• Unstable patients: – Immediate pharmacologic therapy:
• Atropine: 0.5 mg IV q 3-5 min, total dose 3 mg • Dopamine: 3 mcg/kg/min titrated to 20 mck/kg/min if needed • Dobutamine: 5 mcg/kg/min titrated to 40 mcg/kg/min if needed
