We do this in partnership with the Victorian Department of Health and Human Services, Federal, State and Local Governments and our community members. We would particularly like to take this opportunity to acknowledge and thank the Phillip Island Medical and Health Action Group (PIMHAG), Bass Coast Health’s Community Advisory Committee (CAC) and the Bass Coast Health Phillip Island Community Advisory Group (PICAG) for partnering with us to develop the vision for our health care services.

The Phillip Island Health Hub provides an exciting opportunity to bring services to Phillip Island, and we look forward to working with the local community to determine how we should best grow services into the years ahead. To that end, we would like to acknowledge the collaboration between the Cowes Medical Centre, Ambulance Victoria and the Gippsland Primary Health Network and let you know that we are continuing to work together to look at how we can expand after hours services on the Island, in a way that is safe and contemporary.

This booklet has been designed to provide you with a snapshot of some of the healthcare services available to people who live, work and play on the Island. In particular, we are pleased to be able to provide you with information

about the services that Bass Coast Health provides from our three key sites. In case you didn’t know, these are:

1. Wonthaggi Hospital, which provides an accredited Emergency Department, inpatient beds, surgery, maternity, residential care, specialist aged, rehabilitation and palliative care, and a range of ambulatory services, including specialist consulting suites

2. Our San Remo site, which provides residential aged care and a range of specialist ambulatory services, including community nursing

3. Our Phillip Island Health Hub at Cowes, which provides a range of ambulatory services and specialist consulting services.

There are a range of other services available to assist you in maintaining good health, and support you in times of sub-optimal health. This booklet provides you with a snapshot of some of these services. We know that the provision of health care services on the Island is important to you, and we look forward to working with you to better deliver services to Phillip Island over the coming years. A range of routine health services are available on the Island providing day-to-day health care by health care professionals.

COWES

The Cowes Medical Centre and Grandview Family Clinic are open 365 days a year, until 10pm every evening. The clinics offer GP services, minor surgical services, allied health and visiting specialist consultations. To see the full range of services the clinic offers, visit www.cowesmedicalcentre.com.au/services. Appointments made Monday to Friday until 5.45pm are bulk billed or privately billed, depending on the Doctor you see. Appointments made between 6.30pm and 10pm are privately billed, however concession rates are available for concession card holders.

Cowes Medical Centre is located at 164 Thompson Ave, Cowes. Grandview Family Clinic is located at Grandview Grove, Cowes.

SAN REMO

The San Remo Medical Centre provides consulting sessions Monday to Friday from 9am to 5pm with its team of medical and nursing staff offering a range of medical and support services. To see the full range of services provided, visit www.sanremomedicalclinic.com.au/services.

San Remo Medical Centre is located at 123 Marine Pde, San Remo.

The Cowes Medical Centre runs an after-hours clinic available between 6.30pm and 10pm every evening which is privately billed. A GP Connect phone service operates after 10pm every night which provides direct phone access to the on-call Doctor. To access the After Hours GP Service, call 5951 1800.

COWES

Phillip Island Pharmacy (Terry White Chemmart) offers a range of services including blood pressure checks, flu vaccinations, hearing clinics, diabetes medication checks, asthma management advice, bowel screening, health checks and medication reviews. Services available to San Remo and Phillip IslandOur DISTRICT & PALLIATIVE CARE NURSING SERVICE provides a range of specialist nursing services for people in their own homes to support their independence and improve their health and wellbeing. Our Palliative Care Service focuses on supporting client comfort and independence. For more information please call 03 5671 9219.

Our HOSPITAL IN THE HOME (HITH) service provides acutely unwell patients with care in the comfort of their own homes. Patients are under the care of Bass Coast Health’s experienced GP’s and Registered Nurses. Our HITH Nurses deliver a range of services that allow people to return home from hospital as early as possible and still receive professional health care. Our SPECIALIST NURSING SERVICES include Stomal Therapy, for people who are anticipating or have had surgery resulting in the creation of an abdominal stoma; Diabetes Nurses who provide education and support to assist people to self-manage their condition; Respiratory Nurses who offer education and support to people who have respiratory diseases, including asthma. Our MCGRATH BREAST CARE NURSE specialises in breast cancer care and will provide support, information and care to women from the time of diagnosis throughout their cancer experience. This includes support through treatments such as chemotherapy, radiation therapy and surgery, and with other services such as counselling and follow-up services. Our CHRONIC DISEASE MANAGEMENT SERVICE is available for people who have chronic conditions such as asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke. Bass Coast Health’s Phillip Island Health Hub opened in April 2018 as a result of funding from the Commonwealth and State Governments, the Warley Trust and the former Bass Coast Community Health Service. Significantly, land for this development was provided by the Bass Coast Shire Council.

The PAEDIATRIC ALLIED HEALTH SERVICE is available to children who have a mild to moderate developmental delay. Our Paediatric Team consists of Occupational Therapists, Physiotherapists, Speech Pathologists and Dietitians, and counselling services may also be of assistance. As one of the three pillars of the health care system, ALLIED HEALTH professionals provide a range of rehabilitative and restorative services. Assessment and treatment will be provided by Occupational Therapists, Physiotherapists and Speech Pathologists. In addition to this, Podiatry and Dietetic services are also available. GENERAL REHABILITATION is provided by a multidisciplinary team to assist people to return to their daily activities following illness, injury, surgery or a hospital stay.

LYMPHOEDEMA SERVICES assist with assessing and managing swelling in one or more parts of the body due to the accumulation of excessive amounts of fluid. The service is provided by a specially trained Physiotherapist, who can provide early education and tips on skin care and exercise, in addition to providing treatments such as manual lymphatic drainage and compression therapy. Our FALLS PREVENTION PROGRAM works with people who are at risk of falling to improve their balance and decrease their risk of falling. People with poor vision, lower extremity injury, those with a history of falling and some older adults can improve their safety and increase their confidence with specific balance and coordination exercises to help them avoid falling or injuring themselves. One of the uses of our new Therapy Garden at the Phillip Island Health Hub is to help our clinicians assess how well people navigate different levels and surfaces. Our CARDIAC REHABILITATION PROGRAM provides exercises and education for clients who have experienced a cardiac episode. This program is conducted by a multidisciplinary team who help clients to reach their goals and regain endurance to reach their optimal level of activity and confidence in a supported way. Our PULMONARY REHABILITATION PROGRAM is an exercise and education program involving expert input from a multidisciplinary team. The program aims to help people to reach their optimal level of activity, maintain a healthy lifestyle, manage their respiratory condition and build confidence so that they can return to their regular activities. COUNSELLING SERVICES provide specialised support to people in the community who are experiencing emotional and psychological problems.

GENERALIST COUNSELLING provides support for people who are isolated or needing assistance with Family Court matters, relationships or parenting. FAMILY VIOLENCE may be physical, psychological, financial, sexual or emotional in nature and may be perpetrated by current or ex-partners, adolescent children, parents or siblings. YOUTH COUNSELLING service provides counselling to younger people, including support to secondary schools and youth access clinics.

The SEXUAL ASSAULT COUNSELLING AND SUPPORT PROGRAM provides counselling, information and support to women, children and men who are survivors of sexual assault. Support is also available to non-offending family members, partners and friends.

Our MENTAL HEALTH COUNSELLING SERVICE provides specialised therapy for people with mild to moderate mental illness (e.g. depression or anxiety) under a GP Mental Health Plan. For more information please call 03 5951 2100.

ALCOHOL & OTHER DRUG (AOD) SERVICES provide counselling, recovery support; referrals to residential detox facilities and rehabilitation facilities; non-residential withdrawal support; Pharmacotherapy linkages for clients on opiate replacement, a Needle & Syringe Program and support for other clients affected by alcohol and other drug use. Our PODIATRY SERVICE deals with the prevention, diagnosis, treatment and rehabilitation of medical and surgical conditions of the feet. Treatments available include diabetic foot assessments and diabetic foot care, biomechanical assessment and orthotic therapy, wound care, paediatric assessments, general foot care, footwear/foot care education and nail surgery.

Bass Coast Health delivers a broad and expanding range of services from Wonthaggi Hospital to service the health needs of Bass Coast Shire’s residents and visitors.

MATERNITY SERVICES offer ante-natal, birthing and post-natal care for women with low-risk pregnancies.

MIDWIVES work in collaboration with visiting GP Obstetricians to provide all women with information and support on how to care for themselves and their baby. For more information please call 03 5671 3201.

Our SURGICAL SERVICES include two operating theatres, one post anaesthetic recovery room and one day surgery recovery room. Surgical procedures include dental; ear, nose and throat; endoscopy; general surgeries, including colorectal; obstetric and gynaecological; ophthalmic; orthopaedic; urology and plastic surgery. Our Day Surgery room accommodates patients undergoing epidural injections, drug infusions and blood transfusions.

INPATIENT ACUTE WARD is a 30 bed medical and surgical unit for people with complex health care and social needs. Patients and families are actively involved in planning their hospital care and discharge arrangements. For more information please call 03 5671 3333.

SUB ACUTE INPATIENT SERVICES offer slow stream rehabilitation for older people with chronic or complex conditions associated with ageing, cognitive dysfunction, chronic illness or disability. Two other areas of focus in this unit are INPATIENT PALLIATIVE CARE, which aims to assist people with pain management or end of life care and SPECIALIST REHABILITATIVE CARE to enable people to return to full function. Bass Coast Health has two RESIDENTIAL AGED CARE facilities. Kirrak House is a 30 bed facility located adjacent to the Wonthaggi Hospital, and Griffiths Point Lodge, which is a 29 bed facility situated in San Remo overlooking Cape Woolamai. Our HAEMODIALYSIS SERVICE runs Mondays to Saturdays. This service aims to maintain patients with end-stage renal failure by using an artificial kidney machine to clean the blood and return it to the patient. For more information please call 03 5671 3309.

Our DENTAL CLINIC delivers oral health care to a broad range of people within the Bass Coast Shire and South Gippsland Shire. Dental services are available for all children from birth to 12 years of age. These services are free for children who are concession card holders or dependents of concession card holders. Emergency ServicesAmbulance VictoriaCalling 000 means you can provide information on your condition to healthcare professionals straight away. This means they can help you to access the help you need in the quickest, safest way possible.

USING AN AMBULANCE

Ambulances are for life-threatening and serious medical emergencies. Each week Ambulance Victoria (AV) receive hundreds of calls from people whose condition is not serious.

In many of these cases, rather than send an emergency ambulance, AV links these patients with a more appropriate service for their condition. Paramedics are highly skilled and are trained to provide life-saving and emergency treatments. The priority is to ensure these paramedics are available to respond rapidly to patients who require life-threatening and emergency treatment.

In a medical emergency, calling for an ambulance could mean the difference between life and death. People sometimes hesitate to call because they are not sure if the situation qualifies as an emergency. If in doubt, always call triple zero (000). The people who take your call are trained to help you and will direct you to more appropriate resources. This may be seeing their local doctor, visiting a pharmacy, self-treating their condition or in some cases making their own way to hospital.

RECOGNISING A MEDICAL EMERGENCY

An immediate life-threatening emergency needing an ambulance may include:• chest pain or chest tightness• sudden numbness or paralysis of the face, arm or leg• large burns• serious accidents or trauma• severe bleeding• difficulty breathing• extreme pain• unconsciousness.

HOW TO CALL AN AMBULANCE

If you or the person you are caring for needs immediate surgery or life-saving medical assistance, call triple zero (000) and ask for an ambulance to get emergency help as soon as possible, for a greater chance of recovery.

You should:

• ask someone else to make the call if you are providing first aid to the injured person

• call out for help if you are by yourself

• briefly leave the sick or injured person to call for an ambulance if no one responds to your call for help

• try not to panic. Talk slowly and clearly so the operator can understand you.

WHEN YOU CALL AN AMBULANCE

When you call triple zero (000), you will be asked which service you require (ambulance, police or fire rescue). Tell the operator that you need an ambulance. They will transfer you to an ambulance call-taker.

Give your phone number and the address where the injured or sick person is. If you are located on a country property, you will need to know your property fire map reference, or VicRoads map reference.

Describe the problem and what has happened, the operator may send an ambulance while you are talking on the phone, even though you may not hear them do this.

The operator may give you first aid instructions over the phone. Do not hang up until the phone operator tells you to.

Be prepared for when the ambulance arrives. Make sure outside lights are on and doors are unlocked so they can access your house.
