Click on the “Programs & Services” tab
Click Here
Select “Pediatric Research” from the drop
down menu

For Example… If you wanted to access the most up to date
Pre-Registration Form you would click on
“Billing Forms”, and then click on the “CHS
Research Pre-Registration Form” link.
