The UK healthcare sector is very much at the forefront of the public consciousness, with theincreasing pressures on the NHS and the adult social care sector regularly making the headlines.

Despite the much-discussed funding issues, the good news is that the country still continues toprovide new, high-quality healthcare-related facilities to help meet the needs of an expanding andrapidly ageing population.

It is here that architects play a crucial role. Good building design is pivotal to efficient and effectivehealthcare delivery, whether it be through a patient-friendly hospital, sophisticated medical researchand test facility, or a residential care home.

In this issue we look at some great examples of healthcare design, such as the new woodlandcampus-inspired Dumfries and Galloway Royal Infirmary hospital in south west Scotland. As our in-depth feature reveals, a huge emphasis has been placed by the architects and their clients ondesigning a building that endeavours to create an uplifting, health-promoting environment forpatients, visitors and staff.

In rural Wiltshire, we put the spotlight on Wadswick Green, a new kind of retirement village withattractive, spacious homes that adapt to meet residents’ changing care needs as they age, enablingthem to live independently in the same accommodation for as long as possible.

Jess Unwin explores the Jack Copland Centre, the new headquarters of the Scottish National BloodTransfusion Service (SNBTS) and a sophisticated centre of excellence set to provide vital bloodprocessing and testing services, among other services, when it opens this year.

The £297m Midland MetropolitanHospital, under construction in Smethwicknear Birmingham, has seen an importantmilestone with construction beginning on its rainscreen cladding by specialist contractor Prater.Midland Metropolitan Hospital willprovide acute emergency care to theSmethwick area including 670 beds and 15operating theatre suites. It includes around80,000 m2 of accommodation.The design by London-based Edward

Williams Architects is intended to “provideunder one roof cutting edge technologies inmedicine in a beautiful environment,” andat a “human scale.” The building has beenhailed by Sandwell and West BirminghamNHS Trust chief executive Toby Lewis as “aglass palace” due to the extensive glazing tothe facade. He told the Birmingham Mailthat the hospital would mean “movingtowards acute care seven days a week” –completion is scheduled for mid-2018.The architects commented: “Analysis of

the brief produced a natural stacking offunctions with wards at the top, clinicalareas in the middle and the car park at the

bottom – a unique arrangement for a build-ing of this type.” They continued: “Clearand logical stacking of floors and functionsmaximises usable floor area and achievesdesired clinical adjacencies.”The exterior palette includes terracottaand timber as well as glazing, ETFE pillows, concrete, painted metal cores andmetal louvres. Edward Williams Architectssaid: “The facade materials are organisedby vertical elements to break up the mass ofthe building and reflect the rhythm of thestructural grid.” Appointed by main contractor Carillion,specialist building envelope contractorPrater’s scope of works includes extensiveroofing systems comprising inverted brownroofing and extensive green roofing, as wellas over 2,500 m2 of waterproofing. In addi-tion, Prater will also provide SFS andrender rainscreen, windows, doors, louvresand composite cladding to courtyard areasof the site.“We’re delighted to have work underwayon such a large-scale project,” said KatePrater, associate director for marketing at Prater.

Screens up at major new Midlands hospital

What’s thought to be the first fullydementia-friendly housing scheme inAshford, Kent scooped an award atthe 2016 Kent Design andDevelopment Awards.

Phase one of the two-phase, £15.4mproject was completed earlier in theyear, providing the scheme’s existingresidents with 33 new, more energyefficient homes built to the council’sSpace standards and Lifetime Homes standards.Careful consideration was made inthe design of the project to incorpo-rate colours and visual signs, such as“memory shelves” outside front doorsand plenty of natural light. Thegardens were also securely designed toenable residents to enjoy them safely.As well as these design features thescheme includes Age UK’s day centre,communal facilities, a restaurant andan on-site hair salon. A resident-runshop is also in the pipeline.Tracey Kerly, chief executive of

Ashford Borough Council, commentedon the project: “We’re very proud ofwhat we have created here and arethrilled that it has been recognisedwith this award. The council hasworked hard to design not just a shel-tered housing scheme but somewhereresidents can really feel at home.”He continued: “With an increas-ingly ageing population it is impera-tive for us to prepare for the growingdemand on housing schemes and weare looking ahead to phase two ofFarrow Court which will see a further71 homes and eight recuperative careunits being built on the site.”

Swedish developer NCC has been commissioned to design and build theFinspång Medical Centre in south easternSweden to replace outdated healthcarefacilities, in a contract estimated at approx-imately SEK500m (£45m).The project, which is being developed

with the region of Östergötland, will com-prise about 17,000 m2 across five floorsincluding space for a doctor’s office, a labo-ratory, child health centre, midwifery clinic,child and youth health centre, specialistnurse-led clinic, residential rehabilitation,investigation service, and a home care andlocal care department. The building will beenvironmentally certified according toSweden Green Building Council (SGBC) Silver.Stefan Fredriksson, property develop-

ment manager at Region Östergötlandcommented: “We need to adapt our opera-tions to tomorrow’s healthcare, which isbecoming increasingly sophisticated anddemanding. The new building will not only

replace old and dilapidated premises butalso promote the development of theregion’s and the Municipality of Finspång’shealthcare operations.” The project will take the form of a part-

nering contract – a “structured cooperativeformat” including NCC and RegionÖstergötland jointly drawing up systemdocuments, project budget and detaileddesign plans.Henrik Landelius, Sweden head at NCC

Building commented on the partneringaspect: “It’s good to become involved asearly as possible in a project of this nature.It benefits all parties concerned and enablesour competence and experience to beutilised in the best possible manner. Such anarrangement means that expertise in thedesign and function of tomorrow’s health-care facilities can be effectively leveraged.”The project recently commenced with “ashort period of project engineering andbudgeting work.” The new centre isexpected to be opened in autumn 2020.

Construction has begun on the expansion of a hospital in Vestfold,Norway, following a contract beingsigned by Skanska (together withCura-gruppen) and public health trust Sykehuset Vestfold HF, worthNOK1.7bn (£160m).

The project also includes alignmentof the infrastructure, energy facilities,and demolition of existing hospital buildings.The contract is based on the IPD

model (Integrated Project Delivery),where the parties will “cooperate inthe implementation phase withcommon incentives,” including sub-contractors. Skanska has gatheredexpertise from Norway, Sweden, UKand USA to work on the project.Skanska’s share of the contract isworth about NOK 960m, (£90m), andthe work will be divided betweenSkanska Norway and Skanska UK at aratio of 70:30. The additions comprise a psychiatry

building scheduled to be completed inMarch 2019 and a building for‘somatic’ care (nervous disorders)during the first quarter of 2021.

Actavo Building Solutions, claimed to be“the UK’s fastest-growing modular buildingcompany” has been awarded a number oflots on the new four-year, £750m NHSmodular building framework. The firm commented: “With NHS bed

availability at an all-time low, increasingfacilities in a limited timeframe is essential.”The new framework has been set up by theNHS, solely for the supply of modularbuildings, promoting “faster, cheaper and

greener building solutions,” said Actavo.The new framework has been split into

11 lots. Actavo, along with the other suc-cessful contractors, will act as the principalcontractor and will provide a wide range ofservices – from full architectural design forthe initial concept, to services, site worksand completion of bespoke modular build-ings, modular healthcare units and modulareducation units.Matthew Goff, director of UK operations

at Actavo Building Solutions, commented:“Modular building is the NHS’s construc-tion dream. It delivers sustainable, cost-effective buildings in tighter time frameswhich gives a high degree of certainty inmeeting specific needs.”He continued: “As modular buildings are

manufactured in controlled factory environ-ments and assembled onsite, it means that,in many cases, ward blocks, A&E depart-ments, theatres and clean rooms can bedesigned, built and delivered in a matter ofweeks, reducing the pressure on bed avail-ability and the need for short-term hire.”The firm has been working with the NHSfor the past four years.During the tender process, Actavo

Building Solutions completed a comprehen-sive, pre-qualification submission involvingquality and pricing returns with proposalsfor a standalone modular office, a GPsurgery, an operating theatre, a single and double classroom block and a fullschool facility.

Modular firm wins place on frameworkFRAMEWORK

A million tonnes less carbon and £300m insavings have been generated by hospitalsthat have adopted energy efficiency meas-ures, according to a green energy firm.Vital Energi, which installs and

manages energy solutions to 14 hospitalsand seven NHS Trusts and Health,reported the significant milestone to markNHS Sustainability Day (23 March).Project development director Ashley

Main said the savings could help fundfrontline services, adding: “The fact thatjust 14 hospitals can generate over £300min savings and one million tonnes ofcarbon reduction is a great indicator ofhow much potential there is in the NHS,with regards to revolutionising the way itgenerates and uses its energy.”The company has worked on the proj-

ects for the NHS for the last three years,delivering various energy conservation

measures and energy generating solutionssuch as combined heat and power orbiomass systems.The projects, procured through the

Carbon and Energy Fund, require theenergy firm to guarantee minimum finan-cial savings and carbon reductions for theduration of the contracts, which rangefrom 15 to 25 years.One of the first hospitals to take advan-

tage of the scheme was the York TeachingHospital NHS Foundation Trust, where a1.2 MW combined heat and power enginewas installed along with lighting upgradesand other maintenance services. This wasfollowed up with similar projects at theTrust’s Bridlington and Scarborough hos-pitals, with 87,000 tonnes of CO2 reduc-tions expected over the 15-year contract.Under a similar contract, the

Cheltenham General Hospital expects its

carbon emissions to be cut by 40 per cent (equalling 40,000 tonnes) over thecourse of the 18-year agreement with theenergy company.

When the Department of Health embedded BREEAM intothe design process for healthcare buildings in 2008, it didso with the aim of producing better conditions for staff,

outcomes for patients and overall environmental performance.Unfortunately, it isn’t fully achieving this vision.

Together with John Holmes and Graham Capper from theSchool of the Built Environment at Northumbria University, I haveundertaken research that shows only 15 per cent of NHS buildingshave achieved BREEAM certification. Breaking this down, of the110 buildings that are BREEAM certified, half received an‘Excellent’ rating and a third were judged as ‘Very Good’. To putthis into context, more than 150 office developments in the UK arerated ‘Outstanding’ on the BREEAM scale.

So why is this the case? One reason is a lack of value inBREEAM labelling. Looking again at the office sector, BREEAMhas made a big difference in normalising sustainability and elimi-nating false claims. However, hospitals aren’t competing to rent outfloor space, so the key commercial driver that’s made BREEAMsuch a success when it comes to office developments is not a factor.

We need to take a deeper look at the role buildings play inhealing and recovery. While there will always be certain design andengineering constraints – operating theatres and wards should bemechanically ventilated for example – designers can specify health-ier materials, combat noise and vibration, embrace natural lightand pursue energy efficiency. All these factors can play a part inhelping patients get better faster, which in turns frees up bed spaceand reduces operational costs. BREEAM doesn’t allow much flexi-bility in its assessment however, as the context of the site andclinical function often result in a bespoke approach to sustainabil-ity that cannot be easily measured in a standardised assessmentmatrix scoring.

The healthcare-specific BREEAM credits introduced in 2008were not compulsory and not universally used, with generic assess-ment criteria replacing them three years later. With no specificconditions to meet anymore, facilities have been designed to stan-dards that only partially apply. With tight budgets prevalent in thehealthcare sector, low priority has been given to obtaining pointsthat do not contribute to improved patient care.

However, it may also be true that BREEAM has made the designof buildings too prescriptive. Our research also found that manyprojects were doing the bare minimum required to pass the 70 percent ‘Excellent’ banding. It would make more sense to take a

comprehensive approach to the design of healthcare buildings thatencourages creativity and innovation. What is needed is morefreedom for designers and clients to set the sustainability agendafor each project and embrace the robust evidence that is needed toprove performance and delivery. This might mean that labellingcould be a little inconsistent, yet the label itself it not the real aim inhealthcare – the ultimate goal is better outcomes for patients, staffand communities.

I’d love to see the NHS and BRE work together to figure outwhat the next 20 years should look like. There are big opportuni-ties for the NHS to link social and economic benefit with outlay.BRE needs to be more challenging, looking at each site in moredetail and how each building will be used. �

In 2003 forward-thinking American architect Wayne Rugaformally defined his long-standing ‘generative space’ concept fordesigning environments that both systemically and sustainably

improve lives. Today the Caritas Project, the US-based non-profit organisation founded by Ruga, works globally topioneer a new and more desirable and healthier future for individuals, organisations, and communities by developing andcreating more generative spaces.

The generative space conceptIn essence, generative space is creating ‘a place to flourish’ –one where users are able to state, in their own terms, that their lives have been improved as a result of their experience in that space.Generative spaces represent the next generation of designing forhealth and it is a methodology that is applicable to all aspects ofour designed environments, beyond healthcare.Simply put, the design of a generative space requires the designerto understand what a user requires to improve their life, ofteninvolving gestalt psychology to enable a user to harness their expe-rience to inform the design.Any experience is the combination of the social and physical,which is a new approach for many design professionals, as historically the physical design and aesthetic has been the priority.A generative space will deliver measurable improvements. Forexample: to comply with requirements for sustainability, productivity in work and work outcomes, retention of employees,minimising staff absenteeism, and improved patient outcomes, it’snecessary to begin with a baseline metric. Using generative spaceshould then demonstrate improvements beyond the baseline thatcontinue over time.

Applying design research at MacmillanAfter Ruga defined and developed ‘generative space’ for designingenvironments to improve lives, he initiated the Leading by DesignResearch Project (LBD) under the auspices of The Caritas Project. The LBD project works with individuals around the world wholearn about generative space from Ruga and apply their learning totheir lives, projects, organisations, and communities.One of the LBD participants in Simon Henderson, formerly headof cancer care environments for Macmillan. As Simon Hendersonexperimented with a variety of initiatives to create generative spaceat Macmillan, he developed the Macmillan Quality Environmental

Mark to advance the quality of cancer care with the healthcareprovider organisations throughout the UK.Today, Macmillan’s Quality Environment Mark (MQEM) isdelivering its purpose and was accredited in 2016 by the CareQuality Commission (CQC) as an official information source fortheir inspections.

Subtle shiftCreating generative spaces is not difficult – but it does require asubtle shift in the traditional way of thinking that many architectsand design professionals have learned about in school and practice.Generative spaces require an entirely pragmatic and practicalapproach, working with individuals to develop environmental solu-tions that support the advancement of users’ aspirations – fromtheir perspective.As stated earlier, generative space is one that creates ‘a place toflourish.’ It really is quite simple: if the user says they are flourishing, the space is a generative one, conversely if the usercannot claim to be flourishing, the space is simply not a generativeone for them.

Active listening is crucialThe more active, ‘listening’ design professionals can do, tosupport the user requirements, the more chance they have to be‘accountable collaborators.’ The process requires investment inthe front-end of the design process, playing back the user feed-back so a mutual understanding of requirement is reached, beforethe physical design solution event starts.Although the concept of generative space has it roots in healthcare there is much that can be applied to all aspects of architectural design, peoples lives and the varying environmentsthey use.Creating life-enhancing spaces that make individuals feel betterand happier will lead to more productive people, whether childrenand teachers in a school classroom, employees in an office orhealthcare-givers and their patients – the end result of a generativespace can only be positive. �

Older people who are capable ofindependent living often find them-selves moving into comfortable,

retirement homes only to face a disruptiveshift to care-based accommodation whentheir health begins to fail.

There might be suitable accommodationat their existing residence but often it meansmoving to another address. Either way,

such accommodation is most likely to bedesigned more for delivering efficient carethan providing a pleasant lifestyle.

communal life in the spacious home theybought for as long as possible.

The landscaped 25-acre residential estatesits in a picturesque stretch of countrysidebetween Bath and Chippenham on the siteof Royal Arthur Park, a former Royal Navytraining station at Corsham in Wiltshirethat closed in 1993. The 221-apartment,£60m scheme gained planning permissionin 2009 with Phase 1 starting in Jan 2014and completing in November 2015.

The site rises gently north to south,surrounded on three sides by rolling coun-tryside with ancient woodland to the west.A central, three-storey pavilion and an

adjoining catering and entertainment build-ing is set against the woods, well below thetreeline to avoid dominating the site.

Radiating out from the pavilion is a seriesof two and three-story apartment buildings,laid out to form five triangular, landscapedcourtyards rotating around the centre butsignificantly separated from it.

Vital elementWadsworth Green is a project close to theheart of architect Dale Jennings, owner ofPencil and Ink, who was approached aboutthe project by client Rangeford Holdings inearly 2007.

“We had a large space to play with, givingus the opportunity to take a differentapproach to prevailing retirement and carehome concepts,” he says. “In essence, we’reproviding highly flexible care provision forpeople living in spacious, high-qualityhomes, designed to promote communalliving in beautiful grounds.“The care aspect is a vital underpinning

element of the Wadsworth Green’s concept.Unlike other places, we’ve avoided creatingdiffering types of accommodation desig-nated for ‘independent living’, ‘assisted care’or ‘extra care – all apartments are specifi-cally designed to adapt to residentschanging care needs.” All apartments are fully wheelchair acces-

sible and designed to accommodate ahospital-style bed with Rangeford providingthe full range of care available in a typicalcare home. Jennings adds: “Uniquely the combina-

tion of site and staff creates a therapeuticenvironment where people, even with chal-lenging conditions like dementia, can besupported and lead independent livesdespite significant care needs. Should their health begin to fail the vast

majority of residents can stay in the homes

they bought, unless they require constantspecialist support.”Plans to further enhance medical support

include building two doctor’s surgeriesoperated by local GPs as Wadswick Green’spopulation expands.

Social interactionWith the scheme placing a strong emphasison community-based lifestyle, significantopportunity for social interaction has beendesigned in.Much of this is centred around the recep-

tion, library, spa and 14-metre swimmingpool in the main Pavilion building and thelively restaurant and bar areas, known as‘The Greenhouse’ where indoor events arealso held. In good weather, outdoor eventsare staged on the entrance plaza.“The restaurant and bar interiors were

designed to high standard by the LovelyPubs group, to create a lively, atmosphericvenue that is offered to local people as abookable destination venue in its ownright,” says Jennings.“The outer apartments are based on

courtyards where people can sit and feelinvolved with the daily goings on.”

Residents have opportunities to mix withmembers of the local community, who havefree access to the grounds via the countryroads and footpaths linked to it. Dogwalkers, ramblers and people from thenearby villages regularly use the wooded,parkland-style grounds, and are encouragedto use the central facilities and participatein social events.

A golf buggy service moves peoplearound the site and a regular ‘on demand’bus service is available for trips outsidethe estate.

Courtyard livingEach courtyard captures unique views andthat defines its individual character, ratherthan varying architecture or colourschemes. Between 45 and 50 apartmentsform the two long staggered sides of eachcourtyard, with natural countrysidecompleting the triangle,” explains Jennings.

The rare benefit of having lots of landavailable has been exploited to spread theaccommodation out, making it morespacious, elegant and attractive to live in.This contrasts with many residential care

homes where accommodation is typicallyconcentrated in a few buildings adjoiningthe centre, primarily designed fordelivering care.

Architecturally, the courtyard apartmentsare unashamedly modern but utilise tradi-tional materials such as brick, render andtimber to give them a soft, familiar finish.Critically, the apartments are built aroundstaircases, forming a series of staggeredblocks to create the triangular courtyards.There are no communal corridors,but stair-wells and shared hall areas are litby skylights.

Jennings says: “The aesthetics of the twoand three-storey courtyard apartments areinspired by Eshrick House, a classic 1960sflat-roofed woodland home by Americanarchitect Louis Kahn. It's finished withsimple materials and beautiful joinery andfeatures a double-height living room andhuge glazed windows. I wanted to capturethe spirit and feel of that building.

“All courtyard apartments are spaciouswith elegant proportions featuring large areas of glazing at both ends of awooden-floored lounges, so there’s plenty of light and interior finishes are of superior quality.

“The supporting red brick walls are built in Flemish bond for a more interestingappearance. All the buildings have flatroofs, the taller ones featuring single-plymembranes. Lower-lying buildings, usuallyon the edges of the development next toparkland, have sedum roofs sympathetic totheir location.”At one end of the lounges a large picture

window offers superb countryside views.On the exterior these windows are framedby tall, eye-catching, white-rendered rectan-gular features sitting proud of the walls. Atthe other end of the lounges glazed doorslead to wide, flush-floor balconies withhardwood railings overlooking thecourtyards. “You get two experiences in one room: a

private sense of escape with the countrysideview and visibility and engagement with theneighbourhood at the balcony end,” saysJennings.Various strengths of low-e glass provide

solar control, depending on the orientationof the building, while ventilation andcooling is achieved through openingdouble-glazed windows. Underfloor heatingis provided, powered by central gas-firedboilers serving the whole site.

Central buildingsStructurally, the central, three-storeyPavilion holds communal facilities includingthe 20-metre swimming pool and spa, hair-dressing service and reception, plus back-of-house facilities such as laundry, staff chang-ing rooms and a small number of offices.Above it are two floors containing24 well-appointed one and two-bedroomhotel-style apartments with balconies.With a 12,000 ft2 footprint, the Pavilion

is constructed from modular concrete slabup to the first floor clad with vertical cedarboarding and the two floors above madefrom a Metsec metal stud and joist systemfinished with modular brick cladding and aflat, single-ply membrane roof. “Overall, it’s a fairly simple structure

enhanced by The Greenhouse to the front ofit,” says Jennings. This is a 3,000 ft2 single-storey, glass-sided structure containing therestaurant and bar, two terraces, onecovered and one completely open, overlook-ing a communal plaza. “We’ve used asingle-ply membrane to look like a tradi-tional rolled lead roof on the Greenhouse.”Internally, the Greenhouse features

open to provide natural ventilation and atimber soffit to the brise soleil aroundits perimeter.

Rural spiritThe mature, parkland-style landscaping iscentral to the spirit of the place and capi-talises on the rural location, with the layoutdesigned where possible around existingmature trees.

Residents even have their own privateaccess road, a leafy 1 km lane with passingpoints and country estate-style fencing.

Natural golden coloured Cotswold stone,dug up during construction, is reused tobuild low walls and low-lying ancillarybuildings with sedum roofs, complementedby garden-style plantings and beds oftenused to increase privacy for groundfloor homes.

Paved footpaths form an attractivenetwork of walkways lit by subtle, wooden-posted LED downlighters designed tominimise light pollution although in appro-priate locations trees are also subtly up-litas a visual feature.

The grounds also contain two visuallyappealing natural wildlife ponds, containingprotected great crested newts. They havebeen carefully planted to deter casualaccess, while two roosts have been specifi-cally built for the common pipistrelle andbrown long-eared bats native to the site.

With so much thought going into theconcept, layout and architectural design,Jennings is confident that the WadswickGreen model works.

He comments: “The other day a resident’sdaughter said to me, ‘My mum has neverbeen happier than she is here now.’ For me,that’s the true measure of success. �

Inspiration to design buildings fit forpurpose is the lifeblood of every architectural practice – but rarely can

that phrase have been as apt as it is for oneof Reiach and Hall’s current projects.The Edinburgh practice is now very

close to seeing its vision for a new £33m,state-of-the-art blood transfusion centreserving the whole of Scotland become areality.Developed through a joint venture

partnership of Kajima and Interserve, theScottish National Blood Transfusion Service(SNBTS) centre of excellence, or JackCopland Centre as it will be known, shouldbe fully operational by summer this year.It’s being constructed at the Heriot-Watt

University Research Park on the south-western edge of Scotland’s capital and willcombine processing and testing laboratoriescurrently in Glasgow and Edinburgh, alongwith the Service’s national headquarters. Richard Coe, project director at Kajima,

explains: “SNBTS will be able to consolidate at one site and move forwardswith lean pharmaceutical production andtesting of blood, cells and tissues, plusprovision of a range of other clinical services, to meet anticipated increase indemand in a safe and cost-effective way.”That sums up the service goal for the

facility but the demands of the design challenge are enough to raise anyone'sblood pressure!

Unique challengeWhile Reiach and Hall’s Andy Law says hiscompany has a track record of designing

buildings with laboratories, he admits:“This is a unique facility. It’s a hugely technical brief, but there’s a social brief too– it needs to be a good place to work. Withthese sort of facilities, the social side is theone that normally suffers. Our challengewas to manage the technical side in such away that you still have an environment thatpeople enjoy working in. I think we'veachieved that by taking both challenges onat once.”

So, what will the Jack Copland Centre doand what is the technical challenge? Itsprimary function is the collection of blooddonations from five static donor centresalong with those from community andworkplace collection sessions acrossScotland. These donations will then betested and processed into red cells, plasmaand platelets before distributing those endproducts to patients. Additionally the Centre will process, test

and store tissue and cell products, as well ashouse a hub for cutting edge cellulartherapy, research and innovation. All of this work is subject to strict

guidelines and inspections, designed to ensure a safe and secure working environment. One way this was achieved inthe design was to ensure that waste wouldhave a specific route out of the buildingwhich would never ‘cross paths’ withincoming materials or outgoing products.Law explains: “We started with a diagramof the flow pattern that's like a verycomplex bowl of spaghetti.“It’s the SNBTS mindset to be incredibly

rigorous about keeping everything separate.A failure by SNBTS can mean serious risksto patients so they must be incrediblycareful.”

Designs that separate and connectIn order to keep materials and productsapart, yet ensuring staff still feel connectedwith each other and different parts of thebuilding, Reiach and Hall developed a two-part solution: accommodate the facil-ity’s different functions in strips builtalongside and above each other, and exten-sive use of glass throughout.Law explains: “The solution we

presented, which people didn’t expect, wasto make part of the building over three-storeys rather than two. We put the testingand Research, Development and Innovation(RD&I) function on the top floor andlinked these down to the blood processinghall on the ground floor with dumb waitersfor samples to go up and down. That reallycracked some of the difficulties of gettingthe complex circulation systems to work.”In between, half of the first floor is a

dedicated plant area filled with servicesequipment. This is one way the building’sdesign tackles the issue of the sheer volumeof services needed yet ensuring they areclose to where they’re needed.At the heart of the Jack Copland

Centre, and running parallel to the bloodprocessing hall, services floor and top-floortesting and RD&I, is a space known as the

Arcade. Law says: “It’s called the Arcadebecause you do walk along it but it is also athree-storey atrium, with open gallerieslooking into it and containing the verticalcirculation.”With the facility’s main entrance at one

end and a cafe/bistro at the other, Law says the Arcade plays an important role in“pulling the building together.” The use ofglass here and numerous glass partitionselsewhere in the facility, produce a trans-parency that provides visual connectionbetween different areas, as well as an ambience of space and light.On the other side of the Arcade is the

office area spanning three floors. Not only does this office space look out ontolandscaped parkland but internal glasspartitions mean staff in this space can alsoview the processing hall and top-floor labsacross the Arcade.This transparency is repeated elsewhere.

A glazed partition on the north side of theblood processing hall allows views througha tree-lined courtyard to the tissue and celllabs beyond.All this glass, says Law, helps achieve the

social aim at SNBTS of making the JackCopland Centre not only a pleasant place towork but also a place that makes peoplefeel “connected to each other and to thecountryside outside.”

Pressure to succeedWhile glass partitions ‘dissolve’ boundariesvisually, they still provide the required separation – indeed, many of the lab spacesneed to be airtight. Not only that, differentactivities stipulate different air pressures. Law explains. “In some labs staff may be

working with samples that could containsomething nasty which you don’t wantleaving that lab in the air, so we negativelypressurised that lab relative to the spacesaround it. In other labs SNBTS is guardingagainst anything coming in so they will bepositively pressurised.”Eventually, you need to return, via a

‘pressure cascade’, to ambient pressure in places like the Arcade. Following discussions with SNBTS this was achievedby putting extra doors across corridors tocreate ‘air locks’.Specialised glass with specific detailing

has been used for higher-grade clean roomsand labs, while a carefully adapted standardglass partition system is used elsewhere. Another technical requirement demanded

of the building design was the need for adedicated area for staff to change intospecial clothing and ensure a sterile

environment. The solution is to have onestarting point for all employees – helping to connect people from different functions –before they progress through further levels of the ‘changing regime’ as appropriate to their work zones and thetasks they perform.Structurally, while the majority of the

SNBTS facility features a steel framework,the three-storey office element has aconcrete skeleton. The concrete contributesto the natural cooling of the office area,along with louvered vents above windowsthat are controlled by the sophisticatedbuilding management system. It is hopedthat this natural ventilation – plus photo-voltaic panels, combined heat and powersystems and enhanced insulation – helps thefacility achieve a BREEAM Very Good rating.On the outside, most of the facility’s

facades feature a brick cladding. Law

explains: “The reason was to reflect thatthis is an important national facility so itneeds something that demonstrates its gravitas and its permanence.”

A Dutch masterpieceThe use of glass is not the only approachused to also make the state-of-the-art facility a space people will enjoy. Where theservices-only floor meets the Arcade there isa 50 metre-long, four metre-high blankwall. This has proved a golden opportunityto involve Dutch artist Steven Aalders.Law reveals: “We’ve worked with Steven

before and for this project he producedsome fantastic artwork to fill the space.

He’s used a range of colours in a geometricpattern that’s very striking. It’s the domi-nant feature in the Arcade and we’re verypleased with it.“We’ve used his colours as a clue in

choosing the fabrics of the carpets andfurniture of the facility and Steven has also created a floor pattern in the Arcade to relate to his artwork. In addition, wehave to include an element of opacity in the glass partitions as a safety measure tostop people walking into them, so we’vefeatured reduced monochrome versions of his artwork in the glass. It all brings a sense of identity, to the Arcade particularly, and helps to make it into more of a social space.”Reflecting on the SNBTS project,

Richard Coe is full of praise for the close-working relationship between Kajima, Interserve, Reiach and Hall, SNBTS and the NHS. Richard concluded by explaining: “This is one of the biggestand most significant building projects forNHS Scotland, and is pivotal for the futureof the Scottish National Blood TransfusionService. When in life do you get this sort of opportunity – Andy and I have been very lucky to be involved with this amazing building.” �

concept, the new Dumfries and GallowayRoyal Infirmary is an aesthetically appeal-ing, well-organised, user-friendly, low-risedesign that brings ‘the outside in’ whilebeing sympathetic to its surroundings.The hospital will serve many small,

widespread and diverse communities withinthe 2,400 square miles covered by NHSDumfries and Galloway in Scotland’s SouthWest and is centrally sited in a semi-rurallocation off the A75 road.

Architecturally, the facility comprisesthree core elements. A central diagnosticand treatment ‘bar’ is the primary component, forming the backbone of thecampus. Extending out from behind thelinear-shaped bar and projecting into thelandscape, are three independently linkedpavilions of inpatient accommodation. Atriple-sided Women and Children’s Unit –featuring a curved outward-facing facadeand internal courtyard – adjoins the main building.The district general hospital has been

Dumfries itself cannot realistically beexpected to meet demand long-term. Medical facilities in the main building

include a combined assessment unit, accident and emergency facility (A&E), asurgical complex with four major operatingtheatres, oncology and critical care units,and an outpatient centre.The £212m hospital was procured under

the Scottish Government Non-ProfitDistributing (NPD) model, with Ryder asthe lead architect collaborating with NBBJand Laing O’Rourke as main contractor.

Key themesDesigning a greenfield hospital from theground up sounds like an architect’s dreambut the team had to ensure they also metthe detailed healthcare requirements and besympathetic to an earlier ‘reference design’the board had drawn up to obtain outlineplanning approval.Working closely with the board, the

practice evolved a series of key objectivesinto four core themes that informed andunderpinned the architecture and interiordesign, with a strong emphasis on quality. Firstly, the design needed to be people-

centred, attractive and provide uplifting andtherapeutic environments with naturalviews and plenty of good quality daylight.Secondly it had to facilitate efficient operation, good space utilisation, clearwayfinding, innovative use of IT, long-termflexibility and easy analysis of clinicalworkflows. Creating a safe, welcoming and accessible building and enabling staff to deliver effective care formed the remaining themes. Bearing these in mind, Ryder and NBBJ

looked at the reference design to see wherethey could improve it, as partner Paul Bellexplains. “The board’s aspiration to have anintegrated emergency care centre hadn’tbeen realised as the combined assessmentunit and A&E were quite separate. We

redesigned the emergency care area toincorporate the combined assessment unit, which in turn meant we could bringthe ward buildings much closer to the diagnostic assessment ‘bar,’ reducing keyinternal travel distances and improving clinical efficiency.”The architects also segregated the main

internal circulations by floor to avoid creating shared flows or cross-flowclashes. Now, the majority of outpatientand visitor movement happens at groundfloor, while the inpatient flow, linking thesurgical wards and maternity wards to theoperating theatre suites and critical careunits, is on the first floor. Facilitiesmanagement-related traffic happens atlower-ground level. Circulation routesinclude rest spaces for older users andareas designed to promote interactionbetween hospital disciplines.Bell adds: “A key design move was ensur-

In the referencedesign the position of the Women andChildren’s Unit blocked visibility of theemergency care entrance, so we moved it togive a clear route to emergency care. “Respositioning the Women and

Children’s Unit allowed us to locate itsentrance immediately adjacent to the main

hospital entrance, sharing a canopiedentrance plaza to improve wayfinding.”

SympatheticNestling in a semi-rural location, the external appearance of the building has been shaped to be sympathetic to its surroundings.The precast panels on the main building

facias are honey-coloured, reflecting theGlasgow Blonde stone frequently used insouth west Scotland. A reconstituted stonefinish similar to white Galloway granitegives a contrasting, lighter treatment to theinpatient pavilions and Women andChildren’s Unit uses. Pitched-style roofing is used throughout

the development to break up the mass ofthe building, so that it sits comfortablywithin the hillsides and woodlands stretching from the south west to the northeast of the campus.The gables formed by the pitched roofs

are clad with angled concrete panels reflecting the woodland campus idea, withthree different designs for the wards, diagnostic and treatment building andWomen and Children’s Unit.“Our intention has always been that

each of the three ‘fingers’ of ward buildingsprojecting south west from the diagnosticand treatment centre, should create animpression of a series of pavilions,” says Bell.

“Ward accommodation comprises a seriesof single, en-suite bedrooms in low-heightstructures naturally dictating a long, linearshape for the ward buildings. We broke upthis linearity by incorporating verticaldormer features along the sides of eachfinger. These are public circulation areas –day space – with large oriel windows allowing extensive daylight penetration.“Additionally, each ward room features

large, tall windows running from the ceilingalmost to the floors to punctuate the linearity, while giving patients attractivelandscape views from their beds and a goodsource of daylight.”Natural light streams through two large,

unglazed rectangular openings in the roofof each ward building roof, creating lightwells inside. The well walls are glazed,forming open courtyards in the wards,giving staff views across from one corridorto another to help them observe patients orsee if colleagues need support.

Environmental integration“Integration of the internal environmentwith the natural landscape is a crucialcomponent of the project, as is the

landscaping on the campus in general,” saysBell. “We liaised extensively with landscapearchitects Fira, to meet the requirement.”He adds: “Between each ward building

are spacious accessible gardens wherepatients can experience various garden environment spaces, paths and glades, someused for therapy purposes, others forwalking. The relatively loose grouping ofthe buildings also permits landscape viewsfor people inside. The central courtyard ofthe women and children’s unit also has aplanting area. “Palliative care rooms have their own

garden terrace, and beds can be taken ontothem so patients can experience the external environment,” says Bell. “Thecourtyard at the centre of the Women and Children’s unit is generously proportionedand attractively landscaped to include children’s play spaces and quieter areas.Car parking is in curved lines on the

site’s eastern side, softened with extensive planting that extends beyond the perimeterto form a green barrier to the light commercial buildings beyond.The main entrance reception has a

suspended wooden ceiling and is lit fromabove by a long row of skylights andaluminium-framed curtain wall glazing with countryside views. Bell points out:“This creates a pleasant welcome to thehospital, and from there the obviousprimary circulation routes make it is easy to find the wards and other inpatient and outpatient services. partitioning featuring artwork by Scottishartist Liz Myhill, whose designs also appearon glazing elsewhere in the main building.Like the wards, the main building features anumber of light wells to bring additionaldaylight in.Gas and combined heat and power

generators provide electricity and hot water,with excess heat recycled via underfloorheating in the larger ground floor spaces. A mixture of opening windows andmechanical ventilation control the interiorenvironment, and there is a plan to supply much of the water from a naturalaquifer under the site. These features,combined with excellent insulation, LEDlighting and appropriate solar-controlledglazing, have resulted in a BREEAM VeryGood rating.All structures have piled foundations

and are concrete-framed with high-quality precast concrete panels with architectural finishes on the facades. Steel frames create the pavilion shapes,topped with grey aluminium or zinc standing seam metal roofing.The building extensively uses offsite-

manufactured, precast components – aspeciality of Laing O’Rourke – bringing thebenefits of quick, accurate and cost-effective construction.

A ‘hospital without compromise’With the building now firmly on target forcompletion in September, Bell says: “We setout to deliver a hospital without compro-mise to provide excellent healthcare forpeople in the region. A hospital thatmaximises the potential of its wonderfullocation by bringing the qualities of thesurrounding landscape into the site andcapturing the healing properties of light andviews to nature, to provide excellent health-care for people in the region.“The successful delivery of a project

on this scale within such a demandingprogramme, will be a significant achievement only made possible by a great team effort. “We hope we’ve created an uplifting,

healing campus environment offering anattractive and highly effective setting forpatients, staff and visitors – a truly person-centred hospital.” �

Hoisting systems are one of the most significant pieces of kit in any healthcare environment,

providing users with a good quality of lifesimply by facilitating the need to movefrom place to place with dignity.

As healthcare environments are designed and built to cater to the needs ofthe end-user, hoisting systems should beincluded to guarantee that patients arefreely given the tools they might need to get around.

EvolutionRecent industry trends have instigated a makeover on the old faithful hoistingsystem. Sleeker, smarter hoist units are favoured for their combination of subtle aesthetics and robust functionalitythat helps reduce patient stress when transferring. The tracking system itself hasevolved. The inset track sits in the ceilingand looks a lot smarter while being easierto clean than standard tracking.

Alternative solutions to the trackingsystem are available; for example, somehidden hoists can be wall-mounted andfolded away when not in use, fostering amore homely environment.

Room layoutsDifferent room layouts require differenttrack set-ups. Straight tracks are fairly self-explanatory: they are a single rail thatenable basic movement from one point toanother. They can be installed with curvesor with transit coupling and track switchesthat enable change direction.

X-Y systems (also referred to as Hframes) offer a wider range of hoisting andgreater flexibility of care. X-Y systems havetwo parallel fixed rails and a perpendicularmoving traverse rail that can pick up and

hoist across the range of the fixed rails.They can also be adapted to connect toother fixed tracks, or even another X-Ylayout, and can be installed and mountedon wooden joists, concrete, or steel fixings.

Operating environmentsThe next thing to consider is the areas thatneed to be accessed. Beds, showers, baths,and toilets are all common ‘pick up’ points.Multi-user environments are more challeng-ing as they require the equipment to meetthe needs of every individual and work inconjunction with other apparatus, such ascurtain rails.

X-Y systems are designed to be suitablefor multi-user space as they offer access toany part of the area. With the fixed, paral-lel rails installed wide apart, the traverserail is free to move. Curtain rails can benavigated using track systems and gatesthat create a gap in the hoist rail or thecurtain rail to allow them to pass betweenone another.

Rooms for bariatric patients need to belarger and fitted with double doors toenable manoeuvring. Here, an X-Y systemis best suited as it utilises the width of theroom. This system can fit to another X-Ysystem, with the track passing through thecentre of the double-door.

SpecifyingWhen specifying, consideration should begiven to the layout of the room, the trackfixings and the client’s needs. Hoist special-ists should be involved to suggest the mostsuitable track layouts and configurations,while architects can also benefit fromprofessional development sessions.

Until recently, thermostatic mixingvalves were installed as a matter ofcourse in healthcare facilities to

prevent scalding incidents. However, the Health and Safety

Executive’s guidelines HSG274 Part 2 say“The use and fitting of TMVs should beinformed by a comparative risk assessmentof scalding risk versus the risk of infectionfrom legionella.”

As a result, there has been marked inter-est in specifying other technologies todeliver safe, temperature-controlled water.

The risk of scalding depends on the useand the user. A low-level of risk could be ahabitual user who is familiar with the outlet and the type of mixer, for example,healthcare workers using a particular basinfor regular hand washing.

An intermediate risk level could be hospital visitors who use the public facilities when visiting patients. Althoughunfamiliar with the facilities, they have nosensory or mobility issues and are able touse the washbasins safely, especially if thereare visual warnings about hot water.

Pressure-balancingWhere there is high frequency usage by visi-tors, pressure-balancing mixers can providean intermediate anti-scalding solution (seepanel overleaf for details).

The ceramic cartridge is sensitive tochanges in pressure. An internal shuttlecontinually adjusts to the incoming hot andcold water supply pressures to ensure aconstant temperature at the outlet.

If the pressure suddenly drops at the coldinlet, the shuttle reacts immediately and thehot water flow is reduced to a trickle.Likewise, if the hot water pressure drops,cold water is reduced to a trickle.

The pressure-balancing cartridge alsofeatures a maximum temperature limiter sothe visitor is protected from temperature

spikes due to pressure drops or pressuredifferences in the system.

Thermostatic technologyHSG 274 part 2 states: “Where a scaldingrisk is considered significant…then type 3TMVs [TMV3] that are pre-set and failsafeshould be provided.”

To fail-safe, the hot water must shut off ifthe cold water fails and vice versa. This iswhere pressure-balancing cartridges andthermostatic cells differ. The latter is able to provide a complete failsafe to TMV3 standards.

The most serious scalding risk is wherethe user is fully immersed in either a showeror a bath, predominantly in non-criticalpatients’ accommodation and residents’accommodation in care homes.

Where the scalding risk is significant –for example the very young or elderly, thosewith sensory loss, the infirm or significantlymentally or physically disabled people – theguidelines recommend the use of TMVs forsinks or hand washbasins.

Other considerationsA key factor in the development ofLegionella bacteria is standing waterbetween 20°C and 46°C. Pressure-balanc-ing mixers feature a single mechanismwhich controls both flow and temperature.The water circulates from the inlet to themechanism inside narrow copper tubes.

These have two benefits – standing wateris minimised and water flow is accelerated.Both these factors minimise the develop-ment of biofilm, which provides shelter anda source of nutrients for bacteria.

Many mixers used in public and commercial environments are cast in brass.The moulds are made from sand andalthough the external surfaces are platedand polished, the interiors are rough, withniches where bacteria can adhere.

Taps with smooth interiors have fewer niches where bacteria can adhere,significantly slowing biofilm development.Moreover, as a preventative measure, cleaning mixers in a descaling solution willfurther reduce biofilm build-up. If thosemixers can be removed easily, they can be regularly cleaned or disinfected if a contamination episode occurs.

Advances in anti-scalding technology and infection control have led to a

multitude of products that offer differentsolutions for patient, staff and visitor safetyin healthcare environments. Certainly thetechnologies are not mutually exclusive andnot always complex. It is possible to controlLegionella and provide scalding safety formost situations and accommodate thecomparative risk levels.

HOW PRESSURE-BALANCING MIXERS WORKFigure 1 shows a ceramic cartridge where both the cold and hot water supplypressure is 3 bar, and the outlet temperature is pre-set to 41°C. If the coldwater pressure drops to 2.5 bar and the hot water pressure remains at 3 bar,the higher pressure at the hot water inlet causes the shuttle to displace, restricting the hot water )ow into the mixing chamber while simultaneously allowing more cold water to )ow.

As a result, the mixed water at the outlet remains at 41°C (Figure 2). Similarly, if the pressure at the hot water inlet drops to 2.5 bar and the cold water pressure remains at 3 bar, the shuttle adjusts the apertures to increase the hot water and reduce cold water )ow into the mixing chamber.(Figure 3).

The task of hydrating hospitalpatients or residents in a care home is vital. When it comes to the

drinking water system, the emphasis has tobe on hygiene – and the consequences ofignoring it can be dire for vulnerable users. Good hydration has been shown to have

an impressive impact on health and wellbe-ing but it must be the correct type ofhydration – helping clients to meet theirduty of care.

Design processIncorporating drinking water systems intothe design process for healthcare buildingsis crucial. Architects should start with arigorous evaluation of the client’s needs, theoverall design of facility and the nature ofthe care itself. Is it critical care? How dothey manage patient care, and what is theirsanitisation regime? The key question is what’s the

best option for the patient? Cancer patientsare far more susceptible to airborne and waterborne germs and contaminants thatpose a greater risk to health. While in non-critical environments, another type ofsystem which prioritises a more person-centred approach over just hygiene couldbe more appropriate. The design process should allow for both

space and supply. Space is at a premium inhospitals – the risk of patients or nursesbumping into poorly located drinking watersystems is a nightmare scenario, one whichcontravenes duty of care. The key question is, where are the key

areas that water needs to be supplied to? Byconsidering this in the design process,clients are far better placed to ensurepatients and residents are at the heart oftheir plans.

Product optionsFor chilled still water there is the standardpoint-of-use water dispenser or bottled

water cooler, the kind you see in standardoffice buildings, but for healthcare environ-ments it’s often best to consider self-sanitising equipment designed forhygienically sensitive areas.In care environments, these systems

are tailor-made to combat airborne and waterborne hazards. It’s worth considering a filtered water

system that constantly self-cleans thenozzle, killing bacteria, and has a filterpreventing bacteria from travelling. Thesesystems are designed to heat up the nozzleto 110°C every 90 minutes and help stopbacteria moving up the pipework into thecooler itself. Coupled with an additional0.2 micron filter before it goes through the sanitised nozzle area and a 0.5 micronfilter for the pipework coming into the back of the machine from the mains-fedwater – as a result little or no bacteriatravels either way.Another aspect of the decision-making

process is whether to install a centralisedsystem. This eliminates the need to use individual drinking water dispensers atseveral locations in the building. As well astaking up far less space, these systems canbe up to 55 per cent more energy efficientthan individual dispenser units.When it comes to boiling hot water, think

about the safety, freshness and speed thewater is dispensed at. A lot of healthcareproviders use kettles, where time and wateris wasted waiting for a full kettle to boil, orlarge drinking water boilers, where waterloses freshness over time.Instead, architects and specifiers should

look at choosing a system which offersmains-fed, purified instant boiling hotwater from one single tap with a touchsensor control, and zero splash.

home and having access to instant chilled,filtered water or instant boiling water cansave the extra time waiting around for akettle to boil. Forrest comments: “It’s fiveextra minutes a relative can spend withtheir mum or dad. For example, a personwith dementia who is still relatively independent may want a cup of tea butafter a few minutes may forget why they’veboiled the kettle. With this system, they canhave a hot drink instantly.”

Architects’ roleThere are clear advantages and disadvantages that architects and specifiersshould discuss with their clients. Is hygienethe sole concern or is being person-centred

also important? Is a system which may bemore expensive in the short-term going toprovide long-term value? When taking all these factors into

account, architects and specifiers are urgedto seek innovations which support patientcare and not to simply go with the norm. They have a crucial role to play in

educating the client and ensuring the bestoption for the patients and residents ischosen. Healthcare environments will be allthe better for it.

Acarefully considered interior designscheme is essential for any care environment, especially where there

are residents living with dementia. The quality of the acoustic environment

is a vital component of good dementia-friendly design as noise is regarded as ahealth and safety issue and should not interfere with a resident’s normal domesticactivities including sleep and rest.

Noise is transmitted in buildings by bothairborne and impact sound sources and UKBuilding Regulations require that boththese noise types are controlled.

People need to be able to hear well inorder to make sense of their environmentand in order to function well as part oftheir quality of life. Research from StirlingUniversity shows hearing impairment cancompound feelings of isolation and frustra-tion and these feelings contribute tobehavioural disturbance.

It is essential that adaptations thatsimplify and clarify the acoustic environment, and reduce discomfort andauditory ‘clutter,’ are put in place. Goodacoustics can actively contribute to ensuring that a person with dementia can communicate and remain includedwithin their community. Belonging andinteracting are highly dependent oncommunication, which in turn is highlydependent on hearing.

Risk of fallsIn addition, as hearing is linked to balancethere are greater risks of falls and this canbe devastating for an elderly person.Headline-grabbing research carried out bythe Royal Society for the Prevention ofAccidents (RoSPA) in February 2004showed that accidents on hard flooring hadincreased by over 300 per cent in the previ-ous five years.

Carpets can provide a cushioned landing

for any trips and falls and studies carriedout in 1996 have shown that carpet, whencompared with vinyl, can reduce injuriescaused by such accidents. It has also beenproven that gait, speed and step length isgreater in older people walking on carpetedareas than when walking on vinyl.

UK Building Regulations stipulate that asuitable floor covering should have aweighted reduction in impact sound pressure level of not less than 17 dB whenmeasured in accordance with EN ISO 140-8 and calculated in accordance withEN ISO 717-2.

Furthermore, Stirling University in theirDementia Design Series guide – ‘Hearing,sound and the acoustic environment forpeople living with dementia’ recommendthat the design of rooms within caresettings must find ways to minimise transmission of noise from one room toanother and that sound impact ratingsshould preferably exceed British Standards.

BREEAM creditsThere is increasing responsibility on thehealth sector to address sustainability in theprocurement and management of theirestates. Environmental requirements andtargets, including BREEAM Healthcare and the aspiration of zero carbon publicbuildings by 2018, make sustainability akey priority in the design and operation ofhealth facilities.

dwellings). Therefore, to achieve a BREEAM Excellent rating it’sbeneficial to have acoustically sound rooms.In addition to providing an acoustically sound environment,

carpets have extra benefits, especially when installed within thecare environment where residents may have breathing difficultiessuch as asthma.Throughout the past 10 years there have been numerous studies

into the use of carpet versus hard floor surfaces and what effectthese two flooring solutions have on air quality. Fine dust canpresent a significant health hazard, especially for allergy sufferers,as particles may cause irritation when they are breathed in andenter the respiratory tract.Many of the studies suggest that carpet retains dust particles,

unlike hard surfaces where they regularly become airborne. Ifcarpets are regularly vacuumed these dust particles – and allergensthat are bound within fine dust particles – are removed from theroom without causing discomfort.

Warmth and comfortUnlike hard flooring options, carpets add a warm and welcomingfeel to any room and provides a soft cushioned layer for extraunderfoot comfort. Research suggests that the insulation provided by carpet is 10

times higher than that of hard flooring, making it one of the mosteffective thermal insulators. Studies have also shown that thetemperature felt by residents in carpeted rooms is two degreeshigher than in rooms with hard flooring.Acting as a thermal insulator, carpets improve energy consump-

tion and have low heat conduction. It is estimated that up to 30days of heating can be saved by installing carpets, resulting in afour to six per cent energy saving and a consequent reduction inenergy costs.

Low surface temperature (LST) radia-tors have been part of the heatinglandscape for several years now, but

modern versions offer a number of new benefits.

LST radiators are key where there is aneed for specialised heat emitters for safetycritical environments – especially those thatneed to meet NHS Guidance for ‘Safe hotwater and surface temperature.’ This includeshospitals, care and nursing homes, as well assheltered housing, schools and nurseries.

It’s a specification requirement of build-ing designers and architects that the heatemitters in these buildings protect theyoung, the elderly and the vulnerable, whowill live in or visit buildings such as these.

When asking whether LST radiators areneeded, you only need to consider the riskof older people falling and injuring them-selves on a radiator along with potentialdangers from those with a visual impair-ment bumping into or brushing againstradiators.

Some elderly people have a reduced sensi-tivity to high temperatures and otherssimply cannot react quickly enough toprevent injury from hot surfaces if theyinadvertently touch and come into contactwith a conventional radiator, where surfacetemperatures may be as high as 75°C.

Older people and those suffering injurieshave an increased susceptibility to losingtheir balance and falling, and the possibilityof these people falling and becomingtrapped against a conventional radiatorbrings the need for LST radiators sharplyinto focus.

And of course, the injuries that are possi-ble to the very young are too horrific tocontemplate, so it’s clear that serious burnscan be caused very quickly at these temper-atures and that LST radiators are theobvious response to the dangers.

ProtectionIncorporating a casing that covers the radi-ator – providing a physical barrier betweenthe heat emitter and the people they aredesigned to keep warm – makes hugesense. In the case of most LST radiators,the casing also covers the incomingpipework, making sure all the heatedsurfaces are concealed and that anyexposed surfaces stay at a safe temperatureof no more than 43°C.

In addition, LST radiators need to bedesigned to ensure they offer as muchprotection as possible to those for whomthey are designed to provide heating –including ensuring rounded corners andedges to the design to avoid damage from sharp edges.

Additional benefitsRemembering that LST radiators are in themain installed in buildings to offer protec-tion to older people, additional benefitssuch as safety grilles, easy to operateheating controls and even arthritic adaptorsfor people who are unable to grip controlseasily, can be incorporated to provide a userfriendly ‘package’ for those they seek to help.

Bearing in mind where many LST radiators are installed, it’s vital that they are built to be robust and able to withstand and resist vandalism and day to day wear and tear, from being knockedinto by trolleys and wheelchairs – especially in hospitals and nursing homes.With higher quality radiators, the casing isheld by security fittings that are quick andeasy to install.

The security fittings what is thought tobe a unique seam fixing bracket system thatprevents unauthorised access to the emitteraccommodated inside. For additionalprotection, good quality radiators have an

additional long lasting, anti-bacterial paintcoating, which provides an additional valu-able benefit for the environments in whichthey are installed.

BeneficialInstalling LST radiators has certainlyproved beneficial to The Ridings CareHome in Swindon, which has 32 self-contained flats catering for a mix of activeelderly individuals and couples, many with24-hour care needs.

It had been operating an ageing electricunderfloor heating system, with electricimmersion heaters providing hot water. Itwas expensive to run and needed replacingwith a modern, reliable energy-efficientalternative.

A traditional wet heating system wasselected, with modern gas-fired condensingboilers providing the heating source, withtwo additional gas boilers operating inconjunction with solar thermal and solarPV panels to preheat the cylinder andprovide hot water.

But with differing levels of disability andcare requirements to cater for it was impor-tant to select radiators that prevented thepossibility of heat-related injury.

More than 80 LST radiators wereinstalled with additional integral remotesensing thermostatic valves to allow adjust-ment of room air temperature, making themhighly energy efficient. They also featuredan arthritic adaptor designed to make iteasier for older people, who may have prob-lems with their grip, to adjust heat levels intheir rooms.

Ultimately, it was the decision to specifyquality, high-specification LSTs that madethe installation of a desirable, cost–effectivewet system viable.

Water, essential for hygiene, can also be a source of infection if itsquality is not controlled. The

Health and Safety Executive (HSE) recommend that hot and cold water systemsin all buildings open to the public should bemonitored for Legionella, including hotels,student accommodation, campsites etc., andnot just healthcare facilities.

The HSE Approved Code of Practice andGuidance document L8 states that dutyholders should: identify and assess sources ofrisk of exposure to Legionella; put in placeprecautions to prevent or control that risk;and monitor these measures to ensure thatthey remain effective. If contaminationoccurs, point-of-use filters should be used as a temporary control measure until a permanent, safe solution is put in place.

Kemper System’s Kemperol 2K-PURsolvent-free and odourless water-proofing membrane has been used to

refurbish the roof of Poole Hospital’soutpatient department, ensuring that theproject avoided any odours that couldbecome a nuisance to staff and patients in theunit or the surrounding wards.The cold-applied liquid membrane was
