Preface It is our privilege to present to the readers showcases of Arab-German projects in the healthcare sector. This third edition of the “Healthcare Guide” highlights the great interest in the healthcare industry on both the German and the Arab side and pin-points outstanding Arab-German projects.

This edition is part of a series of sector specific publications issued by the Ghorfa Arab-German Chamber of Commerce and Industry such as: “Arab-German Yearbook—Construction and Consulting”, “Education Guide—Partnerships and Perspectives of Arab-German Cooperation”.

We hope that the “Healthcare Guide” will further strengthen Arab-German busi-ness relations in healthcare.

The healthcare sector is one of the most important pillars of Arab-German busi-ness relations. It holds huge potential and is one of the fastest growing fields of coop-eration with bright perspectives for the future. Due to a higher income, a longer life expectancy and an increasing population, the Arab world is investing big budgets in their healthcare systems. The region is experiencing a boom in hospital and clinic construction with healthcare-related projects planned or under way worth US $ 50.5 billion. Over 30 % are in the design or tender phase, i. e. there is still a lot of potential for possible business endeavors. To name an example, in Saudi Arabia allocations for health and social affairs spending grew by 8 % year-by-year to reach US $ 28.8 billion in the budget for 2014. Expenditure for health in the Kingdom is expected to grow remarkably to US $ 47.7 billion by 2017.

The demand for high quality healthcare services is constantly increasing. A strong healthcare sector has a positive influence on many other areas, such as construction when planning or building hospitals, education through training of medical staff, and the IT sector through development of software solutions for hospitals as well as the medical tourism sector.

German expertise and quality have a very good reputation in all these fields and we can see a strong interest for cooperation on the German and Arab side. The high participation of German companies and institutions at the Arab Health Dubai 2014 shows the increasing interest in cooperation with the region. More than 400 German exhibitors participated. Ghorfa supports these activities by promoting the healthcare sector as one of its strategic priorities within the chamber. In this context Ghorfa will host for the seventh time the Arab-German Health Forum that will take place in Stutt-gart on April 14–15, 2014.

We would like to express our gratitude to the companies and institutions that con-tributed to this publication. In particular, this applies to those companies and institu-tions that illustrate their ongoing projects in this year’s edition or that are active members of the working group “Healthcare”. Patients in Good Hands—Advanced Training for Professionals in the Health Sector

At the interface of health, economics and public service administration, hospital managers have to cope with a range of different and sometimes conflicting targets. They need strong professional skills to negotiate in difficult settings, to find compromise solutions and to communicate effectively at each level, both within healthcare facilities as well as in the wider political sphere.

The International Leadership Training—Hospital Management Course of Deutsche Gesellschaft für Internationale Zusam-menarbeit (GIZ) GmbH offers a renowned and advanced train-ing program for professionals in the health sector. Training can be provided both in Germany, or alternatively, as online learn-ing course, which blends e-learning with practical on-site semi-nars.

International leadership—training in Germany

Successful reforms in the health and hospital sector need quali-fied personnel in key positions. The one-year International Leadership Training (ILT)—Hospital Management course offers advanced training for professionals in the health sector. This practice-oriented training enables the participants to design customized solutions to pressing issues in the management of their hospitals and to share their knowledge as multipliers.The participants in the ILT Hospital Management training in Germany are usually young executives from partner countries. They hold an academic degree and have at least five years of experience in the area of administration or management of healthcare facilities.

Together, they develop a vision for a future development of the health and hospital sector and acquire new skills based on international best practice, in-depth management abilities and innovative approaches that position them for an active role in hospital management in their respective countries.

German LanguageDuring a five-month intensive language course in their country of origin as well as in Germany preceding the training course, the participants acquire good knowledge of the German lan-guage. This allows them to interact with their German-speak-ing colleagues once they are on-site in Germany.

Special course in Hospital Management The training itself consists of consecutive modules, which are interactive, practice-oriented and foster intercultural learning. They also include a training-of-trainers approach, which allows participants to act as multipliers within their respective insti-tutions. The curriculum of the four-month course is based on a general needs assessment, but, upon request, it can also be tai-lored to the particular demands of the participants and their respective facilities or institutions.

The general curriculum includes the following topics: – Introduction to the German health and hospital system – General management principles – Strategic management of hospital finances, human

resources, and equipment– Health economics and accounting– IT application in hospitals – Hospital hygiene and sanitation– Desaster management

Practical training The participants gain first-hand experience of hospital man-agement in Germany. During a three-month practical training in selected German partner hospitals, they rotate through vari-ous departments with a focus on administration and manage-ment.

General management A three-module management training before, during and after the special course increases the participants’ competence relat-ing to self- and work-organization skills, presentation skills, communication and networking skills, and helps them to develop their potential as change agents in their countries of origin.

Transfer project During their training in Germany, the participants work on a transfer project of their choice. This project allows them to put the knowledge, skills, and insights they have gained in the dif-ferent training modules into practice. After returning to their countries of origin, they continue to advance their projects, testing and adapting the new approaches in their everyday working life.

International focusThe graduates of the International Leadership Training will have acquired profound knowledge and strategies of modern management and will have improved their intercultural skills. Through their appreciation of global thinking and networking they will be important partners at an international level.

International Leadership—Blended Learning

Alternatively to the on-site training in Germany, the GIZ also offers a blended learning course, with e-learning modules, which can be studied online and are completed by face-to-face workshops with professional German tutors. Like the on-site training, the blended learning provides participants with an opportunity to work intensively with modern management instruments required for an effective, efficient, and demand-oriented administration of healthcare facilities. The graduates will take a lead in the implementation of health and hospital sector reforms in their home countries.

Methods Online modules are followed by face-to-face workshops. The entire course will be supported by tutors who will be in direct contact with the participants throughout the whole duration of the course. Participants will be guided through different mod-ules and closely supervised. Continuous interaction amongst participants as well as between participants, tutors and lectur-ers is an integral part of the course’s design.

A broad number of innovative tools, which enable close interaction, communication and cooperation, are available on the GIZ Global Campus 21 learning platform. They include a chat room for students, a pin board for posting information of general interest, and a comprehensive knowledge management system for exchanging completed material and asynchronous discussion tools.

Structure of the course The course starts with a start-up seminar in the capital city of the respective country. During this introduction, participants will familiarize themselves with the online environment and its various communication tools and get to know the other par-ticipants.

In the following online introduction, the participants take part in the short course “Fit for Global Campus 21”. The online phase is comprised of five consecutive learning modules: 1. Quality management in healthcare facilities2. Health economics and management3. Human resources management 4. Accounting and finances 5. Information management.

In each module, the course participants have to complete a number of exercises, tests and individual assignments. Each module requires an average of 10 to 12 working hours, with an overall completion time of three weeks. Participants will only have access to a new module when they have completed the preceding one.

Virtual working groups, discussions, professional chats, question-and-answers sessions as well as continuous supervi-sion by experienced lecturers allow for an intensive exchange among all participants.

The online phase is followed by a five-day face-to-face semi-nar in a convenient learning and teaching place.

Both the on-site training in Germany as well as the blended learning course provide participants with an opportunity to work intensively with modern management schemes and the instruments required for an effective, efficient, and demand-oriented administration of healthcare facilities. The training is focused on practical problem-solving skills and the transfer of knowledge. The graduates not only become leaders but also multipliers in the implementation of healthcare and hospital sector reforms in their countries.

Networking for future leaders

Global Campus 21® is the learning and networking platform of the Deutsche Gesellschaft für Internationale Zusammenarbeit (GIZ) GmbH. The user interface is available in eight languages: English, German, Spanish, Portuguese, French, Russian, Arabic, and Chinese. The Global Campus 21® toolkit specifically supports learning and cooperation with new and social media.

AGImed—Preparing Iraqi Doctors for Medical Board Specialization Training in Germany

Access Germany Iraq—Academic Counseling & Enterprise Service (AGI), Bonn, Germany

Medical studies in Iraq are at high standards, and Iraqi doctors well qualified. Many of them strive to expand their skills and want to get board specialization training abroad. The program AGImed guides young Iraqi doctors in 8 steps to get acquainted with the German medical system, learn the German language and successfully apply for a board specialization training job at a German hospital.

The duration of the full preparation program takes approxi-mately one year, starting with individual counselling by e-mail and skype, e-lectures prior to arrival, leading into a highly intensive six-month course in Bonn, Germany. The images show the current batch of January 2014.

1. Step in

Understanding the differences between the German and Iraqi medical education system The reputation of Germany and its medical technology awak-ens longings and dreams. However, the challenges of learning and professionally performing in an entirely new language, integrating into a very different society, and competing with other doctors from all over the world for a board specialization training job, are often underestimated.

“Can you get me admission to medical board training stud-ies?”—This is a frequent request we receive from Iraqi doctors. And it is the starting point of a profound counseling and coach-ing process, called AGImed, which starts with an indepth infor-mation about the differences between the Iraqi and German medical education system.

In Iraq and Germany, medical studies take six years, including a final practical year at a teaching hospital. In this sense, there is no difference between the two countries. In Iraq however, medi-cal studies are taught in English—in Germany, in German lan-guage. Medical graduates from German universities are imme-diately entitled to proceed with board specialization training. Medical graduates in Iraq are obliged to an additional rotation year and one year of civil medical service in a rural family med-icine center. These two additional years provide young Iraqi doctors with a broad knowledge and substantial practical expe-rience that equips them well for new tasks in Germany.

Both countries have in common that there are no clinical Master or PhD study programs at universities. Board training takes place on the job, within full-time and salaried clinical jobs at hospitals.

How to get such a board training position in Germany? Whereas in Iraq it is determined by a central assignment if and when a doctor gets a seat for board training, every German hos-pital publishes their open board training positions—called

“Assistentarztstellen”—in public job fairs in the internet or spe-cialized newspapers. Doctors are expected to apply in writing at each individual hospital. The hospitals invite the best appli-cants for job interviews and then select the very best.

At this point, the answer to the frequently asked question “Can you get me admission to board training studies?” becomes apparent: no one else than an applicant himself can get a board training position—based on his personal qualifications, per-sonality and ability to master the complex application process. But he can be guided and supported through the long and chal-lenging journey of preparation and competition. This is where the AGImed program serves as well-structured, professional coaching program.

2. Check out

One month of clinical observership in GermanyHow to get a better idea about what a young doctor will be fac-ing in Germany than coming over and taking a close look? Most doctors who think about board training, first take this opportu-nity and gain insight into the working-routine of a German hospital, observe the latest diagnosis and treatment methods, meet German colleagues, and discuss the challenges and chances of advanced training. Some feel satisfied with the short-term experience, but decide that Germany will not be their preferred country in the future. Others are highly encour-aged and energized to aim for the next step.

3. Build up

Lern German language for medical communication Excellent German language proficiency is the key for success in Germany. By law, intermediate German level B2 is required to apply for a medical practicing license. However, dealing with patients in pain, relatives with worries, and hospital colleagues and staff in fast-paced critical situations, requires outstanding language and communication skills. Throughout the recent years, the German Medical Association has become more and more sensitive to complaints and treatment mistakes due to insufficient language skills of foreign physicians. Nowadays, most hospitals require a high C1 entry level and do not seri-ously consider any application without such certificate.

AGImed follows a two-haul approach: all candidates are requested to acquire a basic German language level A2 in Iraq, provided by selected local German language teachers. While it may seem more effective to commence with German in Ger-many, this individual preparatory phase in Iraq has proven to distinguish the real achievers from the good-willed.

Successful A2 graduates then relocate to Germany and attend the highly intensive B1–C1 language course. The course com-bines classroom teaching with a strong focus on professional communication needs in medical environments with medi-ated out-of-class encounters with the German society. This raises the Iraqi doctors’ sensitivity for intercultural differences and how to cope with them.

Introduction to the German healthcare systemThere are significant differences between the Iraqi and German healthcare system. In Iraq, hospitals are free-of-charge and drop-in centers for all patients. Private clinics serve against cash. In Germany, private clinics are first drop-in points and serve mainly on an insurance-coverage base. German hos-pitals receive patients who are being transferred from private clinics for inpatient treatment. Managers and doctors operate on tight standardized economic parameters, where medical decisions have to cohere with. Even young board training doc-tors, who are primarily concerned with direct medical tasks, need a basic understanding of the underlying regulations of the health system and its impact on everyday work-flows at hospitals. In interactive workshops with external experts, the AGImed course participants receive first-hand information and discuss the role and working conditions of assistant doctors in Germany.

4. Try out

Practice by simulations“When I finished my basic German language class in Iraq, I felt ready to live in Germany. But when I had to simulate my first patient encounter, I realized that there is another level of com-munication and medical terminology needed, which you can only master with special courses and expert training.”

Indeed, a medical doctor is exposed to highly responsible human interactions, where he is in full charge to lead patients through professional anamneses and diagnosis feedback con-versations. In addition, he has to confer with colleagues and staff about treatment recommendations, etc. Therefore, during the third month of the AGImed course, participants get pro-gressively trained in medical situations with simulated patients and German medical doctors by role playing. They learn how to deal with different behavior patterns of German patients compared to Iraqis and practice physical examina-tions and medical report writing according to German stand-ards. By trying out in a safe training environment, they gain confidence and get ready for professional performance at the future board training job.

5. Get connected

Building relationships within the medical community Hiring decisions in Germany are strictly based on qualification criteria. Yet, being connected with other junior and senior col-leagues of the medical community allows valuable exchange about open training positions and latest developments. To open the door, AGImed course participants are guided to attend short medical conferences at local branches of the medi-cal association and to visit with specialist institutions. They get trained to establish individual professional networks within their preferred medical specialization branch.

6. Profile yourself

Why Iraqi doctors are beneficial for German hospitals Medical doctors from Iraq have several special attributes that distinguishes them from doctors of other origins: They are stress-resilient due to life-long exposure to critical living and working-conditions. The additional rotation year provides them with experience in a variety of medical fields, and the civil service at rural primary care centers with high case num-bers and responsibilities at an early age. During the AGImed course, they learn to define these individual strengths and competitive advantages, and how to present them in a well-structured manner that allow hospitals a clear assessment and comparison with other applicants.

7. Aim straight

The “art of job hunting”“It is well known that there is a lack of doctors in Germany—one just needs to know the secret how to get such a job.”

The “art of job hunting” means to get the best job offer as quickly as possible. Effective and efficient time management is crucial for AGImed participants, because during the course, life in Germany is financed on personal budgets, and continuation depends on getting a board training job afterwards. Therefore, the last course phase includes intensive individual coaching sessions. Candidates are constantly supervised to manage the application process as goal-directed as possible. The goal is reached when a successful job interview renders the long-desired board training job offer.

8. Settle in

Legal paper workHolding a board specialization training contract in their hands is usually the greatest moment of hap-piness and relief. Months of preparation in Germany are completed, substantial time and financial investments are about to pay off. However, before the candidates step out of the AGImed group setting and move to new cities and hospitals, a few other administrative tasks have to be finalized: the first temporary medical practic-ing license, returning home to apply for residency and working permit, etc..

After approximately six months of preparation in Germany, newcomers become confident, well integrated new residents.

Cooperation beyond Patients’ Treatment— Perspectives of German-Arab Medical Relations

Medical cooperation between hospitals in Germany and the Arab world is a long and outstanding history of success. Having its focus on the medical treatment of patients travelling from the Arabic speaking countries to Europe and Germany, it has created a reliable network of personal, cultural, political, and medical relations in over 40 years now.

Building capacities in medical institutions in the Arab world has been another approach for cooperation—taking into con-sideration the concrete results, it is still not developed in a very large and, more important, in a very sustainable way.

The reasons for that are many and varied: capacity building in medicine has been seen as another way just to make money in the cooperation with Arab countries. This being a main issue cannot lead to positive results. Furthermore, sustainable coop-eration cannot be gained with the exchange of doctors, nurses or medical administrative staff for only a weekend or two. Over and above that, consultation in hospital building and construc-tion cannot be made by just implementing a copy of the “Ger-man model” in the target countries as it has been done in a lot of cases, unfortunately. And, finally, the approach to build up capacities in a region with a long and outstanding tradition of medicine can be considered as doubtful, at least.

By going through these arguments, another approach has to be chosen:

First: Yes, of course, cooperation in medicine across boarders is not a question of charity. It is a business, but it is not a busi-ness as any other. In the past years, a change of perspectives can be seen. Customers and partners in the Arab world have

developed a very high sensitivity for the good value of goods and services delivered. Going back to medical treatment, it is still unbelievable to have a look on certain invoices of certain hospitals for medical treatment. Suspicion can be very well understood as well as sharp reactions from the payers’ side. So we should always consider our counterparts in the Arab world as partners—and come to fair relationship in order to create win-win-situations.

Second: “Visiting doctors” has been a model of cooperation which was desired by the partners for a quite long period of time. In the meantime, it proofs that priorities have changed. It might be still necessary that a doctor travels to a hospital or to a private customer to give advice or to do some treatment. But the demand has changed: partners should send out people who are interested not to do as many surgeries as possible for as much money as possible for a weekend, and then leave the host’s country, but partners should send out people who are interested in a sustainable relationship with their counterparts, be it institutions, hospitals or patients. That is why we suggest to get rid of the term “visiting doctor” and to use the wording

“reference doctor” in order to show the interest in long-lasting relationships. And, furthermore, this long-lasting relationship

should not be limited to doctors only. We see a huge demand of nurses, specialized nurses, and also administrative staff in the Arab world acting in the role of consultants—staff who is able to contribute to the improvement of processes in nursing and hospital administration.

Third: A lot of projects for hospital planning or construction have been started in the last years—the results are not very convincing. We have got the impression that the non-realiza-tion of many projects has one main reason (besides the argu-ment in the first point that consulting is not just making money). This main reason, according to our opinion, is the fact that too often there has been just a transfer of the German model of constructing a hospital, including the relevant pro-cesses, to another environment. Of course, this is a very simple way of cooperation, but it cannot be a sustainable one. The political, cultural and, more than this, the medical background of the client who has the intention to construct a hospital has to be taken into first consideration. For example, the approach to a hospital visit in the Arab world is different to the approach in Germany, as we can see. Going to a hospital in Germany means that an inpatient treatment is expected. Patients in Arab countries who go to a hospital expect (and get!) extensive outpatient diagnostics and treatment in a first step. The figures of inpatients’ and outpatients’ visits to hospitals in the Arab region show this different approach. Without reflecting these facts in consulting projects, they cannot be successful.

Finally: The “philosophical approach”—we believe that this is the crucial point for the success of consulting processes between the European/German and the Arab world. We have often seen that the consulting approach outside Germany is

the same as everywhere—be it in developing countries or be it in already high developed countries with Arab background. The ability of developing sensitivity for the different historical, cul-tural and religious background of the customer is the main suc-cess factor for all activities in the consultancy process. Espe-cially in the Arab Countries, you should never forget the great and old history of medicine and the great experience people in these countries have in medicine. Respect and, of course, mutual respect and the knowledge of the relevant backgrounds is the main point for the success of cooperation beyond the treatment of patients.

Having this in mind, Klinikum Stuttgart, together with renowned partners, has forwarded a number of projects in the Arab world. Before bringing experts together, awareness for the task and for the respective context has been raised on both sides. This is the fundament for communication and has to be implemented by the persons responsible before bringing peo-ple together. Our partners in UAE, Kuwait, Qatar, and Saudi-Ara-bia, but also in Egypt, Tunisia, Libya, and other countries, esti-mate this approach—which does not, of course, change the special way of negotiating projects with partners with Arab background. But it changes the state of communication and, by that, the state of relation between cooperating people.

Therefore, cooperation between German hospitals and their counterparts in the Arab world is not a question of “capacity building”, it is, if it is well done, a question of mutual learning and exchange, of intercultural and interprofessional coopera-tion between partners on an equal footing. Respecting this, medical cooperation can be widened and improved for the ben-efit of the players, but, over and above all, for the benefit of the patients in the institutions of all partners concerned.
