COPD Facts: • Chronic obstructive pulmonary disease (COPD) is the only major cause of death with an increasing prevalence • COPD is expected to be the third leading cause of death worldwide by 2030 • Total EU COPD expenditure for outpatient care is around € 4,7 bn per year • Total EU COPD expenditure for inpatient care is around €2,9 bn per year • The NHS currently spends £800m a year on COPD • COPD is now the second largest cause of A&E hospital admission in the UK The challenge The Marymead and Knebworth Medical Practice has around 250 patients (2.2% prevalence rate) on its COPD register. A large amount of GP time is dedicated to providing care for COPD patients. Encouraging patients to self manage their own condition could reduce unnecessary GP consultation time. The local Patient Participation Group, wished to make better use of digital technologies to enable care closer to home. Medtronic was challenged to provide evidence that providing COPD patients with home based remote monitoring could release time for GPs to provide alternative models of care. Further information contact: Martin Clark Director of Patient Managed Services EMEAC Mob+44(0)7867318442 [email protected] The solution and outcomes Solution Medtronic provided home –based monitoring technology to support a four month proof-of concept involving 50 COPD patients Patients were fully engaged with the programme through a series of awareness events Training in the use of the technology was also provided Programme and change management support was also provided by Medtronic Outcomes 94% of the cohort would recommend the Medtronic solution to other COPD patients 65% of the cohort reported improved confidence in self managing their trigger symptoms GP visits within the cohort has reduced by 24% during the period of the PoC Testimonials “I have worked with my Practice Manager, our Patient Participation Group and the Medtronic team throughout this COPD PoC. Having spoken to the patients, the majority felt it was highly beneficial to them to have the equipment at home and to recognise how their symptoms related to their clinical readings, coupled with input from a nurse and/or doctor.
