Dear HeliOffshore members,

It is our pleasure to welcome you to Budapest and to the third HeliOffshore Conference and Annual General Meeting.

The deep commitment to safety from across the global offshore helicopter industry is demonstrated by the attendance of so many of you; our industry’s most senior leaders. It is also demonstrated by the growth – in numbers and breadth – of membership in HeliOffshore.

With more than 100 member organisations, we have the critical mass to continue to drive improvements in safety performance across the rotary-wing system, worldwide.

Over the past two years, we have defined our shared strategy and achieved strong alignment behind it. As a result, we are already reducing duplication and focusing our joint efforts on activities that will make the greatest difference to safety at the frontline.

Today, we see an increasing potential to deepen our delivery and advance our shared safety strategy to the benefit of those who work offshore. Our third conference is an opportunity to come together to assess, refine, and accelerate our activities, and to establish measurement and feedback loops so that we can determine the impact of our actions.

We have listened to your feedback and this year’s agenda dedicates more time for discussion with your industry peers. As a result of the many positive outcomes from our 2016 conference, we are confident that once again our conference will deliver commitments that will further

William Edmonds Chiles Chairman of the Board, HeliOffshore

Advisor and CEO Emeritus to Bristow Group until July 2016 and Managing Partner of Pelican Energy Partners, a private equity firm investing in the

energy service sector.

Bill joined Bristow in 2004 after a 32-year career in offshore oil and gas drilling. Starting with Western Oceanic Inc. in 1972, he rose to Vice President (VP) of Domestic Operations in the Gulf of Mexico before leaving to cofound Chiles Drilling Company (later Chiles Offshore Corporation). In 1992, Bill founded Southwestern Offshore Corporation.

In 1997, he founded Chiles Offshore Inc., specialising in ultrapremium jack-up offshore drilling units (it later merged into ENSCO International Inc., with Bill as VP Business Development). He is on the Boards of Gulf Island Fabricators Inc., the National Ocean Industries Association, and the Offshore Energy Museum.

Andrea Cicero Managing Director, Babcock Mission Critical Services

Andrea became Managing Director in April 2017, he was formally Chief Operating Officer between 2016-2017

and Chief Financial Officer between 2010-2016. He joined from INAER where he was Group Controller from 2006. Previously he served as CFO for Logic Control (Sage Spain) in Spain, and as Financial Controller of Japan Eurotex Ltd in London.

Christopher S. Bradshaw President and CEO, Era Group Inc.

Chris was appointed President and Chief Executive Officer of Era in November 2014, after serving as Acting Chief Executive Officer from August

2014 through November 2014. Chris also served as Chief Financial Officer of Era from October 2012 to September 2015, having joined Era as Executive Vice President and CFO in October 2012.

Prior to joining Era, Chris spent 13 years in the financial services industry, most recently as a Managing Partner and Chief Financial Officer of U.S. Capital Advisors LLC, an independent financial advisory firm. Prior to co-founding USCA, Chris was an Executive Director in the Global Energy Group at UBS Investment Bank.

Chris also worked in the energy investment banking groups at Morgan Stanley & Co. and PaineWebber Inc. Chris received a Bachelor of Arts in Economics and Government from Dartmouth College.

General Tan Sri Muhammad Ismail Bin Jamaluddin CEO, Weststar Aviation Services

General Ismail plans and directs all aspects of business policy objectives, initiatives, safety and ensures the

profitability and efficiency of Weststar. He is also the company’s Accountable Manager.

Prior to joining Weststar in 2011, Gen. Ismail served in the Malaysian Armed Forces for almost 40 years. He rose from 2nd Lieutenant to a Four-Star General (Chief of Army) and retired in 2010 as the Chief of Army.

Gen. Ismail sits on the Board of United Offshore Aviation Company Limited, PT Weststar Aviation Indonesia, Coca-Cola Bottlers (Malaysia) Sdn Bhd, and Coca-Cola Refreshments (Malaysia) Sdn Bhd.

Gretchen Haskins CEO, HeliOffshore

Gretchen is an aviation industry leader in safety performance improvement and an internationally-recognised expert in human factors. She has

served on the Board of the UK Civil Aviation Authority as Group Director of Safety, overseeing aviation safety in the UK (including airlines, aerodromes, air traffic, airworthiness and personnel). Prior to that, Gretchen was Group Director of Safety at NATS where she championed activities to support frontline operational safety.

Gretchen previously worked in nuclear certification and safety of intercontinental ballistic missiles, joint airworthiness trials for military aircraft, defence system design – and as an expert advisor to NATO on human performance and safety critical systems. She has a strong aviation background, and has flown jet and piston aircraft in the U.S. Air Force.

Jonathan Baliff President and CEO, Bristow Group

Jonathan joined Bristow Group as SVP and CFO in 2010. Prior to Bristow, he was Executive VP Strategy at NRG Energy, where he led the development

and implementation of corporate strategy, acquisitions and business alliances. Prior to NRG, he was MD of Credit

Suisse’s Global Energy Group, where he advised energy companies on M&A and project and corporate finance. Baliff started his career at Standard & Poor’s and then J.P. Morgan’s Natural Resources Group. He served active duty in the US Air Force from 1985 to 1993 flying the F-4G Phantom, including the first combat missions during the first Gulf War. He retired with the rank of Captain.

Capt. Josiah Choms PhD MNIM Managing Director and Accountable Manager, Caverton Helicopters

Josiah began his aviation career in 1990. His experience in aviation included a stint with Mission Aviation Fellowship

in Nairobi, Kenya as an aircraft engineer. He was a pilot, Training Captain and Manager with Bristow, and held the post of Chief Pilot with Prime Air, flying business jets internationally.

Josiah holds a multi-disciplinary PhD and is a member of several professional bodies including the Nigeria Institute of Management, the Association of African Studies (UK), the Royal African Society, and the National Association of Pilots and Engineers.

Karl Fessenden President and CEO, CHC Helicopter

Karl was named president and CEO of CHC Helicopter and a Board member of parent company, CHC Group, in 2015. He came from GE, where for

over 20 years he amassed a record of strong leadership in building industrial-services businesses, as well as process discipline around resource and capital allocation. He led

multiple global-service business units at GE Energy and GE Aviation. Most recently he directed GE’s integration of the Alstom power-generation business and, prior to that, he managed GE’s $8 billion Power Generation Services unit. His background includes 15 years in the aviation sector with GE and Pratt & Whitney.

Lance Bospflug President and Chief Operations Officer, PHI Inc

Since joining PHI in 2000, Lance has reshaped the leadership team while evolving the strategic direction,

operational focus and safety culture through innovative programs such as Destination Zero, which focuses on human factors. Lance has worked to grow PHI’s base of operations and to extend its position as world leader in rotor wing aviation.

Previously, he spent 25 years in various service-related industries, serving as President and Chief Executive Officer of T. L. James & Company, Inc., (a diversified marine dredging, construction, and timber company operating extensively on the Gulf Coast); and has also held various management positions with the Morrison Knudsen Corporation.

Lance has a Business degree from the University of Jamestown and a Masters of Business Administration from the University of South Dakota, and is a Chartered Financial Analyst. He is also a fixed wing pilot.

Andrew Coleman Commercial Lead, GE Aviation

Andrew leads the digital business for GE’s Aviation business unit. In this role he is chartered with creating new and recurring software and services revenue

streams for the aviation industry. GE Digital supports customers globally with a broad range of industrial Internet applications, from fuel efficiency, recovery, flight analytics, and asset performance management. Andrew is responsible for global sales, marketing, consulting, alliances, customer success, and customer support.

Prior to this, Andrew held leadership positions in diverse areas of NCR & Teradata (Sales, Product Management, Marketing, Finance, and Strategic Alliances) and has a track record of growth, new market penetration, employee achievement, solution development, value creation, and transformational go-to-market practices. Andrew has a Bachelor of Arts from Asbury University, and an MBA with a finance concentration from Xavier University.

Dan Rosenthal President and CEO, Milestone Aviation

Daniel Rosenthal oversees sales, finance, operations and strategy for global leader in helicopter leasing,

Milestone Aviation. Dan served most recently as EVP in the NetJets Inc. Office of the Chairman, where he was responsible for multiple departments, including planning and logistics, owner and employee services, shared IT, and government relations and corporate responsibility. Prior to NetJets, Dan worked as an attorney at Williams and Connolly LLP in Washington, D.C.

Dan serves on the Board of The James Cancer Hospital and Solove Research Institute Foundation, is a member of the Columbus Partnership and has been chairman of the Board of Pelotonia since its inception in 2008. Pelotonia has become the second-largest run/walk/ride fundraiser in the United States, and its riders have raised more than $130 million in eight years. Dan previously served as chairman of Mayor Coleman’s JET Task Force related to airline service and economic development. Dan also served as a member of the Ohio Business Roundtable and was an inaugural co-chair of Columbus 2020!, the Partnership’s ground-breaking economic development effort.

A graduate of Yale University, Dan holds a BA in history and international studies, and a JD magna cum laude from Duke University School of Law. He is fluent in Spanish.

David Balevic Senior Vice President Engineering & Operations, CHC Helicopter

Mr. Balevic was appointed SVP Engineering & Operations in May 2015, coming to CHC with 26 years of

engineering leadership experience at General Electric. During his time at GE, Mr. Balevic held progressive roles in aviation, power generation and oil & gas. Before joining CHC, Mr. Balevic led engineering for the drilling and surface division at GE Oil & Gas. There he was responsible for all aspects of technology development, new product design, requisition engineering, field support, and customer technical engagement. Mr. Balevic holds a Master’s of Science in Aerospace Engineering from the University of Cincinnati and a Bachelor’s of Science in Mechanical Engineering from Clarkson University.

Dik Gregory BA (Hons), CPsychol, AFBPsS Director, GS Partnerships

Dik is a BPS Chartered Psychologist with extensive experience in government and commercial sectors. He has published

journal articles and book chapters on the use of advanced technology for training and operational support. From 1978, Dik worked for the UK Civil Service for 10 years, including more than three years overseas as an exchange scientist. On his return in 1986, he was promoted to Principal Psychologist (Grade 7), and appointed Deputy Superintendent of the MoD’s Applied Psychology Unit, Teddington.

Dik left the Civil Service in 1988 to work in the commercial sector. For over 25 years he has been a Director of his own company, providing human factors and organisational consultancy to business and Government. He has been the principal investigator on a wide variety of safety-critical projects, all aimed at creating clear guidance and practical support for decision makers working under increasing demands for greater resilience within complex systems. Among his many clients, Dik has wide experience of the rail, air traffic control and maritime sectors, for which he has written ground-breaking guidance on human behaviour for everyday use in operational settings.

Duncan Trapp Vice President, Safety & Quality, CHC Helicopter

Mr. Trapp was named vice president of safety and quality for CHC Helicopter in 2011, bringing an extensive record of

executive-level leadership in safety, policy and aviation to the post. In this role, he reports directly to CHC President and CEO Karl Fessenden and is the executive responsible for the company’s strategic and operational safety profile. Mr. Trapp joined CHC in 2007, as the accident prevention and flight safety programme manager for all UK operations. He progressed through a series of European safety leadership appointments before being named to his current global post. Prior to joining CHC, Mr. Trapp served as a squadron commander in the UK’s Royal Air Force and a policy and defence relations officer. He is a trained accident investigator, an accomplished aviator, and a qualified Crew Resource Management instructor. Mr. Trapp holds a Master’s degree in Defence Studies from King’s College in London.

Hank Williams Chief Operating Officer, Cougar Helicopters

Hank has 25 years of experience in the transportation industry with the last 20 years in the aviation sector.

Hank joined Cougar Helicopters in 1997 as Passenger Movements Coordinator, quickly moving to Base Operations Manager (St. John’s) before being appointed as General Manager East Coast Operations. In 2010, Hank was appointed GM Cougar Helicopters, with overall responsibility for all of Cougar’s operations

globally which comprises all of VIH Aviation Group’s IFR operations. A hands-on helicopter manager, Hank has completed the HAI Helicopter Operator Management Course and is a member of London-based HeliOffshore. In 2016 Hank was elected as a Director with the Newfoundland Offshore Industry Association.

John McColl Head of Airworthiness, UK CAA

The Civil Aviation Authority is the UK’s specialist aviation regulator. John McColl is the Head of Airworthiness and manages a team of airworthiness

surveyors across the UK overseeing design, production, maintenance and continued / continuing airworthiness activities.

John started his career with Rolls-Royce at the Hillington facility in Glasgow as a Graduate Trainee. After spending time in production, maintenance, planning, quality, design and development, John’s first position was in quality at the Rolls-Royce engine MRO facility in East Kilbride. After a few years on the shop floor John moved to the Design and Stress team and spent a total of 15 years at Rolls-Royce in a number of management positions.

John has been with the CAA for 21 years, he started in the Propulsion department and was involved in the certification and validation of new engine designs, modifications, repairs, continued airworthiness and organisation approvals. John moved on to take on other roles with propellers and helicopter transmissions before taking up a number of management positions: Head of Organisation Approval and Propulsion, Head of Aircraft Certification, Head of Airworthiness Evaluation and Surveillance, CAA’s Chief Surveyor and is now Head of Airworthiness.

Jonathan Phillips Director, Air Transport Safety, Shell Aircraft Ltd

Jonathan heads up Shell’s Air Transport Safety Group, is Group Technical Authority (Air Transport) for Shell and

a Company Director for Shell Aircraft Limited. He is also deputy chair for the IOGP Aviation Sub Committee.

Michelle Handforth Managing Director, Babcock MCS Offshore

Michelle Handforth was appointed Managing Director of the Offshore Helicopter business in April 2015. Ms

Handforth has 20 years Board and international leadership experience in global, multi-site businesses, primarily in the aviation, rail, maritime, financial services, oil and gas industries. She has extensive experience in transitioning businesses from the public to private sector with a successful record of delivering business results in heavily regulated, unionised and safety critical environments.

Ms Handforth has served on the Boards of First Choice Holidays, Sea Containers (Rail) former operator of the East Coast Rail franchise and Orient Express Group, Aviva a major UK insurance company where she launched Aviva Direct, retailing financial services to 25 million UK customers, Veolia Transdev one of the largest passenger transport companies in the world and was latterly acting CEO of Sydney Ferries based in Sydney Australia.

She is a former member of the Board of National Rail Enquiries and Association of Train Operating Companies (ATOC). Whilst living and working in Australia she was a

Whilst in India she established a community care and education programme for orphaned children in Mumbai and has raised money for incubators at the premature baby unit at Manchester Royal Infirmary.

Dr Paul Shanahan GS Partnership

Dr Paul Shanahan has a PhD in cognitive psychology and is a member of the British Psychological Society as well as a Registered Practitioner of the Chartered

Institute for Ergonomics and Human Factors. He was an Associate Director of Electronic Facilities Design Ltd and a Director of Man-Computer Consultants. Paul has worked as an independent consultant and researcher on human and organisational factors in complex systems for over 30 years.

Paul has contributed to the specification and design of many complex decision-making systems. He has been the principal investigator on a large number of human behaviour research projects and has pioneered new techniques for the analysis of a wide range of organisational data. Many of his projects have clarified the contribution of human performance within complex, adaptive systems – a theme which he has continued across health, rail, maritime and air traffic control sectors.

Robbert Strating CEO Hevilift Australia, Director Operations Hevilift Group

Robbert joined the Royal Netherlands Air Force after High School and studied Military Science at the Royal Military

Academy 1987-1991. After receiving his Commission he joined the Euro NATO Joint Jet Pilot Training program and was trained as an F16 pilot. He served with the RNLAF for 17 years. Between 2010 and 2013 he was assigned to the United States Air Force as an exchange officer and he served as an Operations Director for the 77th Fighter Squadron. During the last three years of his military career Robbert served as a project manager for the F-35 systems design and development team. Robbert retired in 2007 from the RNLAF as a Lieutenant Colonel, with more than 2000 hours on the F16 as a pilot and weapons instructor.

After retiring, he joined Shell Aircraft as an aviation advisor before joining TUIFly Netherlands as Director Flight Operations and Accountable Manager. While at TUIFly Robbert was responsible for the safety, compliancy and financial wellbeing of this charter airline which operated 10 Boeing 737/767 aircraft. Before joining Hevilift in 2015 Robbert worked for two years at Caverton Helicopters as Operations Director. In his current role Robbert is responsible for the Hevilift Group Operations in Malaysia, Myanmar, PNG and Indonesia. As the CEO of Hevilift Australia he was tasked to setup a new charter airline in Australia.

Robbert is a Harvard Business School Alumni, he has a Bachelor in Aerospace Engineering and a Master in Security and Defense. He resides in Brisbane and is married to Noelle. Together they have two teenage children.

Robert Pendle Director of Global Maintenance Operations, Bristow Group

Rob Pendle joined Bristow Group in 2014 as Director of Global Maintenance Operations based in Houston, Texas. Rob

joined from Cobham, where he was General Manager of their European Aviation Services business unit. Prior to that, he was Vice President Maintenance and Technical Services for CHC Helicopter and prior to that held a number of senior positions with British Airways. Rob is a Chartered Engineer and a licensed pilot and has served on the Council of the Royal Aeronautical Society and CBI Wales.

Simon Stewart Safety Director, Babcock Mission Critical Services

Simon joined Babcock MCS in February 2012 as Director of Safety, Compliance and Head of Training. His safety

background extends from an initial 12 years with EasyJet where he involved himself within safety leadership; previously responsible for the implementation and development of the EasyJet Safety Management System (SMS) and Fatigue Risk Management System (FRMS). Simon is recognized as an authority on safety in the aviation industry. His flying experience of over 8500 hours covers both civil and military. He was an EasyJet B737NG Captain and prior to his airline career an Australian Army Aviator (rotary and fixed wing). He is a graduate of the Royal Air Force Central Flying School and an A2 Qualified Flying Instructor.

Steve Predmore VP and Chief Safety Officer, Bristow Group

Steve is VP and Chief Safety Officer for Bristow Group, providing strategic leadership and technical expertise

on safety, health and the environment. Steve oversees Bristow’s delivery of its Safety Management System (SMS) globally, and is responsible for the company’s industry leadership in safety, as demonstrated by its Target Zero safety culture. His long tenure in the aviation industry includes 11 years with JetBlue Airways Corporation as VP and Chief Safety Officer, and six years with Delta Air Lines as Director Safety Performance & Quality. After successive leadership roles with FAA Aviation Rulemaking Committees, the Airlines for America (A4A) Safety Council, and the Global Aviation Information Network (GAIN), Steve was appointed by the US Secretary of Transportation to serve on the Management Advisory Committee to the FAA Administrator. He holds a PhD in Social Psychology from the University of Texas, and a Bachelor’s degree in Psychology from Drake University.

Steve Hawkes Group Head, Global Aviation Services, BP

Steve Hawkes is the Group Head of Global Aviation Services for BP and is responsible for setting strategy

regarding the management of aviation risk and efficiencies across BP’s global aviation portfolio.

Steve holds a BSc and an MBA degree, and is a Certified Aviation Manager. A pilot experienced in global operations, he has personally accrued over 11,000 hours

Tim Rolfe Director Safety Standards and Integration, Bristow Group

Tim graduated from Bristow’s in-house pilot training course in 1990 and flew S61N, AS332L and S92

helicopters in support of oil and gas, Search & Rescue (SAR), military, and VIP transport customers. Amassing 8,500 hours and qualifying as Type Rating Examiner and CRM Instructor Examiner, Rolfe has held roles in Operations, Pilot Training and Safety management. In April 2017, he moved into a newly developed role within the Global Safety Team as Director Safety Standards and Integration with responsibility for standardising operational risk management processes, across existing and new operating regions and partnerships.

With undergraduate degrees in Genetics/Biochemistry and Psychology, and an MSc in Human Factors and Safety Assessment from Cranfield University, he maintains a personal and professional interest in behavioural sciences and their application in aviation. Tim is a member of the HeliOffshore Safety Steering Committee and co-leads the Operational Effectiveness workstream.

Thomas C. Knudson President, Tom Knudson Interests

An experienced corporate director who serves as the Chairman of Bristow Group Inc, Mr. Knudson formerly served as Chairman of Midstates Petroleum

Inc., lead director of Natco Group Inc. and as a Director of Williams Partners Inc. and MDU Resources Inc.

He retired from ConocoPhillips in 2004. His diverse career included engineering, operations, business development and commercial assignments across Conoco’s businesses. Among his more significant assignments were as CEO of DuPont Scandinavia; and vice president and general manager for Conoco’s midstream natural gas business. In 1997, he became chairman of Conoco Exploration Production Europe Limited. He returned to Houston in July 2000, to become SVP for HR, information management and communications, serving prior to his retirement as a member of the management and executive committees of both Conoco and ConocoPhillips.

Knudson received a bachelor’s degree in aerospace engineering from the US Naval Academy in 1967 and a master’s degree in aerospace engineering from the US Naval Postgraduate School in 1968. Following graduation, he served as a naval aviator flying 170 missions in S.E. Asia and was selected two years early for Lieutenant Commander, his rank at the time of his discharge in 1974.

Annette Adkins Executive Assistant to the CEO

Annette joined the HeliOffshore team in February 2017 as Executive Assistant to the CEO. Annette acts as a point of contact and provides administrative

support to the HeliOffshore team on a day to day basis. Annette previously worked for BAE Systems plc for eight years and has experience in both the public and private sectors supporting at CEO and Director level.

Francois Lassale Operations Director

Before joining HeliOffshore, Francois was the Managing Director for a firm in the USA bringing turnkey solutions to the industry, and before that he

was responsible for safety of the President of the United Arab Emirates. Francois has a military background in the South African army and Royal Air Force, flying both fixed wing and rotary wing. Since leaving the military he flew in airline, freight, VIP and Head of State operations. Francois has been an instructor, TRI, TRE and CRMI and has been on the Flight Safety Foundation business aviation Board for more than twelve years. Francois is an Upper Freeman for the Honourable Company of Air Pilots. He serves as Vice Chairman of the European Helicopter Association, Vice Chairman of the International Association of Aeronautical Flight Auditors, and is a certified IS-BAO auditor.

Katherine Hamblin Company Secretary and Membership Manager

Kat helps the HeliOffshore team to optimise the efficiency of operational activities to enhance safety. She

plays a key role in communicating with HeliOffshore members and stakeholders to support the delivery of safety and workstream activities, managing the membership, maintaining and administering the online and collaboration tools to share safety information, and carrying out secretary and treasury functions in support of the HeliOffshore Board. Kat has 10 years’ experience in the energy sector, specialising in communications, event management and stakeholder engagement. These skills, combined with her ability to build and maintain professional relationships, enable her to support the team effectively. Kat has a Bachelor’s of Science degree in Psychology.

Scott Carmichael Interim Project Manager, System Reliability & Resilience

Scott’s role was created to support the System Reliability & Resilience workstream leaders. He has 26 years’

experience in the aviation industry, which began with a role as an apprentice aircraft engineer in the fixed wing industry working on piston engine, turbo-prop, regional jet and narrow body jets. Scott gained his engineering licenses in the categories of B1.1, B1.2 and C Certification, as well as college diplomas in Mechanical Engineering and Aeronautical Engineering.

After 13 years of fixed wing work, Scott joined the offshore helicopter industry. Based in Aberdeen, Scott held roles in safety, compliance, and continuing airworthiness, holding the Part M Post Holder position for CHC (Scotia) for five years, eventually becoming the global Fleet Team Director for the Eurocopter and Sikorsky Fleet. In 2014 Scott took the role of Head of Safety and Compliance for Cobham Aviation Service, Special Missions. In 2015, he took a year out to complete an MSc Air Transport Management with Cranfield University.
