Effective July 1, 2018

The goods and services shown on this price list are those we can

provide to our customers. You may choose only the items you

desire. However, any funeral arrangements you select will in-

clude a charge for our basic services and overhead. If legal or

other requirements mean you must buy any items you did not

specifically ask for, we will explain the reason in writing on the

provided statement describing the funeral goods and services

you have selected. This list does not include prices for certain

items that you may ask us to buy for you; such as cemetery or

crematory services, flowers, and newspaper notices. The prices

for those items will be shown on your bill or the statement de-

scribing the funeral goods and services you selected. This list

does not include all the costs of final disposition that you may

be required to pay. Please note that prices and merchandise of-

ferings may change from day to day.

PROFESSIONAL SERVICES OF FUNERAL DIRECTOR & STAFF

Services include, but not limited to:

Staff available 24 hours daily to respond to the initial request for service

Custody of remains

Arrangement conference with family and/or its representative to gather

vital statistical information for all required documents and advise to de-

termine desired services

Preparation and filing of forms with governmental offices

Preparation of death certificate and filing with local health department

register.

Securing obituary information for newspaper and website publications, if

desired

General administration and record keeping

Coordinating of funeral service plans with clergy, cemetery, crematory,

and all other parties involved in the final disposition of deceased

Compliance with infectious waste regulations and employee safety regu-

lations mandated by government agencies

Overhead expenses relative to our facility and operations, such as insur-

ance, administrative, equipment and inventory costs

This fee for our professional services and overhead will be added to the total

cost of the funeral arrangements you select. This fee is already included in our charges for

direct cremation, immediate burial and forwarding and receiving remains

LOCAL AND LONG DISTANCES TRANSFER OF REMAINS

Local transfers and transportation fees applies within a 45 mile radius of the

funeral home. Beyond that distance there is an additional charge of $2.00 per

mile.

Transfer of remains to funeral home (local) $495.00

Additional assistant for removal of remains from residence $95.00

ADDITIONAL EXPENSES

Arranging the funeral ceremony and committal often requires the funeral

home to pay others on your behalf. We do not charge for this service. This

may include the cemetery/crematory, church/clergy, death certificates, flow-

ers, newspaper notices, and music. Often the timing of the funeral arrange-

ments (hours, date, or holiday) may make it necessary for us to estimate what

advances you want us to make on your behalf.

EMBALMING (includes use of preparation room and sanitary care)

Except in certain special cases, embalming is not required by law. Embalming

may be necessary, however, if you select certain funeral arrangements, such as

a funeral with a viewing. If you do not want embalming, you usually have the

right to choose an arrangement that does not require you to pay for it such as,

direct cremation or immediate burial. If embalming is chosen, the decedent

may be embalmed at a location other than this registered funeral home.

Pennsylvania Rules and Regulations number 13.211(A) requires that any human remains held 24

hours beyond death shall be embalmed or sealed in a hermetically-sealed container that will not

allow fumes or odors to escape or shall be kept under refrigeration; provided, however, that this does not conflict with any religious beliefs or medical examination.

home. Beyond that distance the charge for additional miles will be $2.00 per

another funeral home or airport, air tray or shipping charges.

RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME

Our charge includes: basic services of funeral director and staff, transporta-

tion from Philadelphia International airport, and funeral coach for transfer to

cemetery or crematory. This charge does not include other preparation of

merchandise.

Our charge for immediate burial (without viewing or ceremony) includes:

TERMS OF PAYMENT

We are a small business providing services and merchandise and are totally

dependent upon our clients for prompt payment. Our payment terms are as

follows:

1. Payment of not less than one-third of the total services and mer-

chandise selected amount is required one (1) day prior to any fu-

neral or cremation service. Balance will be due in 30 days from the

contract date.

2. Cash Advances – All cash advanced money must be paid in full

one (1) day prior to any funeral or cremation service. Cash ad-

vanced money may be paid to the funeral home for disbursement to

the third party or checks may be written directly to the third party

and envelopes will be provided by the funeral home.

3. Payment Policy - Total Funeral Bill

A. A 2.5% discount with cash or certified check, on all services and merchandise

(excluding cash advances made by the funeral home) is offered for payments

received, in full within 5 working days of the arrangement conference. The total

balance, in all cases, is due within 30 days of the arrangement conference.

B. Credit Cards: We accept Visa, MasterCard, Discover, American Express

C. Insurance Policies: Verified insurance policies assigned and payable to the

funeral home will be accepted as partial or full payment of the funeral bill. No

late charges will be charged if paid within 30 days.

D. Under circumstances where the estate of the decedent will bear full responsi-

bility for paying the funeral expenses, all late payment penalties will be imposed

for payments received after 30 days.

E. When Monies are not readily available or guaranteed to us for funeral ex-

penses, evidence of financing and payment must be given prior to the time of

the funeral being scheduled and/or announced.

F. In all cases where total payment has not been received within the allotted 30

days, a late payment penalty of 1.5% per month (18% per annum) will be

charged on the unpaid balance, in addition to a rebilling fee of $75.00 per month.

G. The funeral home reserves the right to pursue collection of unpaid funeral

expenses by all prudent and lawful means, including criminal prosecution and/or

civil litigation. All costs incurred by the funeral home during the collection of

delinquent accounts (i.e. court costs, filing and legal fees) will be added to the

unpaid portion of the funeral contract, in addition to all interest charges and pen-

alties stated above.

Final responsibility must be agreed upon by both the client and the funeral home

or its representatives.
