Healthy Teeth Healthy Children has provided on

site presentations to 31 PCP offices from 11/2012 – 8/2013

Offered a full day conference in October that

reached 175 participants from 11 offices

217 primary care providers have been educated through individual site visits, Grand Rounds, and

conferences to perform oral screening, risk assessment, referral and fluoride varnish

application

Results of EPIC trainings Evaluation process – ongoing

Three-month and six-month telephone interviews to follow-up

Out of the 38 practices that participated in a presentation in oral health during the first eight months of trainings, 35 of these practices participated in the interviews.

17 out of the 35 practices were implementing at three-months post presentation for a 48.5% implementation rate.

60% implementation rate at six-month follow up

DPW data shows an increase in claims submission for D1206
