CASE NARRATIVE

With the exceptions noted as flags or footnotes, standard analytical protocols were followed in the analysis of the samples and no
problems were encountered or anomalies observed. In addition all laboratory quality control samples were within established control
limits, with any exceptions noted below. Each sample was analyzed to achieve the lowest possible reporting limit within the constraints of
the method. In some cases, due to interference or analytes present at high concentrations, samples were diluted. For diluted samples,
the reporting limits are adjusted relative to the dilution required.
Calculations are performed before rounding to avoid round-off errors in calculated results.
All holding times were met and proper preservation noted for the methods performed on these samples, unless otherwise detailed in the
individual sections below.
RECEIPT
The samples were received on 2/4/2015 10:00 AM; the samples arrived in good condition, properly preserved and, where required, on
ice. The temperatures of the 4 coolers at receipt time were 1.1 C, 2.8 C, 3.9 C and 5.9 C.
The requested 1,2,3-TCP 524.2 SIM, and 314.0_LL Perchlorate analyses were performed by TestAmerica's Irvine laboratory.
The requested 8315 Formaldehyde analyses were performed by TestAmerica's North Canton laboratory.
The samples requesting "Extract and Hold" on the Chain of Custody will be reported under a separate cover (280-64994-2). All other
samples/analyses requested on the Chain of Custody will be found in this report.
The volume submitted for 314 Perchorate analysis for sample RD-78_020315_01 (280-64994-11) was received with approximately 1/4
inch of headspace. For water samples, headspace should be about one-third of the container in order to reduce potential anaerobic
biodegradation.
VOLATILE ORGANIC COMPOUNDS (GC/MS SIM) - 524.2 SIM
Sample RD-60_020315_01 (280-64994-5)[2.5X] required dilution prior to analysis. The reporting limits have been adjusted accordingly.
No analytical or quality issues were noted, other than those described above or in the Definitions/Glossary page.
VOLATILE ORGANIC COMPOUNDS (GC-MS)
Methylene Chloride was detected in method blank MB 280-263200/8 at a level that was above the method detection limit but below the
reporting limit. The value should be considered an estimate, and has been flagged. If the associated sample reported a result above the
MDL and/or RL, the result has been flagged.
No additional analytical or quality issues were noted, other than those described above or in the Definitions/Glossary page.
VOLATILE ORGANIC COMPOUNDS (GC-MS SIM)
1,4-Dioxane exceeded the RPD limit for LCSD 280-263242/4. The associated LCS/LCSD spike recoveries were in control and
demonstrate that operating procedures were in control. No further action was required.
No additional analytical or quality issues were noted, other than those described above or in the Definitions/Glossary page.

spike duplicate for the following samples and parent samples: (280-64994-6 MS), (280-64994-6 MSD), RD-39B_020315_01
(280-64994-6). When reviewing the samples for target compounds, this retention time shift was taken into account. Individual samples
were compared to neighboring QC as well as individual matrix retention time spikes.
