PROVIDER NETWORKS » Both Anthem and CVS Caremark have large networks of providers (doctors, hospitals, retail pharmacies) to give you the opportunity to receive in-network care.

» Over 98 percent of the retail network pharmacies under our current plan are included in the CVS Caremark network. Once you receive your new ID card from CVS Caremark, you can register at www.caremark.com/startnow or contact CVS Caremark at 1-844-345-2825 to review network pharmacies.

» In the Anthem Pre-Medicare plans, you always have the option to see out-of-network providers, however, you will pay less when you use in-network providers.

• If you see an Anthem Tier One PCP in Colorado, you will have a $0 office visit copay.

» To find a PCP, see the instructions at left.

• If you have a new PCP, we strongly encourage you to make an appointment soon.

FINDING A PRIMARY CARE PHYSICIAN (PCP) » Go to www.anthem.com.

» Click on “Menu” in the upper left corner of the page and choose “Find a Doctor” under “Care.”

» Click “Search by Selecting a Plan or Network” under “Search as a Guest.”

» Select “Medical” from the “What type of care are you searching for?” drop-down menu.

» Choose a state from the “What state do you want to search in?” drop-down menu.

» Select “Blue Priority PPO” under “Medical Networks” in the “Select a plan/network” drop-down menu for searches in Colorado. For searches outside Colorado, select “National PPO (Blue Card PPO)” under “Medical (Employer-Sponsored)” from the “Select a plan/network” drop-down menu.

» Click continue.

» Choose “Doctor/Medical Professional” under the “I’m looking for a” drop-down menu. Enter your preferred search location under “Located near,” and check both boxes for “Accepting New Patients” and “Able to Serve as a PCP.”

• Physicians with a “D” next to their name are Designated Tier 1. All other physicians are Tier 2.

» Current enrollees may change their PCP by calling Anthem.

PRESCRIPTION DRUG COVERAGE

» When you begin filling prescriptions at your retail pharmacy in 2018, you will pay the full cost up to your deductible before the plan begins to pay a share of the cost. There is no deductible for mail order, so consider using mail order whenever possible.

» In 2018, your prescription cost is determined by the category of drug. There are four categories: Generic, Preferred Brand, Non-Preferred Brand, and Specialty. To determine which tier your prescriptions are in, visit the “PERACare for Retirees—2018” page in the “Retirees” section of PERA’s website at www.copera.org. Click the “Provider Directories” button at the top of the page. Under “CVS Caremark and SilverScript,” find the “Prescription Cost Estimator” and choose the Anthem plan in which you are enrolled. You may also visit www.caremark.com.

» If you currently take a Non-Preferred Brand drug, CVS Caremark will send a letter to alert you of a preferred alternative drug of the same class that will cost you less. Contact your physician to let them know your plan has changed and to discuss the preferred alternative drug. If a preferred alternative is appropriate for your condition, ask your physician for a new prescription.

» Specialty drug prescriptions must be ordered from CVS Caremark’s mail-order service and are limited to a 31-day supply.

» The turnaround time on new mail-order prescriptions in January is likely to be two to three weeks. It is important to plan ahead and have an adequate supply of your current prescriptions on hand to last you though January.

PERACARE SELECT

» PERACare Select is designed to offer you enhanced benefits and cost savings when you select preferred providers and/or facilities. There are currently two PERACare Select benefits available to you:

• Colonoscopy—The $300 copay for preventive colonoscopies will be waived for members who visit an Ambulatory Surgical Center (ASC) for the procedure. Visit the “PERACare for Retirees—2018” page of PERA’s website at www.copera.org to find links to in-network ASCs or call Anthem at 1-877-737-2258.

• Hip or Knee Replacement—Your cost for a hip or knee replacement is waived if you use one of the participating surgeons and hospital facilities. For more detailed information, visit PERA’s website at www.copera.org, choose “PERACare” under the “Programs” drop-down menu, and click on “PERACare Select” or call Anthem at 1-877-737-2258.

PERACare QuitLine

If you smoke or use tobacco products and are ready to think about quitting, the PERACare QuitLine is here to help. Quitting tobacco is a process, and the first step is the hardest. The PERACare QuitLine gives you access to a support team of dedicated coaches who can help you cope with common barriers to quitting, such as dealing with stress, fighting cravings, and coping with irritability. You can also receive free nicotine replacement therapy, such as gum or patches, through the QuitLine. It all starts with a phone call.

ADDITIONAL BENEFITS FOR ANTHEM/CVS CAREMARK PLAN MEMBERS

Diabetes Prevention Program (DPP)

One in three U.S. adults has prediabetes, which means your blood sugar levels are higher than normal and you are at increased risk for Type 2 diabetes, stroke, and heart disease. The Diabetes Prevention Program (DPP) is proven to lower your risk of developing diabetes and is covered at no cost to you. The DPP offers lifestyle health coaching and 16 weeks of in-person or online classes where you will learn skills needed to lose weight, become more physically active, and manage stress. Ask your doctor to refer you to the DPP by completing a Patient Recommendation Form available on www.anthem.com.

YOUR RIGHTSWhen it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.

Get a copy of your health and claims records

» You can ask to see or get a copy of your health and claims records and other health information we have about you. Ask us how to do this.

» We will provide a copy or a summary of your health and claims records, usually within 30 days of your request. We may charge a reasonable, cost-based fee.

Ask us to correct health and claims records

» You can ask us to correct your health and claims records if you think they are incorrect or incomplete. Ask us how to do this.

» We may say “no” to your request, but we’ll tell you why in writing within 60 days.

Request confidential communications » You can ask us to contact you in a specific way (for example, home or office phone) or to send mail to a different address.

» We will consider all reasonable requests, and must say “yes” if you tell us you would be in danger if we do not.

Ask us to limit what we use or share » You can ask us not to use or share certain health information for treatment, payment, or our operations.

» You need a User ID and password to access your account. If you have not yet set up your User ID and password, you will first need a PERA Personal Identification Number (PIN).

• You can request a PIN by clicking on the “Request a PERA PIN” link from the “Retirees” menu on the PERA home page. Your new PIN will arrive by mail within seven business days. If you need to access your account before then, please call the PERA Customer Service Center.

» You will then log in with your Social Security number (SSN) and PERA PIN.

• After logging in with your SSN and PIN, you will be prompted to create a user security profile with a User ID and password.

» Once your user security profile is complete, you will no longer use your SSN and PIN to access your account. You will use your User ID and password combination instead.

• You will need to retain your PIN when speaking to a Voya Participant Services Representative to obtain PERAPlus 401(k), 457, and PERA DC Plan information.
