Room Rates/TaxesTo take advantage of the special AHCA/NCAL convention rates, please make your reservations by September 6, 2013. After this date, the official AHCA/NCAL discounted rates may not be available. All room rates are per room and are subject to a 13.27% occupancy tax. When making a reservation, please provide room preferences in the Room Type and Special Request sections of the housing forms. Hotels will assign specific requests at check-in, but please note that requests are not guaranteed.

Changes/CancellationsTo make changes to your reservation, please contact AHCA/NCAL Housing at (866) 227-1876 or (415) 268-2098. Changes can also be made by fax at (415) 293-4743 or online at www.cmrhousing.com/AHCA_8H/Welcome.aspx. Cancellations made after 72 hours prior to arrival will forfeit the deposit or will be charged an amount equal to one night’s room and tax.

ConfirmationsReservations made online and by phone will receive immediate confirmation. Each time you make a change, you will also receive a revised confirmation. Reservations made by fax or by mail will receive a confirmation within 24 hours of receipt. Confirmations will be sent by e-mail. Reservations are processed on a first-come, first-served basis.• Send forms to AHCA/NCAL Housing– DO NOT SEND DIRECTLY TO AHCA/NCAL OR THE HOTEL.• A credit card or check in the amount of one night’s room and tax is required to guarantee your reservation. The hotel will charge your credit card for one night’s room and tax or use the check deposit of one night’s room and tax.• If guaranteeing by check, make it payable to Convention Management Resources, Inc. and mail it with this hotel form to the address below. As soon as your reservation is processed, you will receive a confirmation by e-mail.• If you are interested in booking a suite, please e-mail your request to AHCAHousing@cmrus.com. Suites vary in price and are subject to availability. All suites require the approval of AHCA/NCAL, prior to confirmation.• Make your hotel reservations early to ensure availability.
