Telehealth & Telemedicine in 2015: Remote Monitoring, Wearable Devices Upgrade

Burgeoning IndustryFrom early detection of impending heart failure from as far as 3,000 miles away, to the latest wearable wrapped around wrists or bodies, or simply sitting in pockets, telehealth is having a radical impact on the healthcare industry. The range of telehealth and telemedicine services has expanded in the two years since the Healthcare Intelligence Network (HIN) last conducted a telehealth survey, and while the majority of organizations had telehealth programs in place in both 2013 and 2015, of those that didn’t, nearly triple the respondents in 2015 (64 percent versus 26 percent) said they would launch a service in the next 12 months.

A close look at those future programs offers insight into a market predicted to rise to 7 million in 2018, according to IHS Technology. By bringing on board full panels of providers in primary and specialty care, telehealth services are being designed to tackle healthcare access challenges in the emergency room (ER), rural health clinics, post-discharge home care – the full continuum, in one respondent’s words. Planned enhancements to current telehealth programs include self-management software, the addition of a transitional care model, and the development of dermatology, behavioral health, dentistry, and high-risk population solutions.

But other areas stayed the same: videoconferencing remained a leading technology in both 2013 (59 percent) and 2015 (48 percent), and congestive heart failure (53 percent in 2013, 57 percent in 2015) and diabetes (50 percent in 2013, 58 percent in 2015) continued to be the top conditions or concerns targeted by telehealth programs.

Survey Highlights

�� Wearables, that series of devices including fitness trackers, smart clothing, pedometers and smart watches were being utilized by 26 percent of responding organizations.

�� To meet the expanding population targets for telehealth services, the survey documented telehealth use for homebound, severe behavioral, at-risk for falls, and high-utilizers for the first time.

�� Three fourths of respondents (74 percent) said they expect Medicare to add remote patient monitoring to its list of covered telehealth services in the next 12 months.

�� Of clinical applications for telehealth, remote monitoring jumped from 57 percent to 63 percent.

Program Components

�� The blood pressure monitor/cuff, a new telemedicine device added to our survey in 2015, was utilized by the majority of respondents (59 percent); the fall/monitor mat was also a new addition to the survey, and was utilized by 8 percent of respondents.

�� Self-care/self-management tools and e-mail reminders remained among top telehealth tools for patients and health plan members.

�� The use of telephonic advice lines decreased from 55 percent in 2013 to 26 percent in 2015.

�� Among quality metrics, telehealth has had the largest impact on medical errors, according to respondents.

�� The nurse case manager has primary responsibility for telehealth, according to 32 percent of respondents.

Results and ROI

�� Fifty percent of respondents say it’s too soon to tell what their ROI is from telehealth programs.

�� Thirty-five percent of respondents said that bed days was the utilization metric most impacted by telehealth programs.

�� Nearly 60 percent of respondents said they are reimbursed for telehealth from private payors.

Successful Telehealth ApplicationsHere is a sampling of telehealth successes shared by survey respondents:

�� “Teleradiology, telepsychiatry and teleophthalmology. All the specialties were applied successfully resulting in high patient care, safety and profitability.”

�� “Post-discharge monitoring.”�� “Early recognition of potentially life-threatening situations increased safer

deliveries for mom and baby.”�� “Decreased medical emergency and greater satisfaction of primary care and

specialists and patients.”�� “Medication adherence to reduce admissions; have achieved as much as 80%

reduction in inpatient days.”�� “Congestive heart failure (CHF) remote monitoring: all inpatients receive a follow-

up phone call to assess any prescription needs and questions regarding home care plan, diagnosis, etc.”

About the SurveyThe 2015 Telehealth & Telemedicine survey was administered in March 2015 via the Healthcare Intelligence Network Web site at http://www.hin.com. Respondents were invited to take the survey via e-mail, e-newsletter and social networking reminders throughout the month. A total of 116 healthcare companies answered the survey, which asked 25 questions on telehealth and telemedicine, with multiple responses possible on some questions. Some questions were open-ended, inviting write-in responses. Not all surveys were fully completed. Data is qualitative, with results compiled by the Healthcare Intelligence Network.

Respondent Demographics

Responses to the March 2015 survey on telehealth and telemedicine were submitted by 116 organizations. Of 63 respondents identifying their organization type, 16 percent were hospitals, 11 percent were physician organizations, employers, and health plans, 6 percent were home healthcare, 5 percent were disease management, and 33 percent categorized
