Theo thông tin từ HealthDay News Thứ 4 ngày 17/8/2016, với những bệnh nhân
mới chẩn đoán tiểu đường type 2 thất bại với điều trị metformin, điều trị tích cực
sớm sẽ đạt HbA1c nhanh hơn. Điều này được chứng minh thông qua nghiên cứu
“Kiểm soát thời gian và liệu pháp điều trị tích cực đến khi đạt HbA1C mục tiêu
đối với những bệnh nhân đái tháo đường type 2 mới phát hiện nhưng thất bại với
đơn trị liệu metformin trong hệ thống chăm sóc sức khỏe” công bố trên Diabetes
Care 12/08/2016 tiến hành bởi Kevin M. Pantalone, và cộng sự.
Kenvin M Pantalone, DO từ phòng khám Cleveland và cộng sự đã tuyển lựa
những bệnh nhân mới phát hiện tiểu đường type 2 nhưng thất bại sau 3 tháng
điều trị bằng metformin. Tác giả so sánh thời gian đạt HbA1c ở những bệnh
nhân nhận điều trị tăng cường sớm (trong vòng 6 tháng kể từ khi thất bại với
metformin) và những bệnh nhân nhận điều trị tăng cường muộn.
Mục tiêu của nghiên cứu: “Sự trì trệ lâm sàng” được mô tả như một sự trì
hoãn trong việc tăng cường điều trị ở những bệnh nhân tiểu đường type 2 kiểm
soát đường máu kém. Những nghiên cứu trước đây có thể đã phóng đại tần suất
của sự trì trệ lâm sàng do thất bại trong việc điều chỉnh thuốc thích hợp và cả
những biện pháp can thiệp không dùng thuốc. Nghiên cứu này đánh giá điều trị
tiểu đường tích cực và HbA1c mục tiêu đạt được ở những bệnh nhân mới chẩn
đoán tiểu đường type 2 khi điều trị metformin thất bại.
Thiết kế nghiên cứu và phương pháp: Các bệnh án điện tử tại Cleveland
Clinic đã được sử dụng để xác định bệnh nhân bị tiểu đường type 2 mới được
chẩn đoán từ năm 2005 đến 2013, và thất bại đạt HbA1c mục tiêu sau 3 tháng
điều trị bằng metformin. Phân tích sống còn phụ thuộc thời gian được sử dụng để
so sánh về thời gian đến khi đạt được HbA1c mục tiêu ở nhóm bệnh nhân nhận
điều trị tích cực sớm (trong vòng 6 tháng kể từ khi thất bại với metformin) hoặc
điều trị tích cực muộn hơn. Phân tích được thực hiện ở các nhóm với HbA1c
mục tiêu là 7% (n=1168 bệnh nhân), HbA1c mục tiêu 7.5%( n= 679), HbA1c
mục tiêu 8%( n=429).
Kết quả cho thấy nhóm điều trị tăng cường sớm chiếm 62% trong số các
bệnh nhân kiểm soát đường huyết kém với nhóm HbA1c >7% (HbA1c mục tiêu
là 7%), chiếm 69% ở nhóm HbA1c > 7.5%( HbA1c mục tiêu là 7.5%), 72% ở
nhóm HbA1c > 8%( HbA1c mục tiêu là 8%). Điều trị tích cực sớm có tỷ lệ cao
hơn ở nhóm có HbA1C cao hơn. Thời gian để đạt được HbA1c mục tiêu trong
mỗi nhóm của nhóm điều trị tích cực sớm ngắn hơn so với nhóm điều trị thông
thường ( với p< 0.05)
Nghiên cứu đưa đến kết luận: Một số lượng đáng kể bệnh nhân mới chẩn
đoán tiểu đường type 2 không được điều trị tăng cường sớm khi điều trị thất bại
với metformin. Và khi điều trị tích cực sớm sẽ đạt HbA1c mục tiêu nhanh hơn.

Early Intensification of Tx Speeds Attainment of A1C Goals Findings in patients with newly diagnosed type 2 diabetes who have failure of metformin monotherapy WEDNESDAY, Aug. 17, 2016 (HealthDay News) -- For patients newly diagnosed with type 2 diabetes, early intensification of therapy after metformin failure is associated with more rapid attainment of hemoglobin A1c (A1C) goals, according to a study published online Aug. 12 in Diabetes Care.
Kevin M. Pantalone, D.O., from the Cleveland Clinic, and colleagues identified patients with newly diagnosed type 2 diabetes who failed to reach the A1C goal after three months of metformin monotherapy. The authors compared the time to A1C goal attainment in patients who received early intensification of therapy (within six months of metformin failure) or late intensification. The analysis was performed in 1,168, 679, and 429 patients for A1C goals of 7, 7.5, and 8 percent, respectively.
The researchers found that when poor glycemic control was defined as A1C >7.0, >7.5, and >8.0 percent, treatment was intensified early in 62, 69, and 72 percent of patients, respectively. The higher the A1C category, the greater the probability was of undergoing an early intensification. Among patients who received early intensification, time to A1C goal attainment was shorter, regardless of the A1C goal (all P < 0.05).
"A substantial number of patients with newly diagnosed type 2 diabetes fail to undergo intensification of therapy within six months of metformin monotherapy failure," the authors write. "Early intervention in patients when metformin monotherapy failed resulted in more rapid attainment of A1C goals."
Several authors disclosed financial ties to biopharmaceutical companies, including Merck, which funded the study.
