Deducted by the company from its monthly contributionsfor leaves and indemnities due for that month.(percentage of the contributions is 0,85% applied to grossstock)

If any amounts are yet to be recovered, the company willstart recovery process from FNUASS, using a “recovery ofmedical amounts file”. The file together with all annexesmust deposited to the Health Insurance House withinmaximum of 90 calendar days since the company isentitled to recover the amounts.

o A written request (template Annex. 12 rules of the OUG 158/2005)o Centralized report of sick leave certificates (template Annex. 18rules for applying the EGO 158/2005)o Copy of 112 Tax Return for the exact month the company isdrafting the request (Annex 1 and Annex 1.1_Company)o Filled in medical leave certificates (2 copies -pink), for the monththe company is drafting the requesto Detailed report containing status of medical leave indemnities - itis obtained from the IRS (Account 439).
