East Renfrewshire Council, NHS Greater Glasgow and NHS Argyll and Clyde have joined forces to create a new Community Health and Care Partnership for East Renfrewshire. This leaflet is designed to provideinformation on this new Partnership, including detailsof the Public Partnership Forum which is being createdto give you a greater say in the development of your local services.

What are Community HealthPartnerships?Community Health Partnerships,or CHPs for short, are the neworganisations which are beingdeveloped across Scotland tomanage a wide range of community based health services. As the new EastRenfrewshire Partnership willalso be responsible for all localsocial work services it will becalled a Community Health andCare Partnership (CHCP).Why are these newPartnerships being created?To further improve the waylocal community health servicesare organised and managedand to develop closer partnerships between community health, social careand specialist hospital basedservices. CHPs will build on thework of the Local Health Care

Co-operatives (LHCCs), whichwere previously responsible formanaging local health services,but are larger organisationswith wider responsibilities andgreater influence. They will alsoplay a lead role in improvingthe health of local people byworking closely with local housing, leisure and educationservices and other organisationsresponsible for training and employment. Who will run this newPartnership?The Partnership will be run by NHS Greater Glasgow, NHS Argyll and Clyde and East Renfrewshire Council.

What services will the new Partnershipbe responsible for?The new East Renfrewshire CHCP will beresponsible for a wide range of healthand social care services provided fromlocal health centres, clinics and socialwork premises across East Renfrewshire.These include services provided by healthvisitors, social workers, home care staff,district nurses, school nurses, occupational therapists, physiotherapists, dieticians and podiatrists (foot care specialists). The CHCP will also be responsible for local learning disability, mental health and addiction services in East Renfrewshire and will work closely withlocal GPs, pharmacists, dentists, opticians and specialist staff fromlocal hospitals. How many CHPs are being developed ?Twelve CHPs are being developed across Greater Glasgow andArgyll and Clyde including one covering the area served by EastRenfrewshire Council. What area does the new East Renfrewshire Community Health and Care Partnership cover?The new East Renfrewshire CHCP covers the area served by East Renfrewshire Council (see map enclosed for further details). What difference will this new Partnership make to service users?The East Renfrewshire CHCP will develop closer partnerships withlocal authority and hospital based health services to break downsome of the barriers that have traditionally existed between community health, social care and specialist health services. Thiswill create new opportunities to develop additional services at alocal level and make it easier for you to access the local servicesyou require regardless of whether these are provided by yourCHCP, hospital or local council.

How will I access local health and social care services in the future?

Local health and social care services will continue to be providedin local health centres, clinics and social work offices and centresacross East Renfrewshire. How will the new Partnership be developed?A draft ‘Scheme of Establishment’, which sets out the broad principles for the development of the new East RenfrewshireCHCP, including details of the services it will manage and howthese will be organised, has been submitted to the ScottishExecutive for final approval.A local Steering Group has also been formed to develop the newEast Renfrewshire CHCP over the next year, working closely withlocal staff, community groups and others with an interest in localhealth and social care services.

How can I get involved in the development of local health andsocial care services?

All CHPs are required to develop a Public Partnership Forum (PPF) by 1 April 2006 to ensure that services users, carers, localcommunity groups and voluntary organisations have an opportunity to get involved in the development of local servicesand provide feedback on a wide range of local issues. The PPF will build on existing local community engagement and patientinvolvement arrangements to develop closer links with community groups and voluntary organisations across East Renfrewshire.
