Dear colleagues, this time I am writing something beyond my experience in CCPS or APA. As an organization APA has an international presence. Coincidentally I had opportunity to meet many psychiatrists from India during various APA annual meetings. I have had some idea of the current standards of psychiatric practice and status of mental health services in India, but I was appalled by the statistics of recent suicide deaths in India by the farmers and adolescents (Per WHO, highest in the world). During my recent visit to India in November 2015, I had discussions with some of my colleagues there and did some fact searching work, including journal articles, public records, PBS and NY Times. Statistics show that in USA there is great variability in availability of psychiatric-services depending on the state and the region. Here it can be one psychiatrist for each 4000-5000 population in certain states to one psychiatrist for each 12,000-14,000 population in some other states. Where as in India calculation shows at one psychiatrist for about 340,000 population, mere 5000-6000 psychiatrists for 1.2 billion people with 100 million suffering with some degree of mental illness, 20 million being seriously ill. there. Psychotherapy and social services are rarely

available. Specialized psychiatric services such as Child

or Geriatric psychiatry are unthinkable. Another great

challenge is how religious beliefs and government rules

interfere with these services focusing on the

psychosocial aspect of mental illness and its

management.

Well, the purpose of this note is to draw every

one's attention, understand the real goal of APA and our

extended duty towards the services for mentally ill,

beyond the borders of US.

After fifty years of practice in child, adult and family psychiatry I have finally retired from full-time work. However, as a result of that fifty years’ experience I believe I have discovered a new approach to insight-oriented psychotherapy. This development, I hope, will allow both deeper and earlier work to be done while at the same time shorten the course of the actual treatment.

Accordingly, I have elected to continue work for at least one day a week. I am especially interested in helping people who have been deemed "treatment failures" or "treatment resistant" and not made any improvement with a combination of regular psychotherapy and medication.

I am seeking referrals of these "failures" to ascertain if this new approach has any wide• spread usefulness. The patients have to at least, be willing to meet with me once to explore their situation. There will be no charge for the initial meeting and the usual charges will apply if they elect to continue.

Please screen out schizophrenic or psychotically depressed patients or those with violent/suicidal severe behavioral symptoms. I will have to refer those who need hospitalization to an in-patient psychiatrist for their stay in the hospital.

I will be glad to discuss more of this with you, if you wish, and appreciate your consideration in referring these difficult patients. Sincerely,

We here in Fresno have been working for the last few years to expand, enhance, and improve our care delivery to

patients in both primary care and hospital medicine. The two services we have assigned to these tasks are Integrated

Mental Health (IMH) and Psychosomatic Medicine (PM). Both of these teams have been extremely successful,

garnering several accolades and awards in the last 5 years for the high quality of consultation and teaching. Most

recently, these include the Family Medicine teaching award in 2015 for IMH, and the distinction of having the

highest student rating of all away rotations from the UCSF medical students in PM. These successes have led to

increased focus on and opportunities within our services.

In Psychosomatic Medicine we have extended our busy service with a tele-psychiatry service which now sees

the overflow patients from the PM team. As technology is increasingly being used to leverage our diminishing

mental health workforce, we recognize the importance of working in collaboration with providers at a distance for

the needs of the population. This extends our reach as providers and teachers, and is also a necessary skill set to

teach to our mental health trainees. Additionally, after more than 15 years running the service, our Psychosomatic

Medicine director, Hoyle Leigh, has stepped down from leading the service to go on sabbatical to prepare his

international textbook of psychiatry. Upon his return he will begin transitioning into retirement. This opens up the

position of director of Psychosomatic Medicine for UCSF Fresno. Applications are being accepted at this time.

We are also now in a position to expand our IMH service to two full time physician positions, opening up a

recruitment this month for a co-director of UCSF Fresno Integrated Mental Health. This position would be focused

primarily on supervising and teaching primary care residents and medical students in the evaluation and treatment of

mental health disorders. Our service works intimately with the residents in Family and Community Medicine,

Internal Medicine, Pediatrics, and OB GYN, as well as Psychiatry. We work in collaboration with, and help manage

through weekly meetings, a team of care managers who administer evidence based brief therapy as well as provide

psychiatric social work and brief case management.

The IMH service was established in late 2013 as a collaboration between Community Medical Center, UCSF

Fresno, and Fresno Department of Behavioral Health. The goal was to use the great work done at the University of

Washington on the I.M.P.A.C.T. research study and apply it to our primary care teaching center for the underserved.

True to the title of the study, we were hoping to “improve mood and provide access to collaborative treatment” for

our medical patients who may or may not have access to these services elsewhere. The excellent results of the first

12 months were presented at the APA conference this year in Toronto. Below are our results from the first 18

months of the program, building on the successes of the year before.

In the first 12 months of the program we were able to decrease the PHQ-9 depression scores by an average of

2.02 points (25%) with a peak in Family Medicine where scores decrease by 4.27 points (44%). In the following 6

months of the program (18 months total) we were able to decrease the PHQ-9 depression scores by an additional

average of 1.80 points overall (an additional 11%) with a peak in Family Medicine where scores decrease by another

1.74 points (18%). Total decrease in PHQ-9 depression scores is an average of 3.09 points overall (36%) with a peak

in Family Medicine where scores decreased by 6.01 points (62%).
