HEBER PUBLIC UTILITY DISTRICT REPORT TO BOARD OF DIRECTORS MEETING DATE: May 18, 2017 FROM: Laura Fischer, General Manager SUBJECT: Award the Contract for Custodial Services to Mylo Corporate Maintenance & Janitorial Services ISSUE: Shall the Board of Directors award the contract to perform janitorial services at the HPUD Office to Mylo Corporate Maintenance & Janitorial Services for an amount not to exceed $298 per month? GENERAL MANAGER’S RECOMMENDATION: It is recommended that the Board of Directors award the contract for janitorial services for the HPUD offices to Mylo Janitor Services. FISCAL IMPACT: Monthly service cost not to exceed $298.00. Included in prior year budget and in FY 2017-18 proposed budget. DISCUSSION: Staff was unsatisfied with our janitorial service provider. We requested quotes from several janitorial service providers and have determined that Mylo Corporate Maintenance and Janitorial Services had the best prices and was the most qualified to perform the needed services. CONCLUSION: Staff recommends that the Board award the contract for janitorial services for the HPUD offices to Mylo Janitor Services. ALTERNATIVES: 1) Do not award the janitorial service contract to Mylo Janitorial Services. 2) Direct staff to re-advertise and get additional quotes. 3) Provide alternate direction to staff regarding the janitorial service needs at the HPUD office. INJURY AND ILLNESS PREVENTION PROGRAM In order to maintain a safe and healthful work environment Mylo Janitorial has devel­oped this Injury and Illness Prevention Program for all employees to follow. This docu­ment describes the goals, statutory authority, and the responsibilities of all employees under the Program. It addresses Compliance, Hazard Identification, Accident Investiga­tion, Hazard Mitigation, Training, Hazard Communication, and Program Documentation. By making employee safety a high priority for every employee we can reduce injuries and illnesses, increase productivity, and promote a safer and healthier environment for all Heber Public Utility District Customers, Employees and Mylo Janitorial Inc. person­nel.

GOALS

Diligent implementation of this program will reap many benefits for all Heber Public Utili­

ty District Customers, Employees and Mylo Janitorial Inc. personnel.

• Reduce worker's compensation claims and costs.

• Improve efficiency by reducing the time spent replacing or reassigning injured em­

ployees, as well as reduce the need to find and train replacement employees.

• Improve employee morale and efficiency as employees see that their safety is im­

portant to management.

• Minimize the potential for penalties assessed by various enforcement agencies by

maintaining compliance with Health and Safety Codes.

COMPLIANCE

Compliance with this Injury and Illness prevention Program will be achieved in the fol­lowing manner:

1 . Managers and Supervisors will set positive examples for working safely and require

that all staff under their direction work safely.

2. Managers and Supervisors will use all disciplinary procedures available to them to

ensure that employees follow established safety policies and procedures. Performance

evaluations, verbal counseling, written warnings, and other forms of disciplinary action are available.

3. Managers and Supervisors will identify the resources necessary to provide a safe

work environment for their employees and include them in budget requests.

4. Managers and Supervisors will establish appropriate means of recognition for em­

ployees who demonstrate safe work practices.

Mylo Janitorial Inc. has developed this comprehensive Injury and Illness Prevention

Program to enhance the health and safety of Heber Public District Customers, Employees

and Mylo Janitorial Inc. personnel.

SCHEDULED SAFETY INSPECTIONS

Upon initial implementation of this Program inspection of all work areas will be conduct­

ed. All inspections will be documented using the attached forms or equivalent.

1 . Semi-annual inspection of all potentially hazardous areas will be conducted to de­

tect and eliminate any hazardous condition that may exist.

UNSCHEDULED SAFETY INSPECTIONS

1. Additional safety inspections will be conducted whenever new equipment or changes

in procedures are introduced into the workplace that presents new hazards.

2. Safety reviews will be conducted when occupational accidents occur to identify and

correct hazards that may have contributed to the accident.

ACCIDENT INVESTIGATIONS

Supervisors and Managers will investigate all accidents, injuries, occupational illnesses,

and near-miss incidents to identify the root cause. Appropriate repairs or procedural

changes will be implemented promptly to correct the hazards implicated in these events.

To ensure timely accounting for Worker's Compensation procedures, both employee

and supervisor must complete their respective portions on the Report of Employee Inju­

ry/ Exposure Form

TRAINING

Effective dissemination of safety information lies at the very heart of a successful Injury

and Illness Prevention Program. All employees must be trained in general safe work

practices. In addition, specific instruction with respect to hazards unique to each em­

ployee's job assignment will be provided.

GENERAL SAFE WORK PRACTICES

At a minimum, all employees will be trained in the following:

1. Fire Safety, Evacuation, and Emergency Procedures

2. Hazard Communications (Use of Material Safety Data Sheets)

SPECIFIC SAFE WORK PRACTICES

In addition to this general training, each employee will be instructed how to protect

themselves from the hazards specific to their individual job duties. At a minimum this

entails how to use workplace equipment, safe handling of hazardous material and use

of personal protective equipment. Training must be completed before beginning to work

and assigned equipment, and whenever new hazards or changes in procedures are im­

plemented.

The Operation Manager is responsible for providing managers and supervisors with the

training necessary to familiarize themselves with the safety and health hazards their

employee are exposing to. It is responsibility of the managers and supervisors to know

the hazards related to his or hers employee job task, and ensure they receive appropri­

ate training.

1 . Supervisor and Managers will ensure that all employees receive general and specific job training prior to initial or new job assignment.

2. Supervisors will ensure that employees are trained whenever new substances, pro­

cess, procedures, or equipment are introduced to the work place which will create a new

hazard. Training must also be given when new or previously unrecognized hazards are brought to a supervisor's attention.
