤to complete homework during Extended day hours, Please

COntact the Extended day Coo「dinator in writing. Homework is not done in afterschooI on F「idaYS.

Pick - Up Procedures

The sign - Out tabIe w紺be Iocated inside the cafeteria ofthe school at the front entrance. You w紺

need to sign your name as we= asthe time of pick up in the space provided each day on your ch胴’s

Page. For the safety of your ch胸, Signing out is required on a daily basis. Students may NOT sign

themselves out. Only an ad亜with your approval w紺be a=owed to remove your ch潤. Picture lD w紺be

necessary for aII persons picking up from Extended Day until we get to know YOu. Be sure to list訓

PerSOnS yOu have a"owed to pick up you「 ch胴on the regist「ation fo「m. Your ch胴WIしL NOT be

「eleased to anyone who is not on your consent list. Please contact the Extended day Coordinator at 669-

3254 if an emergency arises and someone who is not listed on your consent form w冊be picking up your

Ch嗣. The Extended day Program ends PROMPTLY at 6:00 pm. da時There w紺be a ;1.00 per minute
