Separate and identifiable evaluation and management service

This code is used when a physician finds it necessary to perform an

evaluation and management code for something above and beyond

the CPD code. Although frequently done, the diagnosis does not

necessarily have to be different

Added to the E/M code when billing to alert the insurance company that

an evaluation AND procedure were done on the same day, and the

E/M was significant and justified.
