Message from the BoardIn 2010, Community Hospital of the Monterey Peninsula was not alone in facing the challenges presented by the rising cost of healthcare, the reduction in govern-ment reimbursements, and the introduction of healthcare reform. Fortunately, due to the proactive development and implementation of a comprehensive strategic plan, Community Hospital maintains a strong financial position and is able to continue its mission to meet the changing healthcare needs of our community.
As part of our progressive plan, in 2010 Community Hospital broke ground to establish an integrated wellness facility in Marina. Peninsula Wellness Center, now open, brings health and wellness programs to the broader community. It provides essential services including a satellite laboratory and radiology services and a second Peninsula Primary Care practice.
In addition to new facilities, Community Hospital is introducing new technology that enables healthcare providers to share health information electronically with each other, their patients, and Community Hospital in a secure and timely manner. With the launch of Peninsula Health Information Link (PHIL), patients are able to create and review their own health records and communicate with their physicians from any computer. Technology has changed the way we shop, bank, and communicate; now it is changing the way healthcare is provided. This important advancement will revolutionize healthcare delivery and result in increased safety and greater efficiency for physicians and will allow us, as patients, easy access to our health information. You will read much more about PHIL and this electronic revolution in this report.
This has been an extraordinary year on many fronts. On behalf of the Board of Trustees, I want to take this opportunity to thank our employees, medical staff, and volunteers for their continued dedication and service to Community Hospital. In healthcare, we often talk about innovation in terms of technology that you can see and feel — a scanner that takes three-dimensional images of the heart in a single beat, a knee joint that will enable a patient to resume an active lifestyle, a medication that can limit the damage caused by stroke.
At Community Hospital, we have also been working on a significant develop-ment that is more behind the scenes, but that is integrated into every aspect of care — an electronic revolution in the way we keep and use our patients’ health records.
We’ve been building the foundation for awhile. But, in 2010, we made major advances by introducing the community to PHIL, Peninsula Health Information Link. Through PHIL, patients, their doctors, and Community Hospital can share, track, communicate, and catalog health-related information in a secure electronic environment. While for most this is a tool for convenience, its promise is much broader: increasing patient safety and efficiency.
Those two goals — increasing safety while improving efficiency — are primary underpinnings of the work we do at Community Hospital. In 2010, we demonstrated that in numerous ways, from our introduction of PHIL to our continuing cost-reduction efforts to our focus on helping our community be as healthy as it can be.
Among the year’s highlights:
❚ Building Peninsula Wellness Center, now open in Marina and providing a state-of-the-art facility for health and fitness with medical oversight
❚ Completing the move to the most advanced digital technology at the Breast Care Center, and establishing a fund to help pay for tests for women who can’t afford them
❚ Signing updated contracts with major insurance companies, securing affordable access to Community Hospital for more than 100,000 community residents
At the same time, in a rapidly changing healthcare environment, we continued to maintain our strong financial health, ensuring that we’ll be here for you in the years to come.

It’s hard to keep track of all the doctors Marilyn Byrne has seen, and all the
tests she’s been through, since her gynecologist discovered a lump in her breast
in November. But each of her healthcare providers — oncologist, surgeon, plastic
surgeon, nutritionist, and everyone she encounters at Community Hospital of
the Monterey Peninsula — has been keeping very careful track of Byrne. In
making a record of her visits and procedures, every office, every lab, every hospital
department she has been to has been writing a part of her history with breast cancer.
That’s how medical histories have been written for years — in bits and pieces, in paper
files and disparate computer systems here, there, and everywhere. Putting those pieces
together is like building a complex jigsaw puzzle: It can be challenging and time-consuming,
but it’s the only way to get the complete picture.
Now, those pieces are beginning to fall into place more efficiently, more cohesively.
Healthcare institutions across the country, from hospitals to doctors’ offices, are working
together to build electronic health record systems designed to support a seamless continuum
of care. Community Hospital is at the vanguard of these efforts, working to improve care,
access, and outcomes, for patients like Byrne.
The approach is a reimagining of healthcare, an electronic revolution aimed at maximizing
safety, effectiveness, and efficiency while minimizing cost, confusion, and inconvenience.
If you’re headed to the hospital for hip replacement, your surgeon and anesthesiologist
need to know what medications you’re taking before you get anywhere near the surgical
suite. If the Emergency department has already X-rayed your broken arm, it would be
a waste of time, money, and radiation exposure for your orthopedic surgeon to X-ray
it again. If your dermatologist has prescribed antibiotics for your badly infected poison
oak rash, your primary care doctor should know the particulars when you show up with
a severe allergic reaction.
Such information has previously been shared through phone call and fax, in-person
interviews and paper files. But the ideal is that every provider who is called upon to treat
a patient should have ready access to all of that patient’s prior, and possibly wide-ranging,
medical history — with the patient’s permission, of course.

That’s beginning to happen in the case of Marilyn Byrne, says her husband Tom.
The couple may be familiar to many; under her maiden name, Getas, she was a news
anchor on KSBW and KION and he was a quarterback for the university of Notre Dame
and briefly, the Detroit Lions.
“My wife’s medical file has gotten pretty large as a result of her current condition,”
Tom Byrne says, “and the transfer of information back and forth between her
care providers has really been streamlined. Everyone has the information they need
to make decisions about her care. It’s been a tremendous opportunity to make sure
Marilyn was getting the best possible care, with no gaps.”
The first half of the story: Creating electronic health records
Community Hospital started down the road toward electronic integration about two
decades ago. “We’ve been automated for the last 20 years at varying levels,” says Tom
McNamara, director of Health Information Technology.
At first, the hospital computerized registraton and billing. Then, computerization began
moving to the clinical side of operations.
“The clinical information system was mostly a results repository at first,” says Charlene
Webber-Schuss, RN, director of Health Information Technology department. “Physicians
used it to review the results of lab tests, X-rays, EKGs, etc.”
And it kept growing. In 2007, doctors began entering orders for their patients directly
into the computer themselves. Community Hospital was among the first 10 percent of
hospitals nationwide to implement the electronic process, called computerized physician
order entry.
“Before that, doctors would write their orders on a piece of paper and a clerical staff
person would enter them into a computer,” Webber-Schuss says.

This change eliminated the age-old problem of doctors’ handwriting and gave doctors
instant access to information that might affect the orders they were writing. For example,
Webber-Schuss says, a doctor ordering a transfusion for a patient will receive an automatic
alert if the patient happens to be a Jehovah’s Witness, whose members don’t accept trans-
fusions for religious reasons. Or a doctor who wants to order a blood-thinning medication
for a patient can quickly view lab results to see if, say, the patient is anemic.
The change has increased a doctor’s role in the process, says Dr. Mike Smith, a Monterey
gastroenterologist and medical director of the medical staff at Community Hospital. In the
past, for example, Smith would write an order for a transfusion and nurses and other staff
would take it from there. Now, he has to oversee all the steps through the computerized
system: ordering the blood, setting the parameters for transfusing it, ordering a follow-up
test to check the results.
“It puts it in my hands, where it belongs,” says Smith.
It takes more time for the doctor, but it’s safer because a series of set protocols has to
be followed and is then tracked electronically, says Smith, who led a team of clinicians in
implementing electronic ordering at the hospital.
“To increase safety, we have to make people as error-free as we can, and the way to do
that,” he says, “is by making the process support clinical decision making, and making
it easiest to do the correct thing.” Nationwide, research has shown that “physician order
entry” prevents 55 percent of order errors.
The next slated addition to the hospital’s clinical information system is barcoded medication
administration, which should reduce medication-related errors by automatically checking
to ensure that the right patient gets the right medication in the right dose at the right time.
Having all of a patient’s information in one place is the most valuable thing about
Community Hospital’s clinical information system, Webber-Schuss says. All of the caregivers
who use it can review the patient’s history, look for trends, find out about any allergies, and
get a comprehensive list of medications being taken.

The system also provides logistical benefits for doctors, who can access patient
information from their offices or their homes.
Doctors in the region are going electronic fairly rapidly. Of approximately 250 local
community-based doctors who practice at Community Hospital, about 60 now have
an electronic health records system, says Dr. Anthony Chavis, vice president of medical
affairs. Community Hospital is helping to bring other doctors in the system through a
subsidy program approved by the federal government to encourage sharing information.
For Community Hospital, it’s an investment in a system that is increasingly providing
integrated patient care by ensuring that caregivers at every level — primary care to
specialist to hospital — can have instant electronic access to complete information.
The rest of the story: Sharing electronic health records
Those electronic connections are being made through human connections. Rose
Wheelus of Community Hospital’s Health Information Technology department is on
a mission to recruit as many area doctors as she can for a program designed to benefit
them, Community Hospital, and, especially, their patients.
Wheelus is a “health information exchange” specialist, and it’s her job to encourage
doctors to share health information electronically using a computer software system
the hospital first began offering free to physician offices in November 2010.
That would be PHIL — or, more formally, Peninsula Health Information Link. PHIL is still
evolving and adding more features, but once their doctor is on board, it can be used
by patients to schedule visits, have prescriptions filled, communicate by e-mail with the
doctor’s office, address minor medical issues through an “e-visit” rather than an office
visit, and view lab results. Even if their doctor doesn’t sign up, patients can also build
their own medical history, helping track everything from vaccines to check-ups to care
for chronic ailments in a single place, accessible from anywhere.

DR. DOuG SuNDE, A MONTEREY PLASTIC SuRGEON AND HAND SuRGEON, HAS INTEGRATED PHIL INTO HIS PRACTICE.
Dr. Doug Sunde, a plastic surgeon and hand surgeon in Monterey, was among the first
doctors in the area to bring PHIL into his office, and he introduced his patient, Marilyn
Byrne, to the system.
Like most physicians, Sunde used to write out prescriptions by hand, then send his
patients off with them to their pharmacies, where they would wait around patiently
(or not so much) for the prescriptions to be filled.
Not any more.
“Now I just hop on PHIL and ‘e-scribe,’ ” Sunde says. “It’s faster and reduces
opportunities for mistakes.”
Plus there’s an excellent chance that the “e-scription” will be ready and waiting by
the time the patient arrives to pick it up.
Not only does PHIL enable physicians to send e-scriptions to pharmacies, it enables them
to exchange e-mails with their patients. And PHIL-mail is encrypted to ensure privacy.
Lead medical assistant Raelene Salgado is in charge of all things PHIL in the Monterey
office of primary care physician Dr. Sharon Wesley, an early adopter of electronic
technology in her practice.
“I’m always online,” Salgado says. “I see e-mail from patients as soon as it comes in,
and I can usually respond very quickly.”
E-mail is especially handy, Salgado says, when there’s a time-sensitive message she
needs to get out to a large group of patients, such as all the women the practice sees,
or everyone in a particular age group. Making separate phone calls is time-consuming.
Each call takes about five minutes when she’s lucky enough to reach someone on the
first try, and much longer if there is a protracted game of telephone tag. Alternatively,
she can send e-mail to each of the affected patients in one fell swoop, which will take
less than five minutes total.

“We have 120 patients signed up now,” Salgado says, “and we want more.”
Among those signed up for PHIL is Maureen Whalen, herself a medical office manager
for a local dentist. When Wesley described the program to her and gave her a sign-up
form, Whalen was resistant. “At first I thought, ‘Oh, no, I’m not going to do this.’”
But she has been a patient of Wesley’s for about 5 years and trusts her judgment.
“Dr. Wesley thinks this is the way to go,” Whalen says. “She says it’s the future.”
And after using it just a short while, Whalen thinks PHIL is already a useful tool today.
She has been managing several health issues, one of which requires regular contact
with the office. Between visits, she’s found that e-mail can resolve questions that arise.
It’s turned out to be easy and effective to communicate via e-mail rather than phone.
“This way I type the message in, and I know it says exactly what I want it to say,”
she says.
Wesley became a believer in electronic medical records while she was doing her residency
at Henry Ford Hospital in Detroit, which was ahead of its time, already linking patient
information through technology in the 1990s. When she arrived in Monterey 9 years
ago, she found herself back in a paper world.
“It was so piecemeal,” she says. “You had to call other offices to get information on
patients. People were faxing things. Charts were getting lost.”
When she made plans to open her own practice in 2006, she commited to doing it
with an electronic record system and says, “It was the best decision I ever made.”
Her old paper records were digitized and everything new is entered directly onto
a computer.
“It has afforded me such a different experience with my patients,” she says. “I’m not
looking around for a chart, lab results, X-rays, family history, or medical history, and I
can do an update (electronically) as I’m talking to them.”

Wesley can also have instant access wherever she is, from her lap-
top. If she’s traveling and gets a call from an Emergency department
about one of her patients, for example, she can open their file and
provide background that can be critical to their treatment.
Now, with PHIL, she can more easily connect the information she’s
been building for five years with her patients and others in the
medical community. And that capacity for three-way sharing —
among patients, physicians, and Community Hospital and other
hospitals if needed — is the greatest value.
“We want to create health records for our patients,” Chavis says,
“that they can have access to anywhere in the country — or even
the world.”
McNamara described a case that’s not unusual in the paper world:
A Southern California man was unexpectedly hospitalized during
a visit to the Peninsula. When he returned home to Southern
California and the care of his regular doctors, he wanted them
to have access to his medical records from Monterey. The records
had to be copied and then mailed. Today, as more providers go
electronic, such an exchange can happen in a fraction of the time —
whether it’s across town or across the state or nation.
A patient’s lab results can be sent directly to both the doctor who
ordered them and to the patient, but they don’t go to the patient
until after the physician has reviewed them and decided whether
it’s appropriate for the patient to see them right away or only after
a personal consultation.

A patient who has more than one doctor signed up for PHIL can share any
results or other information with any or all of those physicians. Which physician
sees what information is always the patient’s choice. “There’s a sort of ‘friending’
procedure, not unlike Facebook,” Chavis says.
The value of the system grows as the number of participating doctors grows.
“We’re in the very early stages,” McNamara says. “But our end goal is for all of
the physicians in the community to have automated systems, and we’ll have ours
here, and all of these systems will talk to each other through PHIL.”
Marilyn and Tom Byrne, for two, are happy for the benefits PHIL gives them
now. The couple are four years apart in age and grew up just four blocks apart
in Pacifica. They went to the same schools, even attended the same church, but
never met until Marilyn was about to graduate from college.
“How clueless was I that I didn’t notice her before?” Tom wonders.
These days, though, he’s completely clued in to every aspect of his wife’s care.
“We’re incredibly sad, but also thankful for the hundreds, thousands, maybe
millions of women who have gone through this before and paved the path
for us,” he says.
And they’re thankful, too, for the technological advances that are making that
path a little smoother.

As the director of Health Information Technology at Community Hospital, Charlene
Webber-Schuss, RN, knows just about everything there is to know about electronic
health records, and she’s a big fan. So when the hospital introduced PHIL (Peninsula
Health Information Link), she signed right up and so did her doctor.
Getting in touch with her doctor could be challenging in the past, Webber-Schuss says.
“The times it’s convenient for me to call are either after work, when the office is closed,
or at lunch, when the office is also closed.”
But timing isn’t an issue any more. Now she can use PHIL to e-mail whenever she needs
to. Like the time her daughter was having an allergic reaction to a medication; because
she’s a nurse she knew it wasn’t so critical that she needed to contact her doctor imme-
diately, but she wanted to let her know that she was discontinuing the drug.
Webber-Schuss has also used PHIL to build a personal electronic health record for herself,
entering the medications she takes along with immunizations, surgeries, and procedures
she has had.
At Webber-Schuss’ choice, her doctor has access to this record, and can add information
to it whenever she wants to. This is also where any of Webber-Schuss’ lab or radiology
results would be reported if her doctor okayed them to be released to her.
If at any time Webber-Schuss sees more doctors, she can add them to her list of doctors.
If they also use PHIL, PHIL will know and tell her so. Then she can choose to let these
doctors also have access to her record (or not).

PHIL, she can print her record out and bring
it along to her appointments.
Webber-Schuss’ PHIL account includes her
whole family, so she has also built electronic
health records for her daughter (who has her
same doctor) and for her husband (who has a
different doctor, but one who also uses PHIL).
But suppose for a moment that Webber-
Schuss had been stranded, with no connec-
tions at all to any doctor who uses PHIL.
Then she could still have signed up, made
personal records for herself and her family,
printed them out, and showed them to her
PHIL-less doctors.
But PHIL’s other advantages — such as e-mail
access, and lab and radiology reports — would
have been unavailable to her.
Which is why, when she says she was an eager
early adopter of PHIL, she quickly adds, “And
so was my doctor, thank goodness.”

functions that paper records cannot deliver.

together in one place, and keeps it current;
clinicians have access to information about
the drugs or treatments prescribed by other
providers, so care is better coordinated.

patients. For example, after a clinical visit
or hospital stay, instructions and information
for the patient can be provided effortlessly;
and reminders for other follow-up care can be
sent easily or even automatically to the patient.

Patients can have their prescriptions ordered
and ready before even leaving the provider’s
office; and insurance claims can be filed
immediately from the provider’s office.

point to additional resources. Patients can be
more informed and involved as electronic health
records are used to help identify additional
web resources.
❚ Does more than just “contain” or transmit
information. Can also compute with it. For
example, certain record systems can not
only contain a list of a patient’s medications
or allergies, they can also automatically check
for problems whenever a new medication
is prescribed and then alert the clinician
to potential conflicts.

all of a patient’s information together and to
automatically identify potential safety issues;
assists clinicians by providing “decision
support” capability.

For example, electronic health records can be

Many in healthcare see electronic information sharing as the key
to better care for less money. The federal government does, too,
and has taken steps to encourage it by offering financial incentives.
under the Health Information Technology for Economic and
Clinical Health Act (HITECH), part of the American Recovery
and Reinvestment Act of 2009, Medicare and Medicaid incentive
payments will be available to hospitals and clinicians that adopt
certified electronic health record technology and successfully
demonstrate its “meaningful use” in ways that improve quality,
safety, and effectiveness of patient-centered care.
To qualify for incentives, the meaningful uses have to meet specific
measurable standards and the systems have to meet a range of
requirements. For example, systems must be capable of electronically
sharing medication and allergy lists from doctor to hospital, hospital
to doctor, and doctor to doctor.
The effort aligns with the path Community Hospital has been
developing over the last 20 years, putting in place systems to
integrate technology in ways that increase efficiency and improve
patient care.
“Nationwide, most hospitals don’t have the electronic capabilities
we do,” says Dr. Anthony Chavis, the hospital’s vice president of
medical affairs. “We believe we’ll be one of 1 percent or 2 percent
of u.S. hospitals that will achieve ‘meaningful use’ in 2011.”

Keeping our Community Healthy
In 1996, we started the Community Benefit program, a way
to make sure we were doing everythng we could — inside
the hospital and out — to advance the health and well-being
of our community. Through it, we have provided more than
$850 million in support of local organizations with health-
related goals, for access to care for thousands of people who
couldn’t pay for themselves, for education on health and
wellness, and in programs for those with special needs. That
commitment continued in 2010, with nearly $153.5 million
and 24,719 volunteer hours committed to community benefits.

Dr. John Baldwin played numerous roles at
Community Hospital during his career as a
surgeon on the Peninsula, caring for residents,
serving as chief of surgery, promoting develop-
ment of a nursing school and other advances,
and providing philanthropic support. In 2010,
he took on a new role: patient.
After retiring to the Gold Country community
of Twain Harte in 1990, Baldwin kept up his
active lifestyle. Last year, it caught up with him.
“I am 76 now,” he says, “but I can’t believe
that, so I probably hurt my back over the last
20 years by pretending to remain 38 years old
— skiing, running, shoveling snow, cutting,
splitting, and stacking oak logs, and taking
fishing groups to Alaska for 200-pound halibut.
But, a disc in my lower spine decided to push
into my sciatic nerve, causing a great deal of
pain. I thought it would go away on its own.”
It didn’t, and when Baldwin came to
Community Hospital six months later, his
right leg, immobilized by pain, barely carried
Dr. John Baldwinhim through the front door. Friends had recom-
mended he go to Stanford Medical Center, but
he was confident Community Hospital would
suit his needs. “I knew that coming home
would be best,” he says.
“My surgeon’s expertly performed neurological
procedure immediately removed the incredible
pain in my right leg, and I credit him for my
complete recovery,” Baldwin says. “Lying there
and suddenly becoming a patient for the first
time in my life, I had no fear, because I knew
that I had come to the right place.”
During his stay, he passed around a 100-page
guest book, collecting signatures and photos
of staff and visitors who came to see him and
his wife, Jeannie, a former Community Hospital
operating room nurse.
“My guest book has 154 entries and pictures
of all the visitors,” he says. They range from
Dr. Steven Packer, president/CEO, to staff
members who were “kids” when he practiced
at Community Hospital and have since moved
up through the ranks.
“I got well quickly because it was such a fun
experience,” he says “thanks to the blending
of technologic expertise and a caring, even
loving staff.”
The Baldwins returned to Twain Harte and
continue their long-distance relationship with
the hospital, as regular donors and as members
of the Maurine Church Coburn Legacy Society,
named for one of his former patients. The
society recognizes those who generously make
bequests to Community Hospital to help ensure
its future.

Stroke is the third-leading cause of death in
the united States. On average, someone suffers
a stroke every 45 seconds in this country, and
someone dies from a stroke every 3.1 minutes.
In Monterey County, an average of nearly two
people per day experiences a stroke. Those
weighty statistics prompted Community Hospital
to become a certified stroke center to advance
the level of stroke education, response, care,
and rehabilitation in this community.
These statistics also hit close to home for
orthopedic surgeon Dr. Charles Snorf, who has
had five members of his family and a number
of colleagues and friends suffer devastating
strokes. Now retired, he and his wife Leslie lead
active, healthy lives in Carmel. Yet the couple,
who share five children and anticipate a 13th
grandchild, are keenly aware of the potential for
stroke and its impact on family.
leslie and Dr. Charles Snorf“What I’ve learned,” he says, “is the impor-
tance of education and early intervention.
The idea of early diagnosis and treatment is
key to how a person survives a stroke.”
To aid in that early education, the Snorfs
contributed to Community Hospital’s stroke
program, and went further by championing
the program among his former colleagues.
Dr. Snorf worked with Community Hospital
to reach out to doctors in the community,
requesting support of the stroke program.
The response resulted in more than 40
donations to the hospital. Since the stroke
program began, more than $1.6 million has
been raised.
“We wanted to donate not just to the hospital
but to emphasize a certain discipline,”
Dr. Snorf says. “We realized it made a lot of
sense to support the Community Hospital
stroke program.”

Bill and Barbara Strohm are joiners. They
appreciate community and a sense of
giving, of getting involved, of belonging.
They’ve done it all their lives and now
they’re doing it through Community
Hospital.
When Bill was diagnosed with cancer, the
couple handled it the way they do most
things — with energy and investigation.
They also engaged the support of
Community Hospital.
“My treatment at Community Hospital was
exemplary,” says Bill. “I have had treatment
before, but nothing like this. It was above
and beyond — delightful, warm, caring.
It made my whole journey easier. And I felt
I wanted to do something for the hospital,
to show my appreciation.”
Bill and Barbara StrohmThe Strohms recalled that when Bill’s
older sister went through her own cancer
journey in another community, she felt
isolated and very much alone. Bill belongs
to a cancer support group and has
appreciated the opportunity to look, with
others, at life after cancer, to share new
ways of living, to shift attitudes, and
set goals with the support of those who
can empathize.
And so, after meeting with Dr. Grant
Swanson, medical director of Community
Hospital’s Comprehensive Cancer Center,
and Phillip Williams, RN, center director,
the Strohms donated $100,000 in seed
money to create a survivorship program.
“We had contributed to the hospital
over the years,” says Bill, “but never
like this. Cancer survivorship programs
are becoming a national trend in bigger
hospitals, and it seemed interesting
and important to us to initiate this kind
of community here. We are excited.”
