This video will review:


Searching for Patients

From the OSIIS Landing page, select “Search”

from the Toolbar


To add a NEW PATIENT, select “Add New Patient” from the Patient drop-down in the Toolbar

You will be required to go through the search process prior to adding a New Patient


Search using a patient’s:

Name & Date of

Birth (DOB)Social

Security Number

(SSN)Phone Number

Once you’ve entered the necessary information,

click “Search”OR

Address


If you are unsure of the spelling of the patient’s name, type how it sounds, along

with their DOB and use “Search with Soundex”


If multiple matches are found, results

will appear as a listThe icons under the

“Actions” heading allow you to:

View the Patient SummaryEdit Patient

DemographicsView/Add

Progress Notes

View Patient Immunization

Schedule

Add/Edit History Shots


If no results are found, this screen

will appearSelect “New Search”

to try again..

Or “Add Patient”

to create a New Patient


Adding a New Patient brings you to the Patient Demographics Screen

Complete all of the required fieldsScroll to the

bottom of the screen and

“Save Patient”


Now that the patient has been added…

Add a Contact

And a Guardian


Adding ImmunizationsSelect

“Immunizations” from the Patient

drop-down

Or select the “Immunizations” tab

from the Patient screen


The Immunization Schedules screen outlines the individual’s vaccination history

Icons in the left hand column

reflect the status of the

immunization series

Current/Up to Date

Not IndicatedPast Due

This is reflected in the “Status”

column

This is the last recorded date

in OSIIS the immunization

was given

Recommended Date is the ideal date to

administer this vaccine

This is the earliest date the vaccine should be given

This is the date at which the immunization would be considered Past Due


Refusals are recorded by clicking on the “Refusals” buttonReactions are recorded

within the Add Immunization screen


Complete the three mandatory fields…Once you’ve selected “Add Refusal”

And select “Save Refusal”


To add an immunization, click on “Add Immunization”


Everything on the Add/Edit Immunization screen is required


Choose the schedule & VIS

version

Once you’ve selected

“Vaccine Given”Once you’ve selected

“Funding Source”Choose the correct Lot

Number

Click “Save Immunization”

**NOTE: You are REQUIRED to add lot number. If the lot

number is not in your inventory, you must add the immunization

as a HISTORY SHOT


Select Yes to keep adding to this person’s record

Select No to go back to the Immunization Schedule Screen


Adding History ShotsSelect

“History Shots” from the Patient

drop-down

Or select the “History Shots” tab from the

Patient screen


From the History Shots screen there are two

approaches to adding a historical immunization

Enter multiple immunizations simultaneously by entering the “Date

Administered” and selected each immunization from the “Vaccines” list

OR select the vaccines from the “Vaccine” list without entering a date

Select “Enter Shots”


“Date Given” is the only REQUIRED field. Once you complete all the information you have, select “Save

History Shots” to continue.


Select “Progress Notes” from the

Patient drop-down

Or select the “Progress Notes” tab from the

Patient screen

Adding Progress Notes

Add any notes in the Add/Edit Progress Note window

**NOTE: ALL Progress Notes are PUBLIC. Please do not use this for

protected medical information!
