2015 Softball Camps February 1, 2015 (Grades 4-7) February 8, 2015 (Grades 8-12) 1450 Alta Vista St. | Dubuque, IA 52001 INDEMNIFICATION AGREEMENT WAIVER AND RELEASE OF ALL CLAIMS 2015 SOFTBALL CAMPS February 1 and February 8, 2015 Please read this form carefully and be aware that by participating in the 2015 Softball Camp on February 1, 2015 or February 8, 2015 (hereinafter Event) you will be waiving and releasing all claims for injuries, as well as agreeing to indemnify, hold harmless and defend Loras College from all claims arising out of such injuries even if caused by Loras College. I, on behalf of myself and, on behalf of any child/ward of mine participating in the Event as well as any parent/guardians of such child/ward (hereinafter individually and collectively referred to as “Participant”), acknowledge understanding of the requisite skills and qualifications necessary to properly and safely participate in the Event and hereby agree to assume the full risk of any injuries, including death, damages or loss regardless of severity, which Participant may sustain as a result of, arising out of, connected with, or in any way associated with the Event. Participant agrees to waive and relinquish all claims Participant may have as a result of the Event against Loras College and its employees and agents and does hereby fully release and discharge Loras College and its employees and agents from any and all claims for injuries, including death, damage or loss which Participant may have or which may accrue to Participant as a result of, or arising out of, connected with, or in any way associated with the Event, even if caused by the negligence of Loras College, its employees or agents. Participant further agrees to INDEMNIFY AND HOLD HARMLESS AND DEFEND Loras College and its employees and agents from any and all claims for injuries, including death, damages and losses sustained by Participant as a result of, arising out of, connected with, or in any way associated with the Event, even if caused by the negligence of Loras College, its employees or agents.. Participant further understands that Loras College does not carry insurance for injuries sustained by Participant. Therefore, Participant must look to their own health insurance policy for any injuries sustained in connection with or arising out of this Event. Participant’s failure to purchase health insurance coverage does not make Loras College responsible for payment of medical or other expenses. If any provision herein is held invalid or unenforceable for any reason, Participant understands and agrees that the remaining provisions will continue in full force and effect. PERMISSION TO SECURE TREATMENT In the event of an emergency, Participant authorizes Loras College to secure any treatment deemed necessary from any licensed hospital, physician, and/ or medical personnel and agrees to be responsible for payment of any and all services rendered. 2015 SOFTBALL CAMPSFebruary 1 and February 8, 2015

Please read this form carefully and be aware that by participating in the 2015 Softball Camp on February 1, 2015 or February 8, 2015 (hereinafter Event) you will be waiving and releasing all claims for injuries, as well as agreeing to indemnify, hold harmless and defend Loras College from all claims arising out of such injuries even if caused by Loras College.

SKILLS CAMP12:45-1:00 p.m. Registration – Cost: $451:00-4:00 p.m. Skills Camp* Campers will be learning: Defensive fundamentals

including throwing and receiving mechanics, groundballs and fly balls; Offensive fundamentals including hitting, bunting and baserunning.

About the CampsThese camps are about having fun and making the most of opportunities to improve your softball skills and to make new friends! These camps are designed to help each athlete develop at her own level. During the camps, we will focus on the fundamental techniques of the game of softball such as: hitting, fielding and baserunning. Whether it is learning new hitting techniques, improving throwing form or gaining new knowledge for the game of softball, these camps will help you reach your individual softball goals.

Camp DirectorThe 2015 season will be the sixth for head coach Ashley Winter. During her softball playing career with the Wartburg College Knights, she was a first team All-Iowa Conference selection in 2008 and a two-time second team pick as a first baseman. In her final season, Winter hit .400 and was in the top five in the IIAC in batting average, slugging percentage, on base percentage, runs and homeruns. In July 2009, Winter was named the Iowa Conference nominee for the NCAA Woman of the Year Award. In 2011 she was inducted into the Iowa Girls High School Athletic Softball Hall of Fame. She was also named to the Wahlert Catholic High School Athletics Hall of Fame in 2012.

Assistant Camp DirectorAssistant Coach Rachael Rogers is in her sixth season with Loras College. She is currently head varsity coach at Western Dubuque High School. She has also coached at Wahlert Catholic High School and Wartburg College. Rachael graduated from the University of Northern Iowa in 2006 where she majored in Social Science Education and played for the Panther softball team. While at UNI, Rachael started every game at shortstop and received All-Conference honors and the Panther Impact Award. Rachael is also a member of the Wahlert Catholic High School Athletic Hall of Fame and Iowa Girls High School Softball Hall of Fame.

Assistant Camp DirectorKevin Schuchmann has been a softball pitching clinician since 1996, providing pitching instruction throughout Northeast Iowa. Schuchmann is a member of the National Fastpitch Coaches Association and has over 800 career high school wins in 28 years, including a 2007 IHSSA State Championship at West Delaware High School and a 2002 IHSSA State Runner-Up finish at Edgewood-Colesburg High School. Eight times in his coaching career, Schuchmann has qualified his team for the IHSSA Softball Tournament. In 2014, Schuchmann was inducted into the Iowa High School Softball Hall of Fame.
