• Family League and the Baltimore City Health Department (BCHD) are working to provide citywide home visiting services to women who have had a fetal or infant loss (interconception care home visiting).
• The services are to be provided by a peer-trained home visitor, trained in a model adapted from an evidence-based home visiting program.
• These services will expand the continuum of home visiting services that are available to families in Baltimore City. They will also be considered a core component of the B’more for Healthy Babies Initiative.

Overview of Interconception Care (ICC)Home Visiting RFP
• Adaptations enable the peer home visitor to focus on the following:• Promotion of Adequate Birth Spacing• Cultivation of Self-Efficacy and Social Support• Promotion of Healthy Adaptation to Loss
• Staffing and Caseload Ratios are the same
• Eleven (11) of the 12 elements of HFA are applicable
• Supplemental home visitor training will include topics such as grief and bereavement, goal setting, motivational interviewing, boundaries and self-care, and family planning/birth spacing counseling
• Accreditation IS NOT REQUIRED

• Health and Nutrition• Emotional Wellbeing• Relationships and Social Support• Family Planning, STDs• Substance Use• Career and Financial Development• Include the father of the baby/partner

Partners for a Healthy Baby CurriculumAdaptations
• The target population for home visiting services and support are women who have experienced a fetal or infant loss.
• Health Care Access Maryland (HCAM) is the primary source for referrals.
• Through this RFP, we expect to serve 30 women over the course of the first year.
• Staffing pattern includes:• 1 Family Assessment Worker• 2 Family Support Workers• Supervisory Support
• Caseload ratio for Family Support Workers is 15:1
