5. Please specify the dates when you watched the presentations.

Enter the number of PDUs to claim (up to 12 PDUs). You are required to watch twenty seven

(27) presentations in order to claim 12 PDUs. Please note: At the end of each presentation, youmust complete the quick survey in order for IIL to keep records of your attendance (for PMIaudit purposes).
