Individuals who qualify can enroll in Medicaid, Essential Plan, and Child Health Plus year round. This fall, there will also be the opportunity for individuals to enroll in Qualified Health Plans and Medicare Advantage. See details below:

A substance use disorder (SUD) is a chronic health condition. It occurs when people repeatedly use a substance, damaging their health or losing the ability to meet responsibilities at work, school, or home. The use of alcohol, prescription drugs such as opioids, and illegal drugs such as heroin can become a SUD.

If you think you may have a SUD, speak with your primary care provider. Help is also available by calling 1-877-8-HOPENY (1-877-846-7369). The New York State Office of Alcoholism and Substance Abuse Services offers help and guidance 24 hours a day, seven days a week. All calls are toll-free, anonymous, and confidential. If you need help right away, call 911.

Have You Been to the Hospital? Visit Your PCP.

If you have been hospitalized or visited the emergency room, visiting your primary care provider (PCP) within a week of your discharge is very important. Call your PCP to let them know you’ve been discharged, and schedule a visit. If the hospital scheduled the visit for you, please keep your appointment Seeing your PCP is key for your recovery. Your PCP can give you valuable follow-up care, and help monitor your progress.

Members can get a FREE flu shot at a network pharmacy. Find one near you at

LEADKeep lead dust away from children. Breathing or swallowing lead dust

can harm children’s growth, behavior, and ability to learn. It can be found in lead paint, often used in houses built before 1978; tap water, mainly when using hot water; and soil, when car fumes mix with dirt on the ground.

MEDICATIONSStore all medications out of reach of children. If a medication is no longer needed, throw it away. Medications taken when not

needed can be harmful.

CLEANING PRODUCTSStore all cleaning products out of

reach of children. Also, never put them in food or drinking containers. This can confuse children. Cleaning products

can be dangerous if swallowed.
