Using a PDF form with Google Health
This presentation describes a fictitious use case where we are using interactive PDF forms in a home healthcare environment, where the health data collected is uploaded to a Google Health record. It will examine the how implementing PDF Forms in a workflow can simplify, secure, and speed transactions between entities with varying levels of automation and technical knowledge. This is not meant to be an actual product or a prototype, it is simply a technology demonstration.

Who might use this PDF form
This PDF form is designed for use by a Homecare Health Service Provider - a HHSP We are assuming that the the patient has been released form a hospital - or - that they sustained injuries that did not require hospital admission, such as a job injury, like in an instance of workmans compensation rehabilitation visit This PDF is not a Electronic Medical Records (EMR) system - but this PDF could be used in conjunction with an EMR system
Why use an Adobe Acrobat PDF form?
Even when wireless internet is available where the patient is located, it may be to cumbersome to connect or prohibited. Even in cases where the healthcare service provider has equipment to connect using a wireless internet card or using a blackberry to connect the laptop, often there is no reliable signal. So, PDF is perfect - we can capture data offline and we can 'connect' when the healthcare worker gets back to the office.
What types of data would be captured?
• Observations• Diagnosis• Prognosis• Plans• Medications• Historical (Familial, Sociological) Health data captured varies greatly. For example, a patient may describe their ailment as a “knee ache” on an admission form; however, the examining physicians may determine the affliction is “a tear of the left Anterior Crucuate Ligament”.
Use Case description Patient home or facility may not have internet access or wireless available. Homecare Health Service Provider (HHSP) downloads the unfilled blank PDF form file to their
Laptop or TabletPC. HHSP visits the patient, renders the care or collects information and enters the information using Digital Pen with Paper or using Acrobat Reader and a PDF form.
HHSP connects and upload when back at office or location with internet access or wireless
available HHSP enters username and password (which provides the security part) to submit the information to an online database using only Adobe Acrobat Reader (version 6 or later is
required. ) HHSP is able to create a new patient record and they can update a patient record. They can type in the healthcard number and or name and hit retrieve and the form will fill itself out if there is already a record on file.

Adobe Acrobat provides tools to create form objects - Here I am adding a form object using Acrobat
What do the pros use to make PDF forms?
Adobe LiveCycle is a great too to make static and dynamic PDF XFA forms which can import and export XML data.
Many non-Adobe tools are also available
PDF Forms work on many devices
There is also a "Digital Paper" form !
How we build a "Digital Paper" form
Existing paper form is reviewed - consideration of what will be check boxes for Superbill, what is clinical and how will it be represented in the CCR XML data file that will be sent to EMR system
