number. This number begins with 20------. 2. Remit the appropriate fee with the registration form. 3. The Science and Business Institute will mail you the course(s) you have selected.4. An affidavit will accompany the course(s) that you have selected.
5. After you have completed the self-study exercise(s), complete the affidavit. sign it and mail it to the us stating that you have completed the exercise(s). a. Do not return the self-study exercise(s) to us unless requested to do
so.* Affidavit must be returned within 150 days from the date on the registration form.
b. Affidavit must be returned within BCLP registration period or contact hours will be null and voidc. Affidavit cannot be transferred from one BCLP period to the next.
FAILURE to comply with 5a, 5b, or 5c will render the registration null and void, no contact hours will be awarded, and no refund will be made.*6. Your name may be randomly selected to complete and return the exercise(s) to us as a condition of obtaining your certificate(s). A minimum passing grade of “C” (70 – 79%) must be achieved.
